1 Vascular Surgery

2 Vascular surgery is a specialty that deals with diseases of the vascular system (i.e. arteries, veins and lymphatics). These are managed by medical therapy, interventional procedures, and surgical reconstruction. Of all cases of vascular diseases resulting in limb amputation, at least 80% are avoidable, if patients had been treated by a Vascular Surgeon. Majority of amputees are diabetics. At least 40% of people with decade-long diabetes develop vascular problems. In a country where 25% of the population is believed to have diabetes, the number of people estimated of having vascular problems is significant. Almost 50% of diabetics, who go in for amputation of a limb, lose their second limb in another year. Vascular disease is an indicator of heart diseases as well.

3 Patients who have undergone Coronary Stenting or Bypass Surgery are also at a risk of developing Peripheral Vascular Disease. Today, with advances in technology, arterial and venous diseases are treated by Angioplasty, Stenting, Sclerotherapy and Endovenous Laser Treatment. These newer procedures provide reasonable outcomes that are comparable to surgery with the advantage of short hospital stay (day or overnight for most cases) with lower morbidity and mortality rates. Not all patients are candidates for interventional procedures, which in any case do not replace surgery. The durability of endovascular arterial procedures is generally good.

4 The scope of vascular interventions encompasses a broad spectrum of diseases and conditions such as: Aneurysms (Ballooning of the artery) The prevalence among Asians is less than that of Caucasians. The common locations are in the stomach area (abdominal aorta) and behind the knee. As the size of an aneurysm increases, there is an increased risk of rupture, which can result in severe hemorrhage and death. If discovered, the aneurysms can be treated with endovascular or open methods. Limb Ischemia A reduction in blood supply to the upper or lower limb can have serious consequences. 1. Acute Limb Ischemia - Sudden onset of cold and painful limb, that are normally treated by: - Thrombolysis - Embolectomies - Anti-coagulation 2. Chronic Limb Ischemia - Pain on walking, ulcers, discolouration of toes, diabetic foot ulcers can be treated by: - Stenting or bypass procedures Mesenteric Ischemia - weight loss, pain after eating and fear of food: - Needs angiographic investigations for possible eventual surgery Extra-cranial Cerebrovascular Disease can be treated by: - Carotid Endarterectomy and other carotid surgery (surgery to prevent stroke)

5 Venous Diseases are mostly: - Varicose veins (see treatment above). - Deep Vein Thrombosis (DVT): treated by anticoagulation. - Venous Malformations: treated normally by embolization. Lymphatic Disease Lymphoedema, or lymphatic obstruction, is a condition of localized fluid collection in the legs. This condition increases significantly the risk of infection. There are many dysfunctions related to diseases of the blood vessels. Hypertension is the most important cause of such conditions. Continued hypertension permanently changes the architecture of the blood vessels making them stiffer and narrower. Arterial diseases primarily affect people who are elderly or have a history of diabetes, smoking, high cholesterol, stroke or ischemic heart disease.
