2 Overview Norfolk-wide recovery focused mental health support starting on 1 st March 2018 Seamless pathway between supported living and community support (including intensive support) Supporting adults aged 18+ with severe and enduring mental health needs 87 people in supported living and 643 people in the community supported at any one time

7 How Decisions will be Made Implementation Steering Group overseeing the mobilisation, transition and implementation phases of the new service Service User Forum providing a collective service user perspective to influence and guide the service Operational Board with representation from Senior Managers across the partnership, ensuring the quality and performance of the service Partnership Board with expertise and experience from a range of stakeholders, providing steering on the overall strategy & supporting the development of the service

9 The Integrated Staff Structure Single integrated locality-wide teams working across supported living and community support A staff structure has been modelled to provide effective, flexible support across each locality. The proposed staff structure will be confirmed through measures as part of the TUPE transfer process and undertakings under TUPE.

13 Hours of Delivery Supported living will be staffed across the week, weekends and overnight, as a 24 hour service (with sleep-ins) Community support will be available from 8.30am to 8pm, including flexible evening and weekend working to meet the needs of service users

14 Person-Centred Support & Navigation Two or three staff members will build a relationship with the service user to promote engagement and to provide a wide range of expertise. One will be the identified Support Navigator. Recovery/Psychological education support, mental health service engagement, dual diagnosis support, recovery plans Recovery Worker (Recovery Orientation) Social Inclusion Recovery Worker (Social Inclusion Orientation) Peer Supporter (Peer Orientation) Providing additional support e.g. for connecting the service user to healthy living activities and community resources, or using specialist expertise to tackle difficult benefits issues Providing additional support e.g. for building engagement, or for building confidence in navigating and accessing statutory services
