Limited information is available on long-term outcomes for patients with unprotected LMCA disease who underwent coronary stent procedure or coronary artery bypass grafting (). Methods We evaluated 2,240 patients with unprotected LMCA disease who received coronary stents (n 1,102; 318 with bare-metal stents and 784 with drug-eluting stents) or underwent (n 1,138) between 2000 and 2006 and for whom complete follow-up data were available for at least 3 to 9 years (median 5.2 years). The 5-year adverse outcomes (death; a composite outcome of death, Q-wave myocardial infarction [MI], or stroke; and target vessel revascularization [TVR]) were compared with the use of the inverse probability of treatment weighted method and propensity-score matching. Results Conclusions After adjustment for differences in baseline risk factors with the inverse probability of treatment weighting, the 5-year risk of death (hazard ratio [HR]: 1.13; 95% confidence interval [CI]: 0.88 to 1.44, p 0.35) and the combined risk of death, Q-wave MI, or stroke (HR: 1.07; 95% CI: 0.84 to 1.37, p 0.59) were not significantly different for patients undergoing stenting versus. The risk of TVR was significantly higher in the stenting group than in the group (HR: 5.11; 95% CI: 3.52 to 7.42, p 0.001). Similar results were obtained in comparisons of bare-metal stent with concurrent and of drug-eluting stent with concurrent. In further analysis with propensity-score matching, overall findings were consistent. During 5-year follow-up, stenting showed similar rates of mortality and of the composite of death, Q-wave MI, or stroke but higher rates of TVR as compared with for patients with unprotected LMCA disease. Several studies have suggested that coronary stenting has been shown to be feasible for patients with unprotected LMCA stenosis (3). In addition, the application of PCI to LMCA disease has increased further with the availability of drug-eluting stents (DES) that significantly reduce the rates of restenosis and repeat revascularization (4 6). However, limited data exist regarding the long-term outcomes of coronary stenting, as compared with standard, for these patients. Furthermore, the long-term safety of DES has been questioned by recent reports suggesting increased risk of late stent thrombosis, mortality, or myocardial infarction (MI) (7,8). Therefore, very-long-term follow-up after DES implantation in a large patient cohort with LMCA disease is important. The MAIN-COMPARE (Revascularization for Unprotected Left Main Coronary Artery Stenosis: Comparison of Percutaneous Coronary Angioplasty Versus Surgical Revascularization) registry was designed to evaluate the real-world outcomes of coronary stenting and for unprotected LMCA disease in multiple centers of Korea, and the median 3-year comparative outcomes were previously reported (9). To obtain a more reliable long-term treatment effect of stenting or, we have now extended the follow-up duration for the study patients, for whom follow-up data were available for at least 3 years and up to 9 years. Methods Study population. The MAIN-COMPARE study enrolled patients with unprotected LMCA stenosis who underwent either or PCI as the index procedure at 12 Dr. Seng-Wook Park has received research support from Medtronic. The LMCA was considered unprotected if there were no patent grafts to the left anterior descending artery or circumflex artery. Patients who had prior, those who underwent concomitant valvular or aortic surgery, and those who had an ST-segment elevation MI or presented with cardiogenic shock were excluded. In this analysis, the follow-up period extended through October 30, 2009, to ensure that all patients had at least 3 years and approximately up to 9 years of follow-up information. From January 2000 through May 2003, coronary stenting was performed exclusively with bare-metal stents (BMS), whereas from May 2003 through June 2006, DES were used exclusively. Therefore, the relative treatment effects were evaluated in the overall cohort, pre-des cohort (Wave 1 of the registry; BMS vs. concurrent between January 2000 and May 2003), and post-des cohort (Wave 2 of the registry; DES vs. concurrent between May 2003 and June 2006). The local ethics committee at each hospital approved the use of clinical data for this study, and all patients provided written informed consent. Revascularization procedures. Patients underwent PCI, instead of, because of either the patient s or physician s preference or the high risk associated with. Methods of stent implantation for patients with LMCA disease have been described previously (10,11). All procedures were performed with standard interventional techniques. The use of pre-dilation, intra-aortic balloon pump, or intravascular ultrasound and the choice of the specific type of DES was at the operator s discretion. Antiplatelet therapy and periprocedural anticoagulation followed standard regimens. After the procedure, aspirin was continued indefinitely. Patients treated with BMS were prescribed ticlopidine (250 mg twice/day) for at least 1 month, and patients treated with DES were prescribed clopidogrel (75 mg once/day) for at least 6 months, regardless of DES type. Treatment beyond this duration was at the discretion of the physician. Surgical revascularization was performed with the use of standard bypass techniques (12). Complete revascularization was performed when possible with arterial conduits or saphenous vein grafts. Study outcomes and follow-up. The end points of the study were death; the composite of death, Q-wave MI, or stroke; and target vessel revascularization (TVR). Death was defined as death from any cause. Q-wave MI was defined as documentation of a new abnormal Q-wave after the index revascularization. Stroke, as indicated by neurologic deficits, was confirmed by a neurologist on the basis of imaging studies. A TVR was defined as any repeat revascularization in any left anterior descending artery or left circumflex artery as well as in the target segment. In the PCI group, stent thrombosis was defined as the definite occurrence of a thrombotic event, according to the Academic Research Consortium classification (13). All outcomes of interest were confirmed by source documentation collected at each

Clinical, angiographic, procedural or operative, and outcome data were recorded in the dedicated PCI and surgical databases by independent research personnel. For validation of complete follow-up data, information about vital status was obtained from the National Population Registry of the Korea National Statistical Office with the use of a unique personal identification number. Routine angiographic follow-up was recommended for all PCI patients at 6 to 10 months or earlier if clinically indicated. For patients, angiographic follow-up was recommended only if there were ischemic symptoms or signs during follow-up. Statistical analysis. Continuous variables were compared with the t test or Wilcoxon rank sum tests, and categorical variables were compared with the chi-square statistics or Fisher exact test, as appropriate. Survival curves were constructed with Kaplan-Meier estimates and compared with the log-rank test. With that technique, weights for patients receiving were the inverse of (1 propensity score), and weights for patients receiving stenting were the inverse of propensity score. The propensity scores were estimated without regard to outcomes, with multiple logistic-regression analysis. A full nonparsimonious model was developed that included all variables shown in Table 1. Model discrimination was assessed with c-statistics, and model calibration was assessed with Hosmer-Lemeshow statistics. For each comparison (the entire cohort, Wave 1, and Wave 2), a separate propensity score for PCI versus was derived. In addition, we also compared outcomes with the use of propensity-score matching in the overall cohort and in separate subgroups according to type of stent (15). Propensity matching was done with the Greedy algorithm (16), and comparisons were completed with Cox regression models, with robust standard errors that accounted for the clustering of matched pairs. The details of the propensity-score matching and analytic methods have been described previously (9). All reported p values are 2-sided, and p values of 0.05 were considered to indicate statistical significance. The SAS software, version 9.1 (SAS Institute, Cary, North Carolina), and the R programming language were used for statistical analyses. Results Patient characteristics. Between January 2000 and June 2006, a total of 2,240 patients with unprotected LMCA disease were enrolled; 1,102 patients were treated with PCI with stenting (318 with BMS, and 784 with DES), and 1,138 were treated with. Of PCI patients, 1,073 (97%) had clinical and angiographic conditions that made them eligible for either PCI or, but they underwent PCI because of the patient s or physician s preference. The remaining 29 patients (3%) had comorbidities ineligible for surgery due to old age with poor performance status, limited life expectancy, concurrent severe medical illness, or poor anatomic conditions. Procedural characteristics of the patients in the MAIN- COMPARE registry have been described previously (9), and selected features are: 1) among DES patients, 77% received sirolimus-eluting stents, and 23% received paclitaxel-eluting stents; 2) the mean number of stents implanted in left main coronary lesions and per-patient (including left main and other vessels) was and , respectively; 3) among patients, 42% underwent off-pump surgery; and 4) 98% underwent revascularization of the left anterior descending artery with an arterial conduit. The baseline characteristics of the study patients are shown in Table 1. The patients had a higher-risk clinical and angiographic profile than PCI patients. Follow-up and outcomes. The median follow-up was 62.0 months (interquartile range 48.0 to 78.3 months) in the overall patients. During overall follow-up, 328 patients (14.6%) died, of whom 221 (9.9%) died of a cardiovascular cause. A total of 22 (1.0%) suffered a Q-wave MI, and 41 (1.8%) suffered a stroke. TVR was performed in 218 (9.7%). During 5 years of follow-up, the observed (unadjusted) event-free survival and crude relative risk according to treatment approach are presented in Figure 1 and Table 2. Details regarding the relative risk of stenting according to type of stent versus concurrent are also presented (Wave 1 in Fig. 2, and Wave 2 in Fig. 3). There was a trend toward lower rates of death and significantly lower incidence of the composite of death, Q-wave MI, or stroke in the PCI group than in the group, whereas the rate of TVR was significantly higher in the PCI group. This trend was more prominent in comparison of DES with concurrent. In 784 patients who received DES, 11 patients had definite stent thrombosis. Among them, 1 patient had acute, 3 had subacute, 3 had late (201, 289, and 350 days after the procedure, respectively), and 4 had very late stent thrombosis (638, 724, 803, and 1,077 days after the procedure, respectively). At 5-year follow-up, the cumulative incidence of definite stent thrombosis associated with DES was 1.5%. After adjustment of baseline covariates with the inverseprobability-of-treatment weighting, the 5-year risks of death and the composite of death, Q-wave MI, or stroke were similar in the 2 groups (Table 2). However, the adjusted risk of TVR was significantly higher in the PCI group than in the group. Similarly, in patients classified by type of stent, overall results were consistent for Waves 1 and 2. After performing propensity-score matching in the entire population, a total of 542 matched pairs of patients were created; 207 matched pairs of patients with BMS and concurrent control subjects in the Wave 1, and 396 matched pairs of patients with DES and concurrent control subjects in the Wave 2. In propensity-matching methods, there was no significant difference in 5-year rates of death and a composite of serious outcomes (death, Q-wave MI, or stroke) between the 2 groups (Table 3). However, the 5-year rate of TVR was consistently higher in the PCI group than in the group. Discussion In this large, multicenter cohort of consecutive patients with unprotected LMCA disease, there was no significant difference in the risks of death and a composite outcome of death, Q-wave MI, or stroke between the PCI and the groups during 5 years of follow-up. These results were consistent when BMS or DES were compared with concurrent. By contrast, the rate of TVR was significantly lower in the group than in the PCI group, without regard to type of stent. Several observational studies and LMCA subsets in the SYNTAX (Synergy between PCI with Taxus and Cardiac Surgery) trial indicated that safety outcomes (death or composite of death, MI, or stroke) were comparable between the PCI and the group, but the advantage of consists primarily of fewer repeat revascularizations (9,17 25). Although observed findings from subgroup analysis should be interpreted with caution and the interpretation must be speculative, PCI seemed to be even safer than in more simple anatomic situations (i.e., lower SYNTAX score tertiles) (26). In addition, a significantly higher rate of follow-up angiography in the PCI cohort due to systematic protocol-driven follow-up in our study (73.0% vs. 14.6% in the cohort, p 0.001) would inherently bias the PCI population toward TVR, which is the major difference between the 2 groups. However, these early studies with short follow-up periods could

DES drug-eluting stent(s); other abbreviations as in Figure 1. have penalized the group, because the long-term benefits of bypass surgery over PCI in other settings have not typically been fully evident until 1 to 3 years after the procedure (27). The present study extends previous findings to a longer-term follow-up and demonstrates that overall findings were unchanged between 3 and 5 years of follow-up (9). We noted, among patients with unprotected LMCA disease who received DES, a 5-year cumulative stent thrombosis incidence of 1.5%, similar to those reported in several

It provides further evidence that LMCA stenting with DES results in lower or, at worst, similar rates of stent thrombosis than rates reported among patients with other coronary lesions in routine clinical practice (7). Because of the narrow margin for error, interventional cardiologists undertaking PCI of LMCA lesions should be experienced and backed by highly competent support from cardiac surgeons. The particulars of clinical practice as well as the specific expertise of the interventional cardiologists and the surgical techniques of the cardiac surgeons in the participating centers might have contributed to our results (31,32). It might potentially limit the reproducibility of these results in other institutional settings and practitioners. Study limitations. First, the inherent limitations of a nonrandomized registry study should be acknowledged. Despite appropriate statistical adjustments, unknown confounders might have affected the results. Therefore, our findings should be confirmed or refuted through large randomized clinical trials with long-term follow-up, such as the PRECOMBAT (Randomized Comparison of Bypass Surgery Versus Angioplasty Using Sirolimus-Eluting Stent in Patients With Left Main Coronary Artery Disease) or EXCEL (Evaluation of Xience Prime versus Coronary Artery Bypass Surgery for Effectiveness of Left Main Revascularization) clinical trials (3). Second, our analysis was still underpowered to detect significant differences in mortality and serious composite outcomes. Third, our study lacks detailed information about the burden of atherosclerotic disease and anatomic complexity, such as the SYNTAX score. Finally, because this study evaluated BMS and the first-generation of DES, the direct application of these findings to the next generation of DES might be limited. Conclusions For patients with unprotected LMCA disease, PCI with stenting and were associated with similar long-term (5-year) rates of death and the composite end point of death, Q-wave MI, or stroke. Rates of repeat revascularization were still higher among patients who underwent PCI than among those who underwent. A large randomized comparison study with will provide more confidence in the long-term safety, durability, and efficacy of PCI with DES.
