1 ICD- to ICD- Crosswalk Adult Codes On October 1, 2015, the Centers for Medicare & Medicaid Services (CMS) transitioned to the new International Classification of Diseases, th Revision System (ICD-), which replaced the ICD- diagnosis coding system. The following document is provided as a resource for occupational therapy practitioners to translate ICD- codes to their related ICD- codes. The codes provided represent the most commonly used diagnosis codes for the adult population. This is not a comprehensive or fully inclusive list of occupational therapy ICD- codes, and you should refer to official billing and coding resources or your third-party payers for final coding determinations as some categories of codes may be more extensive, contain additional instructional notes, or require coding to a higher degree of specificity than indicated.
