1 March 2018 Dear Corporate Friend: We would very much like for your company to be a corporate member of MDCSCO for The mission of the Maryland and District of Columbia Society of Clinical Oncology (MDCSCO) is: 1) To improve the quality and delivery of care in medical oncology in the state of Maryland and the District of Columbia. 2) To promote appropriate standards of care in oncology. 3) To cooperate with investigators conducting cancer research. 4) To assist in the continuing education of oncologists. 5) To educate the public, the government, insurance carriers, and other health care providers about appropriate prevention of cancer and current therapeutic options for patients with cancer. 3 Corporate Membership 2018 The MDCSCO Corporate Membership Program has five distinct levels, with specific rights and benefits commensurate with each level. Corporate memberships run for one year from the start date of the membership, and may be renewed or modified annually. Corporate membership is open to any company that has a connection with oncology or the practice of medicine. The levels of membership, annual dues, and benefits for each level are listed below. All corporate members receive the following benefits. Logo/listings are by membership level. Silver Members - $2,500 Core Benefits Participation in general membership meetings (up to 6 attendee credits) Guidelines for Corporate Members The number of free attendees at general membership meetings is limited by membership level. Additional tickets at $175 per person. The ticket fee is waived for one meeting for up to two people for a prospective new corporate member. Slots may not be carried over beyond the members current membership year. Attendee credits may be used at any of the six membership meetings per year.

5 Exhibitors may give out literature at exhibits, but may not give any items of substantive value at a level that will require Sunshine Act reporting. This provision does not apply to exhibitors excluded from the provisions of the Sunshine Act. The price of exhibit space is $800 per 6 foot table per event, but is included at no charge as part of corporate membership packages. Corporate members may not attend sponsored events hosted by other corporate members. These events are only open to Professional members. The Industry Spotlight program is offered only for higher level corporate Members (Platinum-$10,000 and above). The event will be attendees by MDCSCO leaders. A corporate member may send no more than 3 people to present and the length of the presentation are limited by membership level. Full or partial general membership meeting sponsorships are available on a case by case basis. OPPORTUNTITY FOR GENETIC TESTING COMPANIES AND NON- PHARMACEUTICAL COMPANIES Non-pharmaceutical companies may host board meetings (limited availability). The host must cover the cost of the meeting and may give a brief overview of their product or service. Corporate membership is not required to host a Board meeting. Exhibit space is also available to these companies at $800 per table and meeting attendance credits are available for $175 per person per event.
