12 OVERALL OUTCOMES NICHD, secondary analysis of stillbirth study patients, 48 with marijuana exposure Composite adverse pregnancy outcome and neonatal morbidity/death No increase in composite adverse pregnancy outcome Composite neonatal morbidity /death increased (14.1% vs. 4.5%, p = 0.02) Infection morbidity(9.8% vs. 2.4%) Neurologic morbidity (1.4% vs. 0.3%) both significantly increased Metz, 2017 OVERALL OUTCOMES Systematic review and meta-analysis 31 observational studies, patients with marijuana use vs. 124,867 controls Unadjusted - increased risk of low birth weight (RR 1.43) and preterm birth (RR1.32) No increased risk of either when adjusted for tobacco use Not an independent risk factor for low birth weight or preterm birth Not an independent risk factor for composite adverse neonatal outcomes Could not account for amount of use Conner 2016 SO.WHAT DO I TELL MY PATIENT? Possible effects on fetal growth May increase stillbirth risk Possible increase in infections in newborns Most importantly, it affects receptors in the baby s brain that are still developing, and may be changed with marijuana exposure There is not enough data on long term effects for kids Bottom line.we just don t know enough yet! 12

16 MARIJUANA USE IN PREGNANCY AND LACTATION Screen all women for marijuana use If use is identified: Review potential health consequences if ongoing Encourage to discontinue if pregnant or planning pregnancy Offer referral to treatment if indicated Discontinue medical marijuana if pregnant or contemplating pregnancy Insufficient data on lactation and breastfeeding use is discouraged ACOG 2017 MARIJUANA USE IN PREGNANCY AND LACTATION All women screening for substance use disorder at entry to prenatal care Urine testing should NOT be used for primary screening 2-3 days occasional users Several weeks in chronic users ACOG 2017, Metz
17 Information regarding long-term effects remain insufficient to recommend complete abstention from breastfeeding initiation or continuation Occasional or rare use avoid further use or reduce as much as possible while breastfeeding Positive urine screen recommend discontinue exposure while breastfeeding A recommendation of abstaining from any marijuana use in warranted ABM Clinical Protocol #21, 2015 When advising mothers on the medicinal use of marijuana during lactation, one must take into careful consideration and counsel on the potential risks of exposure of marijuana and benefits of breastfeeding to the infant. At this time, although the data are not strong enough to recommend not breastfeeding with any marijuana use, we urge caution. ABM Clinical Protocol #21,

Dickson B 2018 FOOD FOR THOUGHT - Discriminatory drug testing and reporting Inconsistent education Breastfeeding during marijuana use Punitive or legal interventions related to use Community standards of care based on opinion rather than science FOOD FOR THOUGHT Audio-recorded patient-health care provider first prenatal visits at 5 urban outpatient clinics 468 first OB visits, 90 patients (19%) disclosed marijuana use to 47 health care providers 48% of visits, the provider did NOT respond to marijuana use disclosures or offer counseling When counseling was offered it did NOT include specific information on risks or outcomes related to use We must do better!! 22 FOOD FOR THOUGHT Drug prescription records in Medicaid programs in states with medical cannabis legalization million drug records Liang 2018 FOOD FOR THOUGHT Drug prescription records in Medicaid programs in states with medical cannabis legalization million drug records 29.6% reduction in Schedule III prescriptions 29.9% reduction in opioid dosage 28.8% reduction in related Medicaid spending Liang 2018 CONCLUSIONS Marijuana - not uncommon in pregnancy, with use and effects changing with changes in state legality and in concentrations of THC Universal screening for marijuana use during pregnancy/lactation is recommended If use is identified, counseling is recommended to review the lack of strong data on effects, current unknowns, and the recommendation to abstain from use during pregnancy and lactation Medical marijuana should be discontinued for alternative treatments during pregnancy and lactation Future studies are urgently needed to assess risks for mother, pregnancy, breastfeeding and neonatal/childhood outcomes 22
