1 COMMUNITY HEALTH NEEDS ASSESSMENT IMPLEMENTATION UPDATE 2015 Crittenton Hospital Medical Center Yearly Health s Assessment Update is here to show our progress made towards addressing our identified needs. FAMILY HEALTH ACTION PLANS Home Visits for Newborns Home Visits for Newborns Home visits are not feasible at this time. This action plan will address the need for home visits with a four stage process: pregnancy, post-partum, home, and family support. Employ a Maternal-Family Navigator Program that will guide and support the mother and family beginning in early pregnancy and extending support throughout parenthood. Provide education to families through a variety of opportunities: Consultations, group classes, phone conferences, lectures, website, distribution, written material and team collaboration... Increase participation in prenatal programs Increase number of families participating in support and education programs provided by CHMC. Physicians, Public Health, Great Parents Great Start, Oakland County Family Services, 2015 Outcome Update: Visits for Newborns/family support Ongoing support of the Parent Navigator role within the Health & department has resulted in an increase in participation of programs and referrals to outside agencies: Increased rate of contacting every new mom/family with additional staff. Increase in number of Breastfeeding consultations distribution added with an increase of hits to hospital website Adjusted Breastfeeding Café support group to meet the needs of moms. Provided Pregnancy & Parenting Support flyers to all OB offices Regularly Update Parent Resource links on the website and push education resources through social media and blogs. Increased referrals to Maternal-Infant support programs in the community.

2 Parenting Parenting Expand parenting education programs Increase participation in current programs Increase education through lectures, workshops, support groups, online learning forums, website resource links, Wellspring, and written material Increase number of families participating in parenting services and education programs Increase website visits to parenting links Increase Facebook and Twitter likes Physicians, Public Health, Great Parents Great Start, Oakland County Family Services, Local Childcare Facilities, University Nursing Programs, Wayne State University Family Practice Residents, Rochester Auburn Hills Coalition 2015 Outcome Update: Parenting Ongoing support of increased parenting programming and education has resulted in an increase in attendance of families participating in parenting services and programs through: Continued Mommy Monday Walking Group Participated in local school, community and church health fairs such as Kid Day in the Park (provided general health & wellness education for about 500 families). Participated in Back to School Bonanza ; hand hygiene and parenting education. Approximately 650 attendees. Puberty Classes offered quarterly; Mothers & Daughters. Breastfeeding Café provides support to mothers and their babies. Approximately 25 mothers attended. Held 4th Annual Baby Fair; collaborated with local fire departments, local businesses, physician groups, and local support groups. Approximately 150 attendees. Held 3 rd Annual Teen Health Expo with a focus on Healthy You ; collaboration with Rochester Auburn Hills Coalition, Rochester Counseling, Common Ground, Offered free Childbirth class for young mothers-to-be or mothers on Medicaid. Held 1 st Calling All Tweens event for children ages 9 through 12; 75 students attended.

3 MENTAL HEALTH ACTION PLANS Access to Substance Abuse Treatment: Alcohol, Drugs, Tobacco Access to Substance Abuse Treatment Alcohol, Drugs, Tobacco Expand community awareness of current support groups and programs Increase participation in Rochester Auburn Hills Coalition Expand health fairs and expo to include substance abuse education and resources for treatment Increase education through lectures, workshops, support groups, website resource links, Wellspring, and written material Increase participation in current support groups and programs Physicians, Public Health, Local Crisis Centers, Oakland County Family Services, Local Schools and Universities, Local Churches, Older Person Commission, 2015 Outcome Update: Access to Substance Abuse Treatment: Alcohol, Drugs, Tobacco Increased participation in programs and support groups through: Ongoing participation in the Rochester Auburn Hills Coalition; added participation in Avondale Youth Assistance. Freedom from Smoking Program for the community adults and teens. Information given to local student youth groups. Added additional resources to parent resources on our website. Held 3 rd Annual Teen Health Expo with a focus on Healthy You ; collaboration with Rochester Auburn Hills Coalition, Rochester Counseling, Common Ground, Support group attendee numbers have increased: Alcoholic Anonymous, Bipolar Support Group, Bipolar Caregiver Support Group, and Overeaters Anonymous. Member of the Energizing Connections for Healthier Oakland (ECHO) Steering Committee and Assessment Task Force created to identify community resources and gaps in services/support for Oakland County.

4 Suicide in Teens, Adults, and Seniors Suicide Increase community awareness and prevention through support groups and programs Increase participation in Rochester Auburn Hills Coalition Collaboration with community partners Explore opportunities for grant funding for future programs Increase education through lectures, workshops, support groups, website resource links, Wellspring, and written material Increase participation in current events and programs Physicians, Public Health, Local Crisis Centers, Oakland County Family Services, Local Schools and Universities, Local Churches, Older Person Commission, 2015 Outcome Update: Suicide in Teens, Adults, and Seniors Continued to increase awareness of current support groups; distributed written material, highlighted support groups and updated website. Continued participation in the Rochester Auburn Hills Coalition; representation at all regular meetings and member of the Executive Council. Held 3 rd Annual Teen Health Expo with a focus on Healthy You ; collaboration with Rochester Auburn Hills Coalition, Rochester Counseling, Common Ground, Member of the Energizing Connections for Healthier Oakland (ECHO) Steering Committee and Assessment Task Force created to identify community resources and gaps in services/support for Oakland County. Increased awareness and education through Social Media posts. Targeted stress topics through physician lectures and health/wellness fairs and events. Held Mental Health 1 st Aid Training workshops on 3/19/15 and 4/8/15. WSU Family Medicine resident paneled a Suicide Workshop on 9/17/15 at St. Mary Episcopal Church in Lake Orion.

Nutrition, exercise, and a variety of health screenings were available. Approximately 250 attendees. Promoted Individualized Diet Consultations through social media and website noting an increase in website hits. Offered more options for access to Diabetes and Nutrition Counseling Center. Held 3 rd Annual Teen Health Expo with a focus on Healthy You ; collaboration with Rochester Auburn Hills Coalition, Rochester Counseling, Common Ground, Participated in the Avondale School District Employee Wellness Committee providing education and support on healthy life style changes at employee wellness events. Servicing approximately 350 employees. Includes a monthly Lunch and Learn at the various schools with emphasis on Stress and Nutrition. Participated in the Rochester Schools employee wellness health fair servicing 175 employees. Offered Cardiovascular Physician Lecture Series; total 56 attendees.

6 Offered Health & Wellness Series; total 113 attendees. Provide low cost general health screening for those over 50 years of age monthly, 1 STOP screening includes several potentially life-changing assessments; blood pressure, body fat analysis, weight, health risk appraisal, peripheral vascular disease screening and lab work. Speakers Bureau provided topics to community/worksites; 239 attendees. Participated in community and worksite health fairs, providing general health and wellness information including fitness and nutrition: o Village of Rochester Hills Turn the Town Teal (250 attendees) o St. Paul s Episcopal Church (110 attendees) o Fiat Chrysler o Brooksie Way Women Event o St. Irenaeus o KID DAY in the Park March of 2015, provided week long colon cancer display table in front lobby of hospital with information, education and resources; approximately 75 people visited the table. Increased attendance in our Overeaters Anonymous support group noted by 150. Matter or Balance program instituted (exercise and fall prevention). Prevention class that increases ones activity levels. Diabetes for Pediatrics and Adults Diabetes Increase community awareness and prevention through support groups and programs Explore options for adding additional diabetes education support Increase education and promote healthy life styles through health fairs, lectures, workshops, support groups, website resource links, Wellspring, and written material. Increase participation in current events and programs Increase the number of programs that promote healthy life styles including weight loss and nutrition Physicians, Public Health, Local Schools and Universities, Local Churches, Older Person Commission, 2015 Outcome Update: Diabetes Increased,, and Prevention programs and increased participation through: All About Women Expo component with our Annual Breast Cancer Walk. Nutrition, exercise, and a variety of health screenings were available. Approximately 250 attendees.

7 Promoted Individualized Diet Consultations through social media and website, noting an increase in website hits. Held 3 rd Annual Teen Health Expo with a focus on Healthy You ; collaboration with Rochester Auburn Hills Coalition, Rochester Counseling, Common Ground, Diabetes awareness and education provided through worksite and church health fairs and summer 5K run. Crittenton/Rochester Schools School Nurse partnership provides nursing services to manage student medical action plans for the school district; this includes training, education and resources for school staff, parents, and children. Updated website for Diabetes and Nutrition Counseling. Include blog entries, contact us form, and risk assessment. Wellness Series Lecture: Diabetes: The Latest Updates in Treatment. Lecture held at hospital and OPC with 12 attendees each.
