2 Achieve SAlford recovery ServiceS Achieve Salford Recovery Services brings together people from a range of organisations, backgrounds and experience to deliver services, opportunities, treatments and therapies for people seeking help in tackling their own drug or alcohol use, or that of a loved one. Together we are determined to make a real difference to families, communities and people affected by substance use in Salford. People with lived experience of their own recovery or as a carer play a central role in our services alongside professionally qualified staff from clinical and therapeutic backgrounds. Achieve forms part of: in partnership with: Achieve Salford Supporting Recovery in Salford
3 WhAt We do... Our city-wide recovery services offer support, treatment and advice to adults and young people living in Salford to enable them to improve the quality of their lives. We also work with families and carers using substances, whether in treatment or not. At Achieve, we work in partnership with other organisations and our aim is to provide access to education, training, employment and housing advice. Financial support is also available to enable an individual to achieve their recovery goals. Who We Are... Greater Manchester Mental Health NHS Foundation Trust (GMMH) provides the Achieve adult and young people service in partnership with the following organisations. Early Break - Young Peoples Drug and Alcohol Charity is a specialist, structured 6 month programme delivered by Achieve s Holding Families specialist practitioners working with parents and children affected by substance misuse, aimed at reducing harm within families. It is delivered through group work, individual meetings and family meetings.
4 Great Places Housing Group provides housing advice and floating support, alongside supported housing for alcohol users; based on a harm reduction model. THOMAS - Those on the Margins of Society Is a 12 Step provider of 2 residential rehab centres, one for men and one for women, and supported second stage accommodation. Salford Royal NHS Foundation Trust Alcohol Assertive Outreach Team (AAOT) Work with the top 30 attendees at A&E who have complex needs associated with chronic alcohol misuse. The team provide outreach, individual appointments, therapeutic groups and a drop in service. Salford Royal NHS Foundation Trust provides specialist alcohol nurses offering advice within an A&E setting and on the wards, initiating medicated alcohol detoxification when required and liaising with other services to potentially refer on. Salford Citizens Advice Bureau (CAB) Providing advice and information at all sites across the Achieve partnership. GMMH Young People Team Achieve has a dedicated team who work closely with young people up to the age of 25 offering support and information related to substance use. The team are co-located with the Integrated Youth Support Services. Achieve Salford Young People s Service Achieve Salford Supporting Recovery in Salford
5 Services we offer Advice and information on substance use and reducing substance misuse related harm Assessment within 48 hours and individualised recovery planning Advice and guidance on your alcohol and drug intake Psychosocial interventions to improve motivation to change, and build on your strengths to support you to achieve your goals A diverse universal group programme delivered across Salford Access to inpatient and home detoxification We want to help you in more ways than one We offer information, support and advice with: Accessing other groups and organisations within the local community, e.g. housing providers, benefits agencies, peer led support groups etc Health promotion and health screening, e.g. smoking cessation, dental care Advice, information and support for carers and family members Aftercare support for remaining drug/alcohol free We will retain long-term contact up to 5 years with people in their communities through the We Call Because We Care aftercare follow up service A Recovery Fund to nurture and develop existing community groups and to support new and innovative ideas to build a sustainable and contagious recovery community in Salford Access to a diverse volunteering programme Achieve Salford Supporting Recovery in Salford
6 accessing the service You can refer yourself, or somebody who is working with you can refer on your behalf. After we receive your referral, we will contact you and offer you an assessment the next working day. During your assessment we will establish what your needs and goals are, and start drawing up your recovery plan together. Our service is confidential, and we will ask your consent if we need to share any information, and explain the reason why. If we believe you or somebody else is in immediate danger, we may need to let other people know, but we will always try and explain this to you first. We will try and offer you appointments at a time and venue that is convenient to you. If you wish please bring a friend or family member or carer with you to your appointments, and they can be as involved in your recovery as you like. We can also offer support and guidance to people you care about who have been supporting you, including a Carer Assessment. Achieve Salford Supporting Recovery in Salford
7 SeRvice user forums Achieve offers a range of service user groups and forums including peer to peer support groups across Salford. RecoveRy ReSouRce centre The recovery resource centre is a community based recovery hub providing a range of services including: - family interventions - brief interventions - psycho social interventions - therapeutic activities GET INVOLVED! People with lived experience of substance misuse undertake a key role in our services and we have seen first-hand how their experience has helped others. Achieve Salford Recovery Services offer a variety of volunteering opportunities for people who are passionate about helping others on a road to recovery.
