3 PURPOSE OF THE POSITION Peer support is based on the concept that people who have had a lived experience of mental illness and have experienced recovery, with specialised training, can act as recovery coaches to others (peers) and can intentionally engender hope by role-modelling recovery in a way that others without experience of mental illness cannot. Peer Support Specialists (PSS) are members of the multi-disciplinary team (MDT). PSS is a unique role with specific training and workforce infrastructure, including supervision and on-going professional development. PSS share personal experience of recovery in a way that inspires hope and encourages people to recognise that they are able to determine their own wellbeing. The peer support relationship is based on mutual respect, through shared experience and understanding. Peer support specialists work within an agreed model of practice. PSS share wellness skills and strategies that may support people to achieve their recovery and wellbeing goals. The purpose of the position is to provide individualised support with the goal of recovery, role modelling of hope for recovery, encouraging self-awareness and self-determination, including the development of participation and natural supports within peers own communities. PSS work to enable service users to be partners in their care. Peer support necessitates the development of strong supportive relationships based on mutual respect and shared understanding. 4 NATURE AND SCOPE OF RESPONSIBILITIES Key Accountabilities RECOVERY COACHING To be a positive role model for hope by using own life stories. Ensure people become more active participants in their own recovery process by demonstrating the five Pathways to Recovery when utilising the peer support approach: Hope Choice Empowerment and self-determination Recovery environment Spirituality, meaning and purpose. COMMUNICATION To establish, develop and maintain a supportive relationship with peers and their families/whaanau, recognising the diversity of people s culture and providing support that is culturally responsive, sensitive and appropriate. Standards / Achievements Provide direct (1:1) peer support to peers, including: connecting with peers and sharing own personal experience of recovery in a way that inspires hope, motivation, selfdetermination and empowerment; assisting peers to identify what works best for them, and exploring ways they can develop resilience and become more active participants in their own recovery process; supporting peers to believe in themselves and to maintain that self-belief supporting peers to connect to the community; supporting peers to learn self-advocacy planning and undertaking routine regular contact with peers at their request. Assisting peers with Wellness Recovery Action Planning (WRAP) including the goal planning process and documenting own progress of that plan. Assist peers to develop their own natural supports that will enable them to increase their resilience, confidence and engagement in community activities. Inform peers of relevant recovery related information, e.g. maintain knowledge of community information and mental health and addiction resources. Support peers to become fully informed of the available support, resources and the existing networks within the community. Support peers to make meaningful connections with these community resources. Develop productive relationships with peers and the multi-disciplinary team (MDT) that enable the promotion of service user s participation in their own recovery process. 5 Key Accountabilities PERSONAL DEVELOPMENT To take responsibility for personal and professional education and development within the Peer Support Specialist role. PROFESSIONAL ACCOUNTABILITY Standards / Achievements Provide information to the multi disciplinary team (in MDT meetings and also to individual MDT members when requested) from a Peer Support Specialist s perspective. Treat all people receiving services without regard to their background or history placing the peer s needs reasonably above all other considerations. Provide assistance in a concise, informative manner using a positive and respectful tone that reflects the values of CM Health. Adopt a non-judgemental approach utilising reflective listening, appropriate body language, and positive interactions. Document recovery-oriented peer support notes from all contacts with peers in the clinical files on HCC. Demonstrate respectful and caring attitudes and behaviours towards co-workers and peers and their families/whaanau. Complete any relevant follow-up peer support and recovery training (e.g. WRAP) on a regular basis. Participate in regular and ongoing PSS supervision, both individual and group and participate in coaching supervision as required. Attend relevant training or educational programmes to update own knowledge. Gain further skills as required by CM Health including mandatory annual Mental Health & Addiction Services training. Participating in ongoing individual performance reviews by identifying own learning needs, and setting professional development goals to meet these. Demonstrate personal well-being and resilience. Understand the importance of equal opportunity to healthcare access and outcomes from MH&A services. This may require differing levels and types of service provision. Respect, sensitivity, cultural awareness is evident in interpersonal relationships. Honouring Cultural Diversity Our cultural differences are acknowledged by respecting spiritual beliefs, cultural practices and lifestyle choices. UTILISE INFORMATION TECHNOLOGY Demonstrate ability to access and use available clinical information systems. i.e. HCC. Undertake updates and HCC refresher training as required. Is conversant with applications required for specific discipline/role. 7 COMMUNICATION AND INTERPERSONAL SKILLS Peer Support Specialists will be required to interact on a regular basis with a range of Counties Manukau Health staff members including all members of the multi-disciplinary team (MDT). Externally there will be contact with: Service user and consumer groups Family/whaanau Informal support networks and community groups WRAP Facilitators Community Living Services Community Support Workers Housing and Recovery facilities General Practitioners All other relevant health professionals and agencies involved with the service user or his/her support networks. Situations may often call for tact, diplomacy, and will require information to be handled in a discreet and sensitive manner. In conflict situations, will be required to exercise sound judgement, negotiation and persuasiveness skills, toward facilitating a workable outcome agreeable to all involved. PROBLEM COMPLEXITY The Peer Support Specialist will be regularly challenged by a wide range of complex professional Mental Health & Addiction issues. There will be a requirement to be able to prioritise issues and negotiate time frames, while still providing a quality customer service. The range of problems will be diverse and require solutions customised to meet the circumstances of the person receiving services. Therefore opportunities will arise to invite innovative approaches to the peer based on proven outcomes and in consultation with the care team. There will be demands to meet deadlines, maintain accuracy, quality and privacy of information. Failure to do so could impact on the organisation. The Peer Support Specialist will refer to their Team Manager of the MH&A service and/or Professional Leader Peer Support Specialists for advice or a second opinion. SCOPE FOR ACTION Peer Support Specialists are empowered to make decisions or recommendations relating to the recovery and well-being of peers they are working with - in consultation with the person receiving services - and in collaboration with the relevant clinical team. Peer Support Specialists are encouraged to use initiative and problem solving skills to develop innovative approaches to issues. Position description: Counties Manukau Health 7
8 Discretion is required to be exercised in releasing confidential information to the appropriate parties as per CM Health guidelines and policies. This position description is not exhaustive and the incumbent may be requested to perform any reasonable task within the scope of the position as requested by the Team Manager/Clinical Team Coordinator and/or Professional Leader. This position description will be reviewed from time to time in consultation with the incumbent. DIMENSION OF THE POSITION Responsible to the Team Manager Community Mental Health Centre or Specialist Team for the provision of Peer Support Specialist services within Counties Manukau Health as outlined in this Position Description. Peer Support Specialists will be full members of the multi-disciplinary team (MDT) within clinical services but are not key workers or mental health clinicians. Access to peer support services will be driven by service user choice at their request. Peers can themselves approach Peer Support Specialists (PSS) or they could ask someone to arrange for a PSS to contact them. Service users will have one primary Peer Support Specialist as their contact. PSS will work with allocated caseloads. These caseloads would attempt to be the best fit for the consumer receiving the service. POSITIONS REPORTING IN THIS ROLE Directly : Indirectly : Nil Nil PERSON SPECIFICATIONS Minimum Qualification Peer Support Specialist Certificate (CM Health approved) Completed personal WRAP training A full NZ Driver s Licence with no limitations Experience Personal experience of recovery from mental illness and/or alcohol and other drug issues Knowledge of The Code of Health and Disability Services Consumers' Rights Preferred Secondary/Tertiary level education WRAP Facilitator Certificate Knowledge of Mental Health Legislation and other relevant legislation e.g. Human Rights, The Privacy Act, Vulnerable Children s Act Position description: Counties Manukau Health 8
9 Skills/Knowledge/ Behaviour Minimum Understanding of and commitment to the principles of a recoveryfocused approach to mental health Passion to support others in their recovery Ability to motivate peers and to model a hopeful and encouraging attitude Ability to work in a multi disciplinary team (MDT) and respect the skills and strengths of others in the team Excellent communication skills (especially listening) Ability to use recovery enhancing language Good interpersonal skills with ability to liaise with a broad range of health professionals and community organisations Ability to work alone, make own schedule and balance time. 11 HEALTH AND SAFETY ACCOUNTABILITIES (Management roles) Health and Safety (H&S) is a key accountability of all Counties Manukau Health (CM Health) staff. All CM Health Managers are responsible to: Zero harm Risk management Safety Management System Requirements Work safely Engage in safe workplace practices and manage the health and safety of your employees and other people in the workplace. Engage in safe work practices. Ensure all Incidents involving staff are reported, investigated and any corrective actions are completed and closed off when completed in the Incident reporting system in line with CM Health Policies and Procedures. Ensure all corrective activities are completed and controls are reviewed after incidents. Ensure all staff are Inducted/Oriented to the division/service/work area and recorded in the OneStaff database and participate in Occupational Health and Safety matters. Manage all hazards, risks and incidents in the workplace by identifying, evaluating, reporting and controlling them effectively by maintaining a Hazard and Risk Register that is reviewed and kept up to date on an ongoing basis. Stop work where there is an immediate risk of serious harm and escalate to the appropriate manager. Maintain Occupational Health of employees due to workplace hazardous exposures. Ensure you and the employees understand and follow CM Health s Occupational Health and Safety policies and Health and Safety regulations, what your responsibilities are under the policies and implement these requirements effectively into your work area. Manage the work area s risk profile and how these relate to the corporate risk profile and implement and resource mitigation controls. Engage your employees to participate in Occupational Health and Safety matter. Report for duty in a fit state, free from the influence of alcohol or other drugs. Provide and wear the necessary Personal Protective Equipment for yourself and your staff when undertaking specific activities, or when in nominated areas. Ensure procedures are in place for clinical and plant equipment to be operated safely. Participate in relevant safety training, manage and maintain safety related competencies for yourself and your employees.
