I assume all risks and hazards incidental to such participation, including transportation to and from the activities; and I do hereby waive, release, indemnify, and agree to hold harmless TYFA, the OMYFA, and all Franchises of the OMYFA, organizers, sponsors, coaches, supervisors, officers, directors, administrators, volunteers, medical professionals, participants, and any persons transporting my child to or from the activities from any and all claims, liabilities, and causes of action related to or arising out of my child s participation. 2 Volunteering and Fundraising Acknowledgement VOLUNTEERING The undersigned parent/guardian of, a registered player/cheerleader with the Troy Youth Football Association (TYFA) hereby understands and agrees that all member families are required to volunteer for various posts/duties throughout the organization and football/cheer season. I agree to a $125 deposit for volunteer commitments. I understandthatthisdepositwillbereturnedatendoftheseasonuponcompletionofthefour(4) mandatory volunteer commitments and upon the return of all football/cheer equipment issued to the participant during their participation period minus any garments they are allowed to keep. Failure to fulfill volunteer commitment requirements will result in the loss of my deposit, the loss of Returning Player rights the following season and could lead to expulsion from the TYFA. Volunteer commitment deposit due 5/19/18. Volunteer commitment waived if I bring in a sponsor at the Community Sponsor, Player Development Sponsor, or Presenting Sponsor levels. 3 Equipment / Refund Policy As Parent/Guardian of (print name) I understand by signing this document I am agreeing to the following conditions: Up to June 1 st = Full Refund June 1 st to July 31 st = Refund less $75 (equipment fee) August 1 st to End of season = No refunds given I agree that Troy Youth Football Association (TYFA) and the Oakland Macomb Youth Football Association (OMYFA) cannot guarantee or cover costs associated with any untimely injury that results in my player/cheerleader sitting out the entire season. I also agree to return all TYFA property/equipment in my possession to the Equipment Manager at the announced Equipment Turn-In date following the season. In the event of a participant s resignation or expulsion I agree to return the equipment within 24 hours. This includes all football/cheer equipment issued to the participant during their participation period minus any garments they are allowed to keep. Failure to return TYFA issued property/equipment will result in legal action for which I will assume all associated legal costs. 4 Participant Code of Conduct 1. I will treat each participant, opposing coach, official, parent and administrator with respect and dignity. 2. I will do my best to learn the fundamental sills, techniques, and strategies associated with my activity. 3. I will become thoroughly familiar with the rules of Troy Youth Football Association (TYFA) and the Oakland Macomb Youth Football Association (OMYFA), youth cheerleading and /or youth football. 4. I will become familiar with the objectives of TYFA and the OMYFA. 5. I will strive to achieve the objectives of TYFA and the OMYFA and communicate them to my coaches and parents. 6. I will uphold the authority of the officials who are assigned to the games in which I play and will assist them in every way to conduct them fairly and impartially. 7. I will conduct myself in practices as well as games so that all participants have an opportunity to improve their skill level though active participation. 8. I will cooperate with the administrators of TYFA and the OMYFA in enforcement of rules and I will report irregularities that violate sound competitive practices. 9. Any participant caught smoking, drinking alcohol, or using illegal drugs will be immediately dismissed from participation in TYFA and the OMYFA. 10. Fighting and horse playing will not be tolerated and may make me ineligible to participate in games. 11. Unexcused absences from practice may make me ineligible to participate in games. 12. Swearing of any kind during practices or games may make me ineligible to participate in games. 13. Abuse of playing equipment (throwing helmets, etc.) may make me ineligible to participate in games. 14. I understand that if I am found in violation of any of the above that I or my franchise or both may incur some disciplinary action. TYFA shall have the first right of disciplinary action but the OMYFA s board of directors may also initiate action. Such action could be in the form of admonition, probation, temporary or permanent suspension, a fine or any combination thereof. 5 Parent / Guardian Code of Conduct The Troy Youth Football Association (TYFA) is a Youth Organization with a focus on good sportsmanship. It s for this reason that we have adopted a Zero Tolerance Policy. This policy will emphasize the importance of proper speech, behavior and actions by all those attending any TYFA or Oakland Macomb Youth Football Association (OMYFA) function and will not tolerate anything less. Proper conduct guidelines are listed below. 1. I will treat each participant, opposing coach, official, parent and administrator with respect and dignity. 2. I will conduct myself at practices as well as games so that all participants have an opportunity to improve their skill level through active participation. 3. I will refrain from any improper behavior before, during or after a TYFA/OMYFA event (horseplay, lack of courtesy, unsportsmanlike conduct, etc.) 4. I understand that there is no use of tobacco, drinking alcohol, or using illegal substances in / around TYFA/OMYFA activities. 5. I will not damage or deface any property while at any TYFA/OMYFA event. 6. I will avoid the use or profanity, gestures or derogatory comments as well as negative comments towards any TYFA/OMYFA player, coach, board member, official or TYFA/OMYFA executive board member. 7. I will refrain from any physical contact from a person or an object to another person. No physical contact uninvited will be tolerated by anyone anytime. 8. I will refrain from using negative comments in reference to the OMYFA, any OMYFA franchise or any individual OMYFA franchise member in any social media network i.e. Facebook, Twitter, texting etc.. 9. I will wait for a minimum of 24 hours following games and practices to contact the coaches and or Executive Board with feedback. I agree that if I fail to abide by the Zero Tolerance Guidelines mentioned, I / my guest / my child will be subject to the following action(s) as deemed by the OMYFA and TYFA: 1. Verbal warning by an official and / or Head of League Organization. 2. Parent and or Child game suspension with written documentation of incident kept on file by the organization involved. 3. Written warning of any of the infractions. 4. Game forfeit through the Official or Head of League Organization. 5. Parent / Child season suspension (full or part). 6. Any repeat offense, regardless of the severity could invoke and or all of the above penalties. 7. The OMYFA Executive Board as stated in the bylaws will rule on all severe infractions. Until that time the parent and or child will be put on suspension until punishment is rendered. I agree that if I fail to abide by the Zero Tolerance Policies mentioned, I / my guest / my child will be subject to suspension or expulsion by the TYFA or the OMYFA. This policy is to be signed by all parents / guardians who have children participating within the TYFA. 6 Media Release Form During the course of a season many of our football players and cheerleaders will be photographed during games and/or practices by parents or by the Photography Company we employ during the season. We may on occasion ask for some of these images to help us promote the Troy Youth Football Association on our website ( Facebookpage, official documents, or our end-of-season yearbook. By signing below, you are giving the Troy Youth Football Association permission to display images of your child on their website, Facebookpage, Yearbook, or other official documentation indefinitely. The Troy Youth Football Association has my permission to use my or my child s photographs and/or video images publically to promote the organization. I consent to the use of his/her photograph in connection with the production, exhibition, distribution or other use of any photographs or motion video. I understand that the images may be used in print publications, online publications, presentations, websites, and social media.
