2 STAFF RECOGNITION AWARDS 2018 WELCOME Cardiff and Vale University Health Board s mission is Caring for People, Keeping People Well. At the heart of this is providing excellent patient care, but this would not be possible without our incredible staff. The Staff Recognition Awards is a key event in our calendar and is our way of highlighting the amazing achievements of our staff over the past twelve months. The Staff Recognition Awards is now in its 7th year and we will be inviting nominations across 16 categories. This year we are introducing two new categories, the first is the Acting Today for a Better Tomorrow award which focuses on our work with the Well-being of Future Generations Act The second is the Living Our Values award which will highlight the work which has taken place around the UHB Values and Behaviours Framework. We are looking to hear from you about teams and individuals working across the service who are examples of good practice and deserve recognition for their achievements. Only by recognising these successes will it inspire others to do the same. Entries should be submitted online or by post, but must be received by 5pm on Friday 15th December You are able to submit nominations for as many projects, individuals or teams as you want. However, each project, individual or team can only be entered into one category. Please visit the Staff Recognition Awards internet page for further information on judging, rules of entry and frequently asked questions (FAQs). If you require this in an alternative format such as large print, Braille or Welsh please contact Emma Bendle on or Emma.Bendle@Wales.nhs.uk There is a wide variety of categories which reflect the many ways in which we care for people and keep them well. Each year the number of nominations and our celebration event has grown. Lets make it even bigger and nominate individuals who are making an outstanding contribution to the patients and communities that we serve. Maria Battle Chair S The staff recognition awards are one of the most uplifting events of the year. It s a time when we can stop and celebrate the excellent work of staff and volunteers in the Health Board. I m delighted to announce the opening of the 2018 awards and would encourage everyone to nominate colleagues they believe are deserving of recognition.

3 AWARD CATEGORIES FOR EXCELLENT PRACTICE:- The award categories reflect the size and diversity of the UHB s activities and the many and varied ways in which staff at all levels across the organisation contribute to high quality patient care and service delivery on a local, national and international level. Acting Today for a Better Tomorrow - The Well being of Future Generations Act requires public bodies in Wales to think about the long term impact of their decisions, to work better with people, communities and each other, and to prevent persistent problems such as poverty, health inequalities and climate change. This Act is unique to Wales attracting interest from countries across the world. The 7 wellbeing goals for Wales are to be: globally responsible, prosperous, resilient, healthier, more equal, with cohesive communities and has a vibrant culture and thriving Welsh language. This award will be given to an individual or team who have demonstrated through the goals above that they have thought about the long term, how to prevent problems before they occur, working with others and involving people in decision making. Chair and Chief Executive s Award - caring for people and keeping them well to recognise an individual who, day in day out, demonstrates the UHB s core values of kind & caring, respectful, trust & integrity and personal responsibility. An award specifically for our Unsung Heroes. The Dr Kate Granger Award for Compassionate Care - This award celebrates the life of Kate Granger who sadly died on Saturday 23rd July 2016 from terminal cancer. During a hospital stay in August 2013 with post-operative sepsis, she made the stark observation that many staff looking after her did not introduce themselves before delivering her care. She felt that it was incredibly wrong that such a basic step in communication was missing. This is how the #hellomynameis campaign was born. The Kate Granger award will be awarded to an individual or team from the UHB who has made a fantastic difference to patient care. In particular, looking for evidence of: an ambitious and innovative way of delivering care high-quality management and leadership an approach that can be easily measured and have a real impact how the approach has made a difference to patient care how it makes a difference in the long term how easily it can be replicated across the organisation. Education and Development - An individual who has developed through education or training impacting on both at a personal level and in the workplace. (from Essential Skills to PhD) Equality, Diversity and Human Rights - an individual who has recognised the individual needs of patients/colleagues and taken action to value diversity and promote equality and human rights. Going the Extra Mile - An individual or team who are going above and beyond what is expected of their role in such a way to bring that extra special something to patients, colleagues or the wider community. Health at Work and Wellbeing - an individual who has made positive changes to their life style in order to improve their health; and/or has supported others to promote a healthy working environment for their colleagues. Leadership - an individual who has demonstrated a positive difference through their leadership. This includes encouraging, supporting and inspiring others. This person may not directly manage staff. Living Our Values Award - It is not just what we do, but the way we do things that is important to our patients, visitors and staff. We hear great stories everyday about how staff across the organisation are making a positive impact on the lives of others. These staff live and breathe the values and behaviours that we as an organisation agreed through the Values into Action project. The 4 values are: Kind and Caring, Respectful, Trust & Integrity and Personal Responsibility. This award will recognise an individual or team who day in and day out, lives and breathes these values to give the best possible experience to patients, staff and the wider community. Manager of the Year - an individual who has demonstrated a positive difference through their management. This includes encouraging, supporting and inspiring others. Patient Experience - an individual who has made a positive improvement(s) to the patient/client/carer experience i.e. demonstrates respect, care kindness etc. People s Choice Award - Each year health board staff touch the lives of more than a million people from Cardiff, the Vale of Glamorgan and across Wales. Every day they show care, kindness, dedication and work tirelessly to make people feel better and live longer. They give so much, this is an opportunity to give something back in a small way that would mean so much. If you know a staff member who has made a difference to your life or inspired you by what they do, take a few minutes to show them that it s not gone unnoticed. Show them you re grateful. Show them you re proud. Make them your People s Choice nominee for Quality, Sustainability and Efficiency - an individual recognised in this category will have generated effective ideas for improving efficiency; and/ or have implemented changes that maintain/improve safety/quality of patient care/service delivery whilst improving efficiency and value for money. Research and Development - to recognise staff involved in research and/or development which has impacted positively on patient care and the wider community. The nomination should reflect any development, publications and presentations as well as data outcomes. Volunteer of the Year - a volunteer or team of volunteers who have made a positive difference to the patient/client/carer/staff experience on behalf of the wider community. Welsh Language - an individual who has provided quality bilingual healthcare or has developed an idea to improve the quality of bilingual health for Welsh speaking patients.

4 RULES FOR ENTRY A nominee can be any employee or volunteer of Cardiff and Vale University Health Board. A nominator is any member of staff or volunteer of Cardiff ad Vale University Health Board. Entries must be submitted by 5pm on Friday 15th December Please note any nominations received after this date will not be counted and will be discarded. Prior project submissions will not be accepted un less there has been significant changes in the past 12 months. Please ensure nominations meet the essential assessment criteria which is outlined below. Please complete the nomination in full including contact details for nominees and nominators. Please note nominators are not automatically invited to attend on the night. Please ensure your nominations is within the word limit of the individual sections. You can enter as many projects, teams or individuals as you wish, but do not enter the same in multiple categories. The Judges reserve the right to move a nomination to another category if appropriate. In the event that there are no nominations in a particular category that reach the criteria, the judging panel reserve the right to not select a winner regardless of the number of entries. The Judges decision is final. TIPS FOR WRITING A GOOD NOMINATION If you have never written a nomination and are unsure where to start please visit the Tips for Writing a Good Nomination page which can be found on the Staff Recognition Awards internet page. JUDGING PANEL A panel of senior staff including Executive Directors will judge all nominations. Each category will be marked by a minimum of two judges. Marks will be given according to the criteria. The scores at this stage are indicative of the best nominations but are used only as an important guide in the final decision of who are the winners. Runners Up will be awarded at the judge s discretion. Due to the increasing popularity of the event it may be necessary to initiate a short listing process for each award. If this should occur we cannot guarantee that all nominees will be invited to the event. Dependent on the level of nominations, nominees will be invited to attend the Staff and KlearRecognition Awards on Friday 9th March The lucky winners and runners up will be announced on the night and will be invited to accept their awards on stage. ASSESSMENT CRITERIA All submissions must include the following criteria: Written in plain English and easy to follow with no acronyms. This individual or team has made a difference during the last 12 months. Patients / service user benefits How they have im proved our services or patient care. Service Improvements How they have improved a service using new ways of working. Achievements Personal or at work. UHB Values where the individual has provided a good example for others to follow i.e. kind & caring, respectful, trust & integrity and personal responsibility. Wellbeing & Engagement including and consulting staff in for example the process of change, service improvement, or day to day management of an area, taking account and caring for the health and well being of the staff. Incorporating savings or cost benefit Specific to the Quality Sustainability and Efficiency award only How they have improved a service for our patients and the community whilst reducing cost.

5 Cardiff & Vale HealthCharity ElusenIechyd Caerdydd a r Fro Cardiff & Vale Health Charity is the official charity supporting all the work of Cardiff and Vale University Health Board. The Charity holds 300+ endowment funds one for nearly every ward and department and a general purpose Make It Better Fund. Proceeds from the Staff Lottery go into the Make It Better Fund, which is allocated to projects that support staff and patients by the Charitable Bids Panel.
