Samaritan Health Services 2 Improving community health is at the core of our mission As we complete these activities each year, we believe it s important to report back to the community regarding progress that has been made in each goal area. On the following pages, you ll find success stories as well as data that will help illustrate how we re partnering with many others in our region to build healthier communities together. Sincerely, In 2017, Samaritan invested more than $156 million in community health activities such as services for low-income individuals, free health screenings, health-related research, training for health professionals and grants to local non-profits in support of health initiatives. These investments are designed to help address priorities and gaps as identified through hospital Community Health Needs Assessments and the resulting Community Benefit Implementation Plans. The current focus of our investments is in the following priority areas: Healthy families Greater access Better networks Healthy kids Healthy teens Healthy seniors Doug Boysen, JD, MHA President & Chief Executive Officer Samaritan Health Services Watch a video and learn more at samhealth.org/impact.

Samaritan Health Services 4 What are community benefits? Community benefits are health care-related programs and services that Oregon s non-profit hospitals provide often with little or no compensation to address critical health needs in the community. Immunizations for children in low-income households Health screenings Donations to community clinics Transportation vouchers to appointments Cancer research Continuing medical education Support group Housing for low-income people Violence prevention programs

Community benefits 5 How Oregon hospitals contribute to their communities Learn more at MyOregonHospital.org Charity care Health care services provided to people who are unable to pay. Community health improvement Activities designed specifically to improve the health of the community, like education events, health screenings, clinics, hotlines and support groups. Health research Clinical and community health research, as well as studies on health care delivery, with results being shared outside the hospital. Health professions education Training future health care professionals by providing a clinical setting for training, internships, vocational training and residencies. Cash and in-kind contributions Funds and services grants, scholarships, food, equipment, meeting space to individuals or groups in the community.

Samaritan Health Services 6 Samaritan recognized nationally for community service In honor of its mission to provide exemplary health care and increase community benefit contributions, Samaritan was recognized as one of four national finalists for the 2017 Foster G. McGaw Prize for Excellence in Community Service. As a finalist, Samaritan Health Services received $10,000 to be used in support of our community health initiatives. Yale New Haven Hospital in Connecticut earned first place. As one of the most revered community service awards in health care, the Foster G. McGaw Prize is sponsored by the Baxter International Foundation, the American Hospital Association and its non-profit affiliate Health Research & Educational Trust. The annual prize recognizes the initiatives that hospitals and health systems take to improve the health and well-being of their communities. We are pleased and honored to be named a finalist for this prestigious award, said Doug Boysen, President/CEO of Samaritan Health Services. The work we do in the community is particularly rewarding and impactful, and we are committed to working with our local partners to build healthier communities together. Among other programs, Samaritan was recognized for its community service initiatives, including the Coast to Cascades CATCH Program, the Samaritan Early Learning Center, Community Outreach Inc., Healthy Smiles for All and Samaritan Maternity Connections program.

Samaritan Health Services 8 Above: COMP-NW Lifestyle Medicine students help teach the community healthy lifestyle habits Goal One: Healthy families Increase physical activity, fitness and access to healthy, nutritious foods for children, youth and families Medical students learning about lifestyle medicine at the College of Osteopathic Medicine of the Pacific-Northwest in Lebanon believe one thing to be true: the way we live our lives can have a big impact on our health. A grant from Samaritan Albany General Hospital helps fund the Lifestyle Medicine track for students at COMP-NW, who are often found out in the community sharing information about how nutrition, cooking and lifestyle choices can improve health. including the Coast to the Cascades Community Wellness Network s Culinary Health Education and Fitness Program (C.H.E.F.), Boys & Girls Club, senior centers, Special Olympics, soup kitchens and fire departments, to teach classes that give participants practical tools and resources to make a positive change. The students community classes include topics such as healthy cooking, gardening and exercise. We teach our medical students at COMP-NW that community service is an important part of their education and that service should be incorporated into their practice of medicine, said Assistant Dean for Medical Education Louise Muscato, PhD. The Lifestyle Medicine students partner with local organizations and programs,

Healthy families and greater access 9 Peer Support Manager Amelia Wyckhuyse and Executive Director Jeff Blackford Goal two: Greater access Increase access to medical, dental and mental health support and services in the community C.H.A.N.C.E. is a non-profit organization located in Albany committed to helping individuals who are struggling with substance abuse or mental health issues to improve their lives. The organization s mission is to provide necessary resources, guidance and positive support to participants. C.H.A.N.C.E. meets individuals where they are at on their personal journey to recovery, and its staff members use their own personal experiences as an example that recovery is achievable and imperative. The organization recognizes that recovery is multi-dimensional, which is why staff take a holistic approach by addressing housing, employment, education, health and other factors that affect someone s journey. Their Second Chance Program, which is supported by grant funding from Samaritan Albany General Hospital, focuses on one of the first barriers people often encounter when trying to change their lives obtaining legal identification and birth certificates. Getting a birth certificate or ID can be the first catalyst that helps someone take steps to improve their life, said C.H.A.N.C.E. Executive Director Jeff Blackford. One of our participants who just recently got a birth certificate was already able to go see her doctor, get stable housing and is on her way to the life she wants.

Samaritan Health Services 10 Goal three: Better networks Increase social supports for families For the elementary and middle schoolers in the Healthy Youth Program at Seashore Family Literacy in Waldport, fresh produce and nutritious cooking are quickly becoming a part of their everyday healthy habits. The 10-week program, which receives grant funding from Samaritan North Lincoln Hospital and Samaritan Pacific Communities Hospital, gives kids a positive place to spend time during the summer where they learn to plant and care for their own food in Seashore s 17,000-square-foot JOY Garden. From harvesting produce in the garden to learning about nutrition and cooking skills, the program takes a well-rounded approach to helping kids learn about living a healthy lifestyle. Living a healthy lifestyle should be our most important task in life, said Seashore Family Literacy Founder and Director Senitila McKinley. It is our responsibility to show that to our

Better networks 11 children. The whole process is healthy not only for the children, but for the adults and families as well. The program also features Fresh Fridays, where the kids prepare and cook produce from the garden to share with the community for tasting and purchase. Fresh Fridays are a great way for local families to get access to nutritious lunches and local produce who might otherwise not have had access to it, and for the students to share what they have been learning in the program. Above: Seashore Family Literacy Founder and Director Senitila McKinley

Samaritan Health Services 12 Goal four: Healthy kids Increase services and supports for children Playing with toys, enjoying art projects, reading stories with their parents these activities might appear to be just fun activities for children, but in reality, these are some of the biggest learning opportunities for young kids. This philosophy that encourages positive parent-child interactions and learning through playtime, literacy, arts and music is the backbone of Lincoln County School District s Learning is Fun Together (LIFT) Program. LIFT is a kindergarten readiness class offered in Waldport, Newport, Lincoln City and Yachats that serves homeless and at-risk children ages 3 to 5 and their families. Supported in part by grants from Samaritan North Lincoln Hospital and Samaritan Pacific Communities Hospital, the program provides a variety of play-based learning activities that help children reach their potential and prepare for school. In addition to helping children learn and prepare for kindergarten, the program also serves as reliable, high-quality childcare. For many parents, this can remove a significant barrier in being able to find and keep steady employment.

Samaritan Health Services 14 Goal five: Healthy teens Increase services and supports for adolescents Pregnancy Alternatives Center in Lebanon provides support, resources and medical care to women, including teens, who are pregnant and in need of a safe, supportive environment. Their services include pregnancy testing, patient advocacy, sexually transmitted infection testing and treatment, limited ultrasound exams and consultations with a health care provider. Pregnancy Alternatives Center also goes beyond just providing medical care. A grant from Samaritan Lebanon Community Hospital helps support the Healthy Moms Safe Babies program, which provides medical supplies for their clinic and classes through their resource center. Patients learn about parenting, nutrition, finances and more. By participating in the classes, participants have the opportunity to earn essential items, including diapers, clothes, car seats and even a crib. We provide a service that doesn t exist elsewhere in our community, said Executive Director Debbie Tracy. We combine health care and social services in the same visit. We have a more holistic approach to empower women and help them change their lives. Below: Participants attend a parenting class at the Pregnancy Alternatives Center

Healthy teens and seniors 15 Goal 6: Healthy seniors Increase social supports for seniors residing in the community Above: Participants at the Grace Center benefit from positive social interaction and engaging activities On any given day at the Grace Center for Adult Day Services in Corvallis, there are a wide range of activities happening chair tai chi in the gym, haircuts in the salon and special presentations from organizations such as the Chintimini Wildlife Refuge. These diverse and engaging activities are all designed to support a dynamic day service for seniors and individuals living with disabilities. Grant funding from the Good Samaritan Regional Medical Center helps support the Grace Center s financial assistance program through grant funding, which allows more individuals to join the program who may not otherwise be able to afford it. The Grace Center offers a positive place for participants to spend time that provides cognitive stimulation, socialization and physical activity. It also provides much needed respite care when family caregivers need a break. Many of our participants benefit from the financial assistance program, said Director of Community Relations Tera Stegner. The financial support from community partners like Samaritan Health Services truly helps us serve a need in the community and support our mission.
