And really, the cold hard truth is that we need you! You have the ability to carry this message to the masses and get people talking. 3 As you know, colon cancer has become a surprising reality for too many people younger than age 50. Recent data also shows that incidence rates in people over 50 has decreased, but the under 50-population has actually seen an increase in new cases. Given these alarming statistics, it is imperative that organizations in the colorectal cancer community JOIN FORCES and TAKE ACTION around this issue. We can do more together than alone on this emerging issue. OVERVIEW: Who Are We? History The was founded in July 2014 by the Colon Cancer Alliance, who recognized the need for greater awareness surrounding young onset colorectal cancer. By bringing together other nonprofit organizations and medical professionals that were also passionate about this issue, the Never Too Young Coalition became a collaborative effort with one message: you are never too young to be diagnosed with colorectal cancer. Mission The is united to take on young-onset colorectal cancer through action, education and research. Goals of the Coalition Support research that investigates the causes and factors associated with a rise in young onset colorectal cancer incidence rates. Disseminate research findings as a vehicle for increasing knowledge and increased high-risk screening in the under 50 population. Utilize the Never Too Young microsite and collateral materials as a vehicle for the Coalition to communicate information to patients and providers. Host an annual fundraising and awareness campaign that highlights young onset colorectal cancer as a public health issue on a national and international scale. Page 3
4 Benefits and Expectations of Coalition Partners Benefits The opportunity to build strong connections and have access to peer support from other leaders in the colorectal cancer space Organization logo to be included on the Never Too Young microsite Organization to be highlighted on the Coalition Facebook page The opportunity to participate in groundbreaking research Organization will have access to all educational and promotional materials created by the Coalition Expectations Membership and active participation in a minimum of one committee Attendance at a minimum of one Coalition at-large meeting per year As an active partner, you will be expected to act as a Coalition liaison to your organization, promoting Coalition initiatives and providing relevant information to your colleagues Contribution to annual dues requirement (See breakdown below) Active participation in Young Survivors Week Relevant organizational collateral material to be provided for various conferences and events Inclusion of the logo on relevant collateral material Individual NEVER TOO YOUNG COALITION Partnership Levels Researchers and medical professionals $250 Patients, Survivors, Caregivers, Family Members $10 For families of three or more $25 Organizations Based on a nonprofit organization s annual income under $25,000 $50 $25,000-$49,999 $75 $50,000-$99,999 $100 $100,000-$199,999 $125 $200,000-$299,999 $150 $300,000-$399,999 $200 $400,000-$499,999 $250 Page 4

By participating in at least one committee, you will keep your finger on the pulse of the. Committee Chair Two-year term Holds seat on Steering Committee Provides updates on committee at Steering Committee meetings and Coalition at-large meetings Holds committee meetings via conference call Committee Co-Chair Two year term Holds seat on Steering Committee Provides support to Committee chair Records and shares notes from committee meetings Committee member Attends committee meetings Takes an active role in committee initiatives & tasks as outlined by committee chair Page 5
6 Committees Coalition Director Oversees the Steering Committee. Provides direction for Coalition initiatives. Steering Committee Serves as the Coalition's leadership team. Includes chairs and co-chairs from each committee. PR/Marketing Committee Responsible for any marketing, educational materials and/or events that are created by the Coalition Community Advocate Committee Consists of colorectal cancer patients, survivors, previvors, caregivers and family members. Responsible for disseminating Coalition materials and promoting Coalition initiatives in their local area. Research Committee Responsible for any research conducted and supported by the Coalition. Fundraising/Development Committee Responsible for acquiring funding to support Coalition initiatives. Financial Advisory Committee Reviews all financial statements and the Coalition budget. Provides guidance and input on all Coalition expenses over $500. Provides periodic financial updates to the Coalition partnership. Director Stephanie C.
