4 Introduction This guide provides an introduction to the services you will receive from Walsall Council s Transitional & Leaving Care Team (TLC). The aim of the guide is to help you understand the type of support on offer and to make sure your journey into adulthood and independence is successful. Who Will Receive Support Leaving care can be a confusing time and therefore to help you understand the type of support that you will receive from TLC you will need to understand some key words and what they mean. If you what any further information or for things to be explained in more detail your Personal Advisor or Social Worker will be able to explain. Types of Care Leavers You are an Eligible Care Leaver if : You are aged 16 and 17 and You are currently a looked after You have been looked after for a period of 13 weeks (or periods amounting to 13 weeks) which began least 13 weeks since the age of 14 and ended after you reached 16. You are a Relevant Care Leaver if : You are no longer being looked after by the local authority but you have been eligible and You are aged 16 or 17 years old You were in custody or hospital when you turned 16 but immediately before that you were looked after 4 P a g e

5 You are a Former Relevant Care Leaver if : You are 18 years or above and either have been a Relevant or Eligible child when under 18 If at the age of 21 or before reaching the age of 25 you are in full time education or training, then you will remain a former relevant care leaver until the end of agreed programme If after 21 you inform us that you want to pursue or are pursuing a programme of education or training You are a Qualifying Care Leaver if : Young people under 21 (or 24 if in education or training) and You were looked after immediately prior to the making of a special guardianship order which was in force when you reached 18, or If at any time after you reached 16 but while you were still a child was, but no longer are looked after, accommodated or fosterer, or You are privately fostered and assessed to be in need As a qualifying care leaver you are able to access advice and assistance based on a needs assessment completed by TLC 5 P a g e

6 What type of support can I expect? Walsall Councils Local Offer for you as a Care Leaver includes support relating to: (a) health and well-being; (b) relationships; (c) education and training; (d) employment; (e) accommodation; (f) participation in society. We want to get the support right for you and we will consult with you about which of the services you feel would assist you the most and how you want to receive that support in preparing for, adulthood and independent living. The Role of Social Workers & Personal Advisors As an Eligible Care Leaver you will be allocated a Social Worker from Walsall Councils Corporate Parenting Team. Your Social Worker will be responsible for the preparation of your Pathway Plan. You will remain allocated to your Social Worker until you are 18 years old and become a Former Relevant young person. At 17 you will also be allocated a named Personal Advisor from the Transition and Leaving Care Team who will work along side you and your Social Worker and support you in preparing for your transition into adult life. Your Personal Advisor will then take over from the Social Worker when you leave care at the age of 18 and become a Former Relevant care leaver or if you become a Relevant care leaver before the age of 18 years. If you are a child in care who is supported by the Children with Disability Team your current social worker will support your transition to Adult Services. At the age of 17 the Transition & Leaving Care Team will allocate you a Personal Advisor to work along side your Social Worker. Your Personal Advisor will take over responsibility for your Pathway Plan when you reach 18 and become a Former Relevant care leaver and will continue to work closely with Adult Services. Support from a Personal Advisor (PA) All eligible relevant and former relevant care leavers will be allocated a PA when they reach the age of 17 years. Your personal advisor will help you and support you to develop skills so that you can live independently, when you are ready to do so. A key part of the PA s role is: 6 P a g e

7 1. to help you find accommodation 2. to coordinate support that will help you keep and maintain your accommodation, including developing independent skills 3. to help you with your continue education or 4. to help you find appropriate training and employment 5. to help you manage your finances and claiming any benefits you are entitled to Your PA will also provide you to access any specialist support you may need, when you need it, including counselling, advice on issues such as drug and alcohol, sexual health and building healthy relationships. Contacting Your PA Your PA will provide you with a card with their name, contact details telephone and address and the address of the TLC Hub, if your PA is not available any one of the TLC team will be happy to help you. We have recently moved to your new TLC HUB : Its within 10 minutes of walking distance from the town centre has been sourced, following consultation and joint visits with young care leavers. Transitional and Leaving Care Team are now located at the Hub.. The address is TLC Hub, Brineton Street, Pleck, Walsall, WS2 9EP How Long Will I Receive Support for From My PA You will receive a leaving care service until your 21 st Birthday or up to your 25 th Birthday with your agreement and if you choose to be supported or if you are in full time education or training until you have completed the training programme. Out of hours support We are currently reviewing the out of hour s support that the service can provide and we are in consultation with Council 4 Kids to get their views on the type of support 7 P a g e

8 they feel would be helpful. In an emergency place contact the Emergency Duty Service on Pathway Plans We will prepare Pathway Plans for all Eligible, Relevant and Former Relevant young people. Your pathway plan will replace your care plan from the age of 16 years. The plan is about your needs and what we will all need to do to ensure your successful journey into adult life. How will I be involved? For your pathway plan to be effective it will be based on an up to date needs assessment; setting out the support that will be offered to you to achieve your aspirations, this may include housing, education, training and employment, health services etc. We aim to fully involve you in the development of your pathway plan and it will reflect your wishes and views. The diagram below shows some of the areas that will be covered in your Pathway Plan Education Training & Employment Emotions & behaviour Finance Health & Self Care Identity Family & social relationships A place to live What about changes to my plan? Your pathway plan will be updated and reviewed at least every 6 months or within 28 days of any significant change to your plan, such as moving home or if your request a review. Your pathway plan will initially be written and available at your statutory review meeting which is chaired by your Independent Reviewing Officer (IRO), a review must take place before making a decision to confirm that you are ready to leave care 8 P a g e

9 Where Will I Live After I Leave Care? This will depend on your individual circumstances and needs. There are a range of accommodation options that are available to meet the individuals needs of young care leavers in Walsall and your PA will assist you in making the right choices for you, including helping you to apply for social housing when you are ready, The options include: Staying Put Once you have reached the age of 18 you will not technically be in care especially where you are living in foster placements, it may be possible for you to remain in your care placement for a period of time after your 18 th birthday. If you want to remain you should discuss this with your social worker or PA. Shared Tenancies Walsall Council work in partnership with Walsall Housing Group and have set up a number of shared tenancies, where you and another young person can share a flat. You get your own bedroom and shared the bathroom, kitchen and living room. Many young people like the option of shared accommodation and you also get to share the bills and having a flat mate to breakdown isolation and loneliness. Your PA and the Housing Team within Walsall Council will talk to you about this, you will get to look at the accommodation and meet the young person you could potentially be sharing with. Supported Accommodation Supported accommodation is best described as a housing scheme where housing, support and sometimes care services are provided as an integrated package, most are for people who need support to live independently and designed to help people acquire the emotional and practical skills needed to move on into more mainstream housing. Supported Lodgings The Black Country YMCA manages the supported lodging scheme in Walsall. It offers young people (ages 16-24) accommodation in the homes of individuals, couples and families who live in the Black Country. A young person placed with a Host will receive support from YMCA Black Country Group to help them gain the confidence and skills they need to become independent adults. Emergency Nightstop: A young person, subject to passing a risk assessment, is given an emergency Nightstop placement with a Host family while a more 9 P a g e

10 permanent living situation is sorted out. Dinner, bed, and breakfast are provided on this service, and daystop if required. A vulnerable young person can be placed in Supported Lodgings with a Host family for up to 2 years while homelessness remains a risk. Each individual is given a Project Worker, who works with them to develop skills for life. We encourage young people into education or employment to lead them into independence, and a sustainable future. Accommodation & Support for those with complex needs Walsall Council work closely with St Basil s who have an accommodation service in Walsall known as Hope Street which is an 8 eight bed unit. St Basil s also delivers a specialist program that prepares young people to move into their own accommodation. St Basil s will Assess your needs and put together a plan of action to ensure that the skills you need as an individual are met. Will meet with you regularly along with your support providers and set targets so that all is kept on track. Offer life skills as part of our program, mental skills training, TKO, anger management and partner with other organisations so that all of your needs can be met. A quote from a young person from Hope Street The staff are friendly, helpful and have different activities going on during the week. There is always someone on site if you need help LB Social & Private Tenancies When you are ready for independent living your PA will help you to apply for housing with both social and private landlords in Walsall. All social landlords, such as Walsall Housing Group, Accord, and Friendship Housing have agreed to award all care leavers, when ready, top priority on their waiting list and your PA will work with you to make the application and nominate you to the appropriate landlord. Private landlords will also offer tenancies and again your PA will talk to you about the options available through this route, the finance that maybe available to you to secure accommodation, such as deposits, rent in advance and/or a guarantor, Walsall Council maybe able to act as a guarantor for you where no one else is able to take that role, again your PA can talk to you about this. Jobs, careers and Training Education, Training and Employment are an important part of both becoming an adult and living successfully in your own home. Studying to enable you to have a 10 P a g e

11 career in your chosen profession, training to obtain the skills you need to do the job of your choice and finally obtaining employment not only gives you confidence, self esteem and a feeling on independence, standing on your own two feet and earning your own salary, is what we all should aim for. Your PA will advise you about employment and training opportunities and put you in touch with a our specialist Advice Information & Guidance Officer or an Impact Officer to discuss your options and the opportunities available to you for your career. What Financial Help Am I Entitled to? The amount of financial support that you will receive will depend on your care leaver status, are you relevant, former relevant etc, we have a booklet specifically titled Financial Support for care Leavers which gives you a detailed break down on the financial help available, if your PA hasn t given you a copy please ask them to, it is also available on our website How Do I Get Involved In Helping To Improve The Service For Care Leavers? We would like you to be involved with all areas of the Transition & Leaving Care Service Including your new TLC HUB - Consultation has started with a number of care leavers as to what services they would like to see within their TLC hub. So far they have told us they would like to Be proud of our place, it is there to help us. Most of all make it about the leavers; somewhere we feel safe and cared about. Keep one room for our children, have soft toys, books and paint. Hold Parent groups in that room. Keep one room for TLC groups; e.g. drop-ins, New Belongings, make that room about the group; eg. Pictures. Have a kitchen for the leavers so they can make drinks (hot and cold). We are adults treat us like adults. Can we have a TLC nurse and can she be base there (free sexual health screening and free access to condoms) Set up and hold clothes bank with leavers and member of staff. This helps with confidence, skills training and self-esteem. Hold food bank for leavers in the new building so leavers won t feel embarrassed getting their food when they are struggling. Keep computers with access to interest so leavers can do job search, apply for benefits and for setting up home grant. Have garden club so leavers can be proud of their space, pretty flowers, and homes for wildlife. Have a room allocated for mental health with a Counsellor or someone just to talk with. Have a benefit person there for when benefits get sanctioned etc. 11 P a g e

12 Hold talks with local police and tell them what we think should be changed also they can tell us what to look out for. We should change the way people think about care leavers, invite people from Walsall in to see what we are really like. Social Groups There are three groups we would like you to be involved with depending on your age: Council4Kids Respect Group New Belongings Council4Kids meet 4 times a year In consultation with Respect and New Belongings we have reviewed how you want your Council4Kids to work. It is important that both the Voices of Children Looked After and Care Leavers are heard. The Council4Kids is now the overarching forum of children and young people. You can influences those who make decisions about services and to monitor how Walsall delivers on their Pledges to you, whether you are in care and have left care. This forum is relatively new and we have a number of young people working with us to increase membership and help develop it further. Members of Corporate Parenting Board as well as other guests from other services and groups will attend at your invitation to ensure our children and young people s voices are heard, the pledges made to you are delivered and improvements are made. Respect Group, Respect group is a youth club for looked after Children and Young People aged between 9 years and 19 years of age. The group meets every Tuesday from 5.45 to 8.15 pm at My Place Young People s Centre, 60 Walstead Road West, Walsall WS5 4PE. Young people views 12 P a g e

13 We are respect group we are a group of young people who come together to make new friends. 3 words to describe Respect Group FUN, LOUD LIVELY, you are able to get involved with activities such as Film project Corporate Pledges Total respect training Dear social worker posters Community feasts Cinema and Bowling trip C4K Achieved food hygiene certificates Boxercise classes Helped create sensory garden The group meets in a safe environment with other young people who have similar experiences to make friends, and learn new skills New Belongings for those of you 18 yrs plus and who have left care The New Belongings Group meets once a month at the TLC Hub on a Tuesday evening between 5.30 pm and 7.30 pm They are a group of young care leavers that has been developed to give care leavers a voice. At this group we discuss any problems young people may have or be experiencing and to try to help resolve them as supportive group. Increasing involvement We would love for you to be more involved, please speak to your PA about the opportunities and make a difference, you PA can and will come along to the groups with you. Ambassador Scheme We are consulting with you to develop a TLC Ambassadors Scheme aimed at those of you who are older and more mature and have supported New Belongings and worked hard to develop services. The aim of the scheme will be to promote the service and be involved in a recognised range of activities such as attending interviews, development programmes, facilitating work shops answering queries about the service and participate in national and local conferences such as National Leaving Care Benchmarking forums. These skills and experience that you will gain from being a part of this will help develop and raise your aspirations that can be taken into your adult life. 13 P a g e

14 Your Health Passport You can request information about your health history from our Children and Young people s Looked After Health Team. This offer is available even after you have left care. Sarah Kirk is the Health Advisor for young people leaving care or who have left care. She will be spending some time that the TLC Hub and you can pop in to see her or you can always contact Sarah on or ask you Personal Advisor. Access to your Social Care Records The law says that you have a right to see information that is written about you, unless we think that it would be emotionally harmful to you, this includes social care files, your Social Worker will have kept records about your health, education, family members, foster placements etc so that if you want to and are ready to you can look back and see what has happened to you and have a record of your time in care. When you reach 18 years of age you are able to see the records, please talk to your PA about this as they will support you and help you to understand some of the information. You don t have to rush to see your records, Walsall Council keep your records until your are 75 years old and you can see them at any time until then Comments Compliments and/or Complaints If you want to make a compliment or you are unhappy with something and wish to complain you can do so by writing in to the Manager of the team, by completing a complaints form available at: Or you can speak directly to your Personal Advisor or to the Manager who will deal with your concerns and formally reply to you.
