1 Job Description Job Title: Project: Base: Drug & Alcohol Recovery Coordinator Forward Leeds (Substance Misuse Services) Irford House, Seacroft Crescent, LS14 6PA Salary: 16,954-24,239 Hours: Duration: Clearance required: Responsible to: Job Purpose: 37.5 hours (worked flexibly within the service opening hours) Permanent Enhanced DBS Lead Practitioner As directed by the Project Manager, you will be an integral member of a dynamic team delivering high intensity one to one and group structured psychosocial interventions to individuals within a recognised theoretical framework. You will use NICE/NAT recommended interventions; (e.g. Social and Behavioural Network Therapy, Cognitive Behavioural Therapy and Motivational Enhancement Therapy) to work with individuals with a strong recovery focus. Main duties and responsibilities: Use a range of medium/high intense structured psychosocial interventions including motivational interviewing, ITEP, CBT-SBNT and MET in both one-to-one and group settings to promote engagement and retention in treatment services, encourage self-esteem, well-being, selfresponsibility and enhanced motivation To be part of the PSI team supporting service users to move forward in their treatment journey by delivering SPSI s (Structured Psychosocial Interventions) in accordance with NICE guidelines Delivering a robust and varied combination of structured work and group work to tackle some of the underlying issues that prevents service users from successfully progressing and exiting treatment Page 1 of 7

2 To be proactive and use information systems and informal discussions with colleagues to identify service users who would be suitable for an PSI, e.g. those with low mood or those about to undertake a reduction or detox programme To advertise the availability and benefit of PSI work at team meetings and other forums to generate referrals To individually asses and use the most suitable intervention CBT, MET, SBNT with each service user To work in collaboration with the others and independently on the development and facilitation of specific therapeutic workshops and group work with the PSI team and Structured Group workers, to provide a compliment of courses and group work provision for Forward Leeds users. Explore and select methods with individuals that are most likely to be effective including Mindfulness and other resilience and strength based approaches Where necessary carry out triage, strengths based comprehensive assessments and risk assessments, and to contribute to the development and implementation of individual recovery plans Contribute to the effective preparation, monitoring and review of individual and group PSI intervention plans Key Duties: Manage a service user caseload and review on going care and treatment liaising closely with the full range of specialist workers within the multi-agency team, including family members and significant others as appropriate Will support service users to access resources and services to increase their recovery capital to help them achieve their recovery goals and promote family, carer and service user involvement To exercise excellent communication skills and therapeutic skills when dealing with service users, carers, caring agencies and be able to manage work with individuals on any tension between their hopes and expectations and the reality of resource limitations To work closely and in partnership with the multi-disciplinary team from across the partnership and externally and provide advice and guidance to managers and other colleagues, through attendance at reviews, team meetings and group supervision and via the telephone Comply with all monitoring and evaluation systems, collect, and monitor information as appropriate and provide reports when required and actively contribute to the Partnership achieving successful outcomes for service users Page 2 of 7

3 Maintain accurate, up to date records and work in accordance with organisational guidelines and procedures To comply with all aspects of our Code of Conduct and adhere at all times to national/local and DISC Safeguarding policies To provide effective coordination of the individual s recovery plan and promote family, carer and service user involvement To improve the outcomes for children of service users by reducing the impact of drug and alcohol related harm on family life and to promote positive family involvement in treatment. completing a home visit within the first 4 weeks with all service users who have children To reduce substance related harm to the individual and the wider community Refer service users who have contact with children for parenting interventions To contribute to on-going assessments, including risk assessments, Safeguarding assessments and plans, and recovery plans Provide information within accordance to the confidentiality policy to the team involved with individuals to ensure a holistic approach Actively participate in supervision and be willing to personally reflect on interpersonal dynamics within the therapeutic relationship and provide evidence of achieving performance targets alongside other supervision requirements as detailed in DISC policy Recognise personal and professional boundaries and work within DISC clinical governance framework at all times To work with Peer Mentors to provide advocacy/support to service users and Foster a culture of peer support, leadership and family inclusion The post holder will be required to work flexibly across operational sites and within an agreed number of hours to maintain the most appropriate level of service provision. This will include evening and weekend working To report all problems and incidents to the Area and Project Managers at the earliest opportunity Uphold data protection legislation and, local and formally agreed information sharing protocols and service user confidentiality agreements Provide a full range of services in an outreach and in-reach capacity including satellites offices and in partnership with others Support service users through structured reduction programmes, planning and supporting any community/impatient detox or residential rehabilitation requirements Page 3 of 7
