DISCONTINUED CODES- NOT TO BE USED AFTER 12/31/16: Drugs/Vaccines: J0760 J1590 Medicine: Radiology: A9544 A9545 Surgery: NEW CODES- EFFECTIVE 1/1/2017: Drugs/Vaccines: Description Influenza virus vaccine quadrivalent (cciiv4), derived from cell cultures, subunit,preservative and antibiotic free, 0.5 ml dosage, for IM use J9034 Injection,bendamustine hcl (bendeka), 1 mg Medicine Fluorescein angiography and indocyanine-green angiography (includes multiframe imaging) performed at the same patient encounter with interpretation and report, unilateral or bilateral Moderate sedation services provided by the same physician or other qualified healthcare professional performing the diagnositic or therapeutic service that the sedation supports, requiring the presence of an independent trained observer to assist in the monitoring of the patient s level of consciousness and physiological status; initial 15 minutes of intraservice time, patient younger than 5 years of age Initial 15 minutes of intraservice tiem, patient age 5 years or older each additional 15 minutes of intraservice time Moderate sedation services provided by a physician or other qualified
