5 MISSION St. Joseph s Care Group is a Catholic organization that identifies and responds to the unmet needs of the people of Northwestern Ontario, as a way of continuing the healing mission of Jesus in the tradition of The Sisters of St. Joseph of Sault Ste. Marie. VISION St. Joseph s Care Group is a leader in client-centred care. CORE VALUES CARE St. Joseph s Care Group will provide quality care for our clients body, mind and spirit, in a trusting environment that embraces diversity. COMPASSION St. Joseph s Care Group will demonstrate dignity and respect for those in need, accepting people as they are, to foster healing and wholeness. 6 FOCUSING ON OUR CLIENTS, OUR PARTNERS, OUR PEOPLE, AND OUR FUTURE Our Strategic Plan ushered in one of our largest periods of growth since The Sisters of St. Joseph of Sault Ste. Marie first opened our hospital in Some called the plan overly ambitious, but we are proud to have accomplished much of what we set out to do. Our construction projects are certainly the most visible of our changes over the past four years with the completion of Sister Leila Greco Apartments, the Hogarth Riverview Manor Expansion, and the new East Wing of St. Joseph s Hospital, set to open in Fall More important are our evolving models of client-centred care. We are continuing to advance a self-management approach to care, and we are involving our clients more closely in their own care decisions to achieve the health outcomes that are important to them. We are using technology to help overcome challenges to healthcare including the sheer size of Northwestern Ontario to bring care closer to the people we serve. In other words, we are accomplishing our Mission of responding to the unmet needs of our clients throughout Northwestern Ontario, and improving our level of care all the time. 7 After this period of rapid growth, we are focusing on the delivery of our new commitments and maintaining our excellent level of care. We are on the right track. Our staff and volunteers are proud to work at St. Joseph s Care Group, and our clients tell us how every day our people go above and beyond expectations, and live our values of Care, Compassion, and Commitment. We will continue to set the bar higher. Our single most important goal is to enhance the client-centred care we provide today and ensure we continue to meet the needs and expectations of Our Clients, Our Partners, Our People, and Our Future. That includes recognizing the unique partnerships we must continue to develop with Indigenous Peoples in Northwestern Ontario. We will also focus on our people and the resources that will enable us to continue to provide excellent care and leadership in the future. 17 While we have always had to effectively manage our finances, now more than ever our funders are insisting on increased efficiency and accountability. We want to make sure that we are remaining true to our Mission while continuing to provide safe, high-quality programs in a way that is sustainable now and for our future. 18 HOW WE DEVELOPED OUR PRIORITIES A strategic plan is a complex document. It requires an understanding of where we are as an organization, where we want to be in five years, what challenges we are likely to face, and how best to overcome those challenges. That s why research and engagement are so important. St. Joseph s Care Group completed an environmental scan to better understand the future needs of the people that we serve and trends in healthcare delivery, as well as provincial strategy and funding approaches. None of the data are meaningful until we understand what is important to our clients, our tenants, our residents, our staff, our volunteers, and the populations that we serve. That is why St. Joseph s Care Group conducted multiple interviews, focus groups, surveys, meetings, and planning sessions. We spoke with our clients, volunteers, direct care staff, clinical leaders, professional staff, management team, board of directors, service delivery partners, and our local and regional stakeholders. We gained insight into the things we do that are valued, how data are viewed through the lens of local experience, and about opportunities to meet unmet needs. We would like to extend our heartfelt thanks to all for sharing your candid feedback, experiences, and your hopes for the future. Seeking input from clients and families on this Strategic Plan was a very natural thing for us. It is an extension of how we work together whether we re developing a care plan or developing a new building and environment for service. We are here because of you. 16 SJCG Strategic Plan
