2 Celebremos La Vida! Midwest Style Program Implementation at the Family Health Partnership Clinic Clinic Overview The Family Health Partnership Clinic has been providing primary health care to the uninsured and underinsured of McHenry County, Illinois, for over 12 years. This is accomplished through the efforts of 24 volunteer physicians, 2 contractual nurse practitioners, a dedicated staff and many other community members and volunteers. The Clinic is not a Federally Qualified Health Center, receives no federal or state grants, and does not take Medicaid. The Clinic raises its funds through community support, foundations, and some fee for service agreements. The annual budget is over $900,000 per year. McHenry County, Illinois, is one of the fastest growing counties in the nation and one of the counties in Illinois with greatest increases in Latino population since While overall the county seems to enjoy relative prosperity, the high incomes for some families masks pockets of poverty that exist. Because the county does not have a population center, poverty is very spread out. The Family Health Partnership Clinic is a leader in providing services to reverse health disparities. It is the only health care provider in the county for the uninsured and has developed and led the Breast Cancer Task Force to help broaden access as well, funding these services through various fundraisers. It has an excellent relationship with many local funders and community leaders, ensuring that support for the Clinic and its programs particularly in early detection and screening services remains strong. Program Status and Accomplishments With the financial and technical support of the Prevent Cancer Foundation, the clinic launched Celebremos La Vida! in May 2004 in conjunction with Centegra Health System. At Centegra Memorial Hospital in Woodstock, the Family Health Partnership Clinic (FHPC) takes over the Women s Center the second Tuesday of every month, and utilizes the exam rooms to provide pap smears, and sends the women directly across the hall for their mammogram. FHPC does all of the scheduling for the patients, and Centegra is responsible for all mammography services. FHPC additionally coordinates follow-up care through a variety of medical groups, including a portion of follow-up care through Centegra. Each month, around 25 women participate in the Celebremos Day, averaging over 275 each year. These women receive clinical breast exams, mammogram, health education, and a goodie bag with toiletries donated by an area company. Additional services are provided to the women including spa-like services and healthy food and drinks to encourage the festive atmosphere. None of the women have insurance, and none of the women have a primary care provider. This program enables women (and their families) to be absorbed into the regular FHPC system, so that regular care can also be provided. Recruitment Efforts Word-of-mouth continues to be an excellent recruitment tool. Many of the women coming to the CLV clinics are return patients, coming for their annual exams. Additional recruiting takes places at churches, through temp agencies, presentations at networking

3 meetings, and presentations at ESL (English as a Second Language) classes at the local community college. Unanticipated Outcomes This program has been very positive for both the Clinic and the hospital system. It has allowed the hospital to interact with a patient population that it normally sees only in the emergency department and has resulted in the hospital s becoming more aware of the need for bilingual services, information and signage throughout the system. Many of the patients and their families have begun to use the services of the Family Health Partnership Clinic for their regular primary care, and this has improved the health of the patients. Lessons Learned 1. Develop a good relationship with your mammogram provider. FHPC contracts with the hospital and pays for the mammograms out of dollars generated through a breast cancer walk fundraiser held each October. 2. Make sure everyone at the hospital is on board with the program, including the front desk. There will be lots of traffic that day, so make sure all front line staff know about the program. The first day of Celebremos, we had a massage therapist come. The hospital front desk did not know about it, so sent him off to the nursing office to give massages, because they thought it was a Nurses Week present! 3. Ensure you have a good system for follow-up; both for the results follow-up and the patient follow-up. Hospitals sometimes forget to send the results to the Clinic, patients sometimes they forget that they have an appointment. Reminders for everyone are key! 4. Enlist the support of a local company to do a supplies drive. This is a great way to build a relationship with a company because it s the employees who donate their travel sized shampoos, soap, etc, rather than the company making a financial commitment. The Clinic packages them into little goodie bags and hands them out to all the patients. 5. Involve the local extension agency. Many have nutrition educators who can help provide information and demonstrations during the day. 6. Always have food. But you knew that already, didn t you??? 4 Celebremos la Vida! A National Program Created by the Prevent Cancer Foundation Since 1994, Celebremos la Vida! (Celebrate Life!) has provided breast and cervical cancer education, screening and early detection to medically underserved Hispanic women in the Washington, DC, metropolitan area and McHenry County, Illinois. The program, sponsored by the Prevent Cancer Foundation, provides life-saving services to women who are unable to access other breast and cervical cancer screening and education programs. Celebremos Program Locations: Spanish Catholic Center, in collaboration with the George Washington University Mammovan, two locations in Washington, DC and Langley Park, MD Georgetown University - Lombardi Cancer Center, Washington, D.C. Family Health Partnership Clinic, McHenry County, Illinois Celebremos la Vida! ensures that participants receive comprehensive education, screening and follow-up in a setting that is culturally appropriate and nurturing. This environment encourages women to return for yearly check-ups, take care of their health and spread the early detection message to other women in their family and community. Each of the three clinics provides monthly breast and cervical cancer screening and education for Hispanic women. Follow-up for abnormal tests and patient navigation is provided by each site s bilingual and bicultural coordinator. If breast or cervical cancer is detected, each partner works with collaborating medical centers to ensure medical follow-up and treatment at no cost. Over 98 percent of participants in a 2007 patient satisfaction survey said they planned to return to the program and that staff and volunteers were helpful and communicated very well in their preferred language. Additionally, the majority of participants said that they would recommend the program to a friend and that they learned a lot about cancer prevention during their visit. Results: Since its inception, the Celebremos program has provided more than 3,900 screening and diagnostic mammograms and more than 4,200 Pap tests. 5 Overview of the Prevent Cancer Foundation The Prevent Cancer Foundation is the only national, nonprofit health foundation whose primary mission is the prevention and early detection of cancer through scientific research, education and community outreach. The Foundation focuses on those cancers including lung, breast, prostate, colorectal, cervical, skin, oral and testicular that can be prevented through lifestyle changes or detection and treatment in the early stages. Since 1985, the Prevent Cancer Foundation has provided more than $97 million in support of cancer prevention research, education and outreach programs nationwide and has played a pivotal role in developing a body of knowledge that is the basis for important prevention and early detection strategies. Our Mission: The mission of the Prevent Cancer Foundation (formerly the Cancer Research and Prevention Foundation) is the prevention and early detection of cancer through research, education and community outreach to all populations, including children and the underserved. Outreach and Programs: Research Grants and Fellowships: The Foundation has provided funding for cancer prevention research and training to more than 300 scientists from more than 150 of the most prestigious academic institutions and medical centers across the country. Of particular significance is our support of early-career scientists. Community Grants: The Foundation awards grants to support education or services in cancer prevention and early detection, especially to underserved communities, across the U.S. Celebremos la Vida! is a long-standing breast and cervical cancer education, screening and early-detection program for medically underserved Latinas over the age of 40 in Washington, D.C., Northern Virginia and McHenry County, Illinois. Mammovan: The Foundation raises funds to support the George Washington University Cancer Center s mobile mammography facility, the Mammovan, which provides free mammograms to uninsured women in the D.C. metro area. Dialogue for Action: Annual National Colorectal Cancer Conference attracts hundreds of leaders in fields related to colorectal cancer screening. This successful program has been replicated in 13 states. Summit Series on Clinical Trials: Since 1996, the Foundation has served as one of five conveners of an annual workshop on clinical trials. Annual Lung Cancer Workshop: This meeting brings together professionals to explore the use of spiral CT imaging as a tool for evaluating response to new therapies for lung cancer. Save Your Skin Campaign is a skin cancer awareness and sun safety campaign that features an interactive exhibit that uses age-progression photo technology and visits universities across the country.

6 Check Your Insides Out From Top to Bottom Exhibit: This colorful exhibit features hands-on interactive activity stations to teach visitors about preventable cancers as it travels across the country. National Colorectal Cancer Awareness Month is a nationwide public awareness campaign that takes place each March and features The Super Colon, a 20-foot long, 8- foot high, inflatable replica of a human colon. Congressional Families Action for Cancer Awareness is a bipartisan group of spouses of members of the U.S. Congress, Executive Branch and Supreme Court that carries the message of cancer prevention and early detection to people in their home states. State Fair Health Awareness and Screening Booths: Rural populations in seven states are reached with information and health screening services through partnerships between the Foundation and local medical institutions. Children s Initiatives: The Foundation supports pediatric oncology research and support programs for children with cancer through Hope Street Kids and Tracy s Kids.
