3 Part I: Instructions & Information I. OVERVIEW AND EXPLANATION In an effort to allow chapters to more efficiently and effectively provide information to the General Fraternity about their annual operations and activities while also encouraging chapters to submit applications for the various awards and special recognitions of Sigma Chi, the Chapter Annual Report submission form has been combined with the major awards application for Historically, much of the same information included in this report has been requested in the reporting and application process of prior years. The combination of the two reports along with the ability to utilize technology and online submissions will allow chapters to see an easier submission process that will result in less duplication of efforts. II. J. DWIGHT PETERSON SIGNIFICANT CHAPTER AWARD The Peterson Significant Chapter Award, sponsored annually by the Sigma Chi Foundation, is the highest honor bestowed upon an undergraduate chapter. Named for Past Grand Consul J. Dwight Peterson, Indiana 1919, the award recognizes excellent, but not impossible, performance by chapters in all major areas of operation and programming. For many years, the Peterson plaque has been a symbol of dedication, idealism, and achievement. There is no minimum or maximum number of chapters that may win the award in any given year. Award criteria are assigned on an objective scale and encompass the entire gamut of chapter activities, from financial stability and recruitment to chapter and member reputation to Ritual and initiation performance. Because chapter operations vary among campuses and chapters, each chapter measures itself against these absolute Sigma Chi standards, rather than against other chapters. Winners of the J. Dwight Peterson Significant Chapter Award will be those chapters whose evaluation of the information included in their Chapter Annual Report submission yields a result of 90% or higher. New this year is a level of special recognition for chapters who achieve a significantly high score in winning the J. Dwight Peterson Significant Chapter Award: Peterson Blue & Gold Awards J. Dwight Peterson Significant Chapter Gold Award To achieve this award, chapters must achieve a 97% or higher on the score ascribed to their annual report. These chapters will receive a gold embossed seal on the certificate and be specially recognized at Balfour LTW. J. Dwight Peterson Significant Chapter Blue Award Chapters winning this award will be those chapters that score between a % on the score ascribed to their annual report. These chapters will receive a blue embossed seal on the certificate and will receive the award at Balfour LTW. III. OTHER AWARDS A. James F. Bash Significant Improvement Award The winners of this award will be chosen from the Chapter Annual Report / Awards application submissions. The James F. Bash Significant Improvement Award, funded by the late 49th Grand Consul James F. Bash, Butler & Indiana 1946, and his wife, Connie, is given to chapters demonstrating major improvement in total chapter operations from year to year. Significant Improvement is defined as a gain of at least 25 percent in the score ascribed to the chapter s annual report from one year to the next. For example, if a chapter scored a 60% on their CAP Report in 2009, they would need an 85% on their CAP Report in 2010 to win the Bash Award. If a chapter wins a Peterson Award, they would not also receive a Bash Award. The chapter must have achieved a score of at least 50% on the previous year s report to be eligible.

4 B. Daniel William Cooper Award The winners of this award will be chosen from the Chapter Annual Report / Awards application submissions. Each year, the Fraternity honors an undergraduate chapter that has the most outstanding scholarship program with the Daniel William Cooper Award. The winning chapter receives a plaque and has its name engraved on the Daniel William Cooper Award trophy on display at Headquarters. The winner of this award is chosen from the Legion of Honor Award recipients. C. Legion of Honor Award The winners of this award will be chosen from the Chapter Annual Report / Awards application submissions. The Legion of Honor Award honors undergraduate chapters with commendable scholarship programs. The scholarship program in every Sigma Chi chapter should encourage and create positive scholastic attitudes and enhance the individual brother's desire to reach his fullest potential during the course of his college education. The award is intended to stimulate chapters to establish scholarship programs that benefit all brothers in the chapter. The winners of this award are chosen from the scholarship details and questions included in the Annual Report and Awards application. D. Robert E. Joseph RMF Award Interested chapters should visit the RMF Web site, for more information. E. Charles G. Ross Undergraduate Chapter Publications Award The Fraternity presents the Charles G. Ross Award to the undergraduate chapter with the most outstanding publications program. Criteria include content of both undergraduate and alumni news, writing and editing, layout, general appearance, frequency of issues and the quality of the publication. The award is named for the late Significant Sig Charles G. Ross, Missouri-Columbia 1905, newsman and press secretary to Harry S. Truman. The winner of this award is chosen from newsletters mailed to Sigma Chi Headquarters between June 1 st through May 31 st of each year. To be considered for the Ross Award, send copies of your chapter s newsletters and external communications to the Sigma Chi Headquarters at: P.O. Box 469 Evanston, IL IV. CHAPTER ANNUAL REPORT A. Objectives The Chapter Annual Report is a process for the chapters to efficiently submit data and reports of their annual activities to the General Fraternity. With this information, chapters will have the ability to comprehensively analyze their performance against a set of standards that Sigma Chi considers vital to the overall health of a chapter. Concurrently, the Fraternity will utilize the data to identify chapters that may need assistance or additional resources to help them achieve excellence in all areas of the chapter s operations and objectives. Additionally, the chapters and the General Fraternity will be able to utilize this information to track historical trends and identify areas of growth and successes over time. B. Requirement to Submit Every chapter is required to submit annual reports every year regardless if they intend to apply for each award. C. Annual Report Results Each chapter will have a score attributed to their annual report. The results given to the chapters will be based on a percentage score as opposed to a point system. D. Submission Information The following concerns the submission information to be included in the Annual Report. a. Deadlines The deadline for submission of the combined Chapter Annual Report and Awards Application is May 1 st of each academic year. 4

5 b. Validity of Information Information furnished should be for the academic year and should be accurate as of April 15 th, c. Verification of Validity Each report must be verified by the inclusion of your Consul s address which affirms that the information provided is accurate and valid. There are specific questions throughout the report that pertain directly to Grand Praetors and Chapter Advisors. To receive points for those questions, the respective advisor must submit answers on the chapter s behalf. d. Supplying Accurate Information Sigma Chi expects an utmost level of honest and integrity in the information submitted and presented in each Annual Report and Awards Application. Unintentionally supplied inaccurate information will result in a loss of points to be determined by the grading committee depending on the situation. e. Online Submissions All submissions for the Annual Report and Awards Application must be submitted through the specific submission Web site. At the date of release of this information, the Web site is not yet available as construction is underway. Due to the planned improvements, the Web site will not be the same as last year and chapters should not revisit that site for this year s report. Information on the submission site will be released via a separate announcement at a later date. V. ADDITIONAL ASSISTANCE? A special effort has been made to streamline the efforts that our chapters are making to properly and timely submit information for the Chapter Annual Report as well as apply to win the various awards and recognitions provided by the Fraternity. A special task force of dedicated Sigma Chi s was commissioned by the Grand Consul and the International Executive Committee to help improve the process. We look forward to recognizing many chapters for the outstanding efforts that they make this academic year. 6 II. Report Criteria CHAPTER OPERATIONS Chapter Statistics Chapter size at end of fall term. Average size of fraternities on campus at end of fall term. Expected chapter size at end of academic year. Expected average size of fraternities on campus at end of academic year. Number of pledge classes per year. Name of pledge classes (fall 2009, spring 2010, winter 2010, etc) Chapter Web site What is the chapter's Web site URL? Has the Web site been updated within the last 3 months? Does the Web site have one or more links to SIGMACHI.ORG? Chapter Goals Did the chapter create a strategic plan for the current year (could have been at Balfour LTW)? Did the chapter submit the strategic plan to the Fraternity? Did the chapter meet the goals of the strategic plan? Chapter Officers Does the chapter maintain an effectively functioning Executive Committee or board that meets weekly? Do the officers maintain files that contain policies, procedures, guidance, and responsibilities for each office? Does the chapter conduct an officer transition retreat for new officers? Does the chapter maintain a committee structure? Are 75% of the brothers either an officer or member of a committee? Does the Annotator create and distribute accurate and useful minutes of all chapter meetings? Did the Historian create a summary of the significant operations and events of the chapter? Does the Historian maintain a rich local chapter history for the purpose of member and pledge education? Other Does the chapter use Roberts Rules of Order or an equivalent to conduct business in chapter meetings? Does the chapter maintain bylaws and operate according to them? Does the chapter recognize and reward brothers who exemplify positive Sigma Chi values (e.g., annual Brother of the Year award, Leadership Award, etc.)? RITUAL PERFORMANCE Chapter Meetings Number of regular chapter meetings held. Number of regular chapter meetings held using Ritual. Average attendance of brothers at regular chapter meetings. Does the chapter conduct a literary exercise each chapter meeting? Is Ritual performed proficiently? CAMPUS RELATIONS How many brothers are involved with at least one other extracurricular activity? How many brothers are involved with academic honors societies on campus? Did the chapter sponsor or participate in a campus-wide event? Did the chapter receive any special recognition from the institution? Is the chapter s involvement with the IFC sufficient? Total contributions to Children s Miracle Network Total contributions to Sigma Chi Foundation. Community Service How many community service events did the chapter participate in? What was the average number of brothers participating per event? What was the total number of hours served by the chapter this year? What was the per-man average of hours served in community service events this year? Did all brothers participate in at least 2 community service events? Does the chapter have a community service chairman? CONDUCT AND DISCIPLINE Chapter-Level Does the chapter maintain a functioning Judicial Board? (to be answered only by Chapter Advisor) How many incidents did the Judicial Board review last year? Does the chapter have ongoing conduct issues that need to be addressed?
