Now days the term episiotomy comes into consideration because the perineal tears can gives many complications, like infection, sepsis. But healing becomes problematic even after suturing (s the vaginal PH becomes alkaline in post partum period acc. to modern science) there may be soiling by fecal matter & it gives complications. s the modern science get advanced with new antibiotic, the resistance to various antibiotic increases s episiotomy wound can be considered as Sadya vrana, Ropan karma should be done on that Sadya Vrana [3]. The present study entitled, To study the efficacy of panchavalkalsiddha ghruta [3] application compared with betadine ointment in yonigata sadya vrana during prakrut prasava (i.e. episiotomy) and aims & objectives will be decided according to present study. In present study Panchavalkalsiddha Ghruta used as a trial drug for this study. The clinical study was conducted on 8 patients which selected randomly and divided in 2 groups 1 st is Experimental group treated with Panchavalkalasiddha goghruta lepa which will be applied on wound. 2 nd is control group treated with application of Betadine ointment. The clinical assessment was done on the basis of grading criteria with specific symptomology of patients like Pain, Swelling, Discharge, Edges, and Smell of sadya vrana. ccording to observations and results conclusion has been done. It can be conclude that patient applying Panchavalkalasiddha goghruta lepa in yonigata sadya vrana during prakrut prasava (i.e. episiotomy) is quite effective than that of application of Betadine ointment reducing the Pain, Swelling, Discharge, Edges, and Smell of sadya vrana. From all ancient literatures we came across conclusion that Prasuti Shastra & Balarog Vigyana was very developed branch at that era. Though medical science has achieved new milestones by the time, NORML LBOUR i.e. EUTOSI [1] is still prime topic in obstetrics over centuries and will remain there so. The main peculiarities of normal labour are 1. the most common presenting part of baby is head which is largest of body parts in proportion and the engaging diameter of birth canal hardly allows it for passage. 2. During uterine contraction, the head compresses the perineum with great force.many of times perineum fails to withstand this force and results into lacerations of varying degree. That is why active management is required during process of vaginal labour. Maharshi Sushruta has told three stages of Prasava as: The properties of both Panchavalkala choorna and ghruta We have decided to study the healing properties of the Ghruta in Yonigat Vrana caused during Prasav [3]. Reason for Selection of Topic s seen later, the Yonigat Vrana is difficult to heal as it has Stree vishista Marma called Sevani. 4 through yurvedic views, such as lakshanas of Ruhyaman, Ruddha & Suddha Vrana. The study is divided into two groups: 1) Experimental group- Group of 30 patients in which wounds will be treated with Panchavalkala churna goghuta lepa which will be applied on wound. 2) Control group- Group of 30 patients in which routine care of episiotomy wound will be taken with application of Betadine ointment. Route of drug administration-local application Matra-s per wound size Duration-7days Site-perineal skin at wound Plan of work 1) The patient of any age,caste, parity who have undergone episiotomy will be included in study. 2) There will be 2 group of 30 patient each,first group will be treated with Panchavalkala siddha Ghruta & control group with Betadine ointment. 3) Lakshna will be categorized in grade I, II, III. 4) Data will be collected with the help of observation & assessment criteria. 5) Discussion will be made scientifically in detail after observation. 6) Result will be drawn on the basis of observations & discussion. Selection criteria ) Inclusion criteria 1) The patient of any age, caste, parity, income group & any occupation will be selected. 2) Patient who has undergone episiotomy will be selected. B) Exclusion criteria 1) Patient having blood coagulopathy. 2) Patient of fever, vulval edema, any systemic disorder like P.I.H., D.M., severe anemia, in which Hb% is less than 6% 3) Perineal tear with injury to rectum or urethra. 4) Immunosuppressed patient will be excluded. 5) Patients with perineal lesions like boils, abscess, hematoma, etc. 6) Patients with severe adverse effect. SYMPTOM Panchavalkalsiddha Ghruta application reduces pain. PIN SWELLING - It reduces discharge and increases granulation tissue formation. DISCHRGE EDGES SMELL - There is no side effect, intolerance to Panchavalkalsiddha Ghruta applicayion. TEMPERTURE STTISTICL METHODOLOGY:- The statistical analysis will be done by representing the data in the form of diagram and graphs. t last to get valid conclusion, unpaired t test or a appropriate statistical test will be applied 6 Discussion It is concluded from the present study that Panchavalkalsiddha Ghruta pplication is useful in Pain, Swelling, Discharge, Edges, and Smell of sadya vrana. - The present study indicates that the treatment is safe, effective, harmless and comparatively cost effective.
