1 Health and Social Care Alliance Scotland: People at the Centre Nancy Greig, Network Development Officer, Health and Social Care Alliance Scotland (the ALLIANCE)
2 The Long Term Conditions Alliance Scotland (LTCAS) formed in 2006 to advocate for a shift to asset-based approaches, articulated through the self management agenda. LTCAS became a powerful alliance bringing together a wide membership and building an influential position as strategic partner, and critical friend, of the Scottish Government. Board feels strongly that we now need to look to the future and adapt to the changing policy context. Central to this has been re-naming the organisation.
3 The ALLIANCE s vision is for a Scotland where people of all ages who are disabled or living with long term conditions, and unpaid carers, have a strong voice and enjoy their right to live well, as equal and active citizens, free from discrimination, with support and services that put them at the centre. Over 270 members; mainly third sector organisations, along with statutory and private sector associate members and individuals who use health and social care services. Membership spans long term conditions, mental health, disability and unpaid carers.

5 Strategic partner of Scottish Government Manages 2m a year Self Management Fund for Scotland History of partnership, innovative programme development/delivery, with voice of people at its heart Provides a voice for users of health and social care services at a national policy level Works as a conduit between people and decision makers.
6 Ensure people are at the centre, that their voices, expertise and rights drive policy and sit at the heart of design, delivery and improvement of support and services. Support transformational change, towards approaches that work with individual and community assets, helping people to stay well, supporting human rights, self management, co-production and independent living. Champion and support the third sector as a vital strategic and delivery partner and foster better crosssector understanding and partnership.
7 Change Fund: Enhancing the Role of the Third Sector- programme designed to boost influence and impact of the sector within Reshaping Care for Older People. My Condition, My Terms, My Life self management awareness raising campaign launched First phase focused on raising public awareness and now on health and social care professionals. Self-Directed Support and Health: Building the Connections. Looking at the question of how self directed support and health will interact under new legislation and forthcoming integration.

9 National Person-centred Health and Care Programme- ALLIANCE was invited to lead co-production strand. People Powered Health and Wellbeing Programme Director now in post. Programme Director will drive forward work to help change culture across sectors. ALLIANCE Involvement Network and Dementia Carer Voices Project - part of a wider programme drawing on voice and personal experience
10 The Big Lottery Found is creating a fully independent 50 million trust. Over the next 10 years the Life Changes Trust will deliver transformational change for young people leaving care and people with dementia and their carers. It has been agreed that the ALLIANCE will manage the programme for people with dementia and their carers. The Trust will deliver investment via commissioning and grant processes. One of these strands will be investment in innovations to address the issues faced by people living with dementia and their carers.
11 Delivering Assisted Living Lifestyles at Scale is a UKwide programme funded by the Technology Strategy Board aimed at using technology and innovation to improve care in community settings. Four programmes were identified across the UK one of these being the Scottish collaboration Living it Up. Living it Up is a unique partnership of around 28 organisations across health, social care, the Third Sector, academia, industry, enterprise, media, technology and innovation.

13 Living it Up aims to harness familiar technologies to connect people to the right place, at the right time on the right device for them. There probably are things out there that would make my life a lot easier, it s getting over that fear, isn t it?
14 Community engagement work to date has enabled; Innovative person-centred approaches to telehealth and telecare to be co-designed. Integration of individuals and communities - linking their ideas and experiences - with service provider, industry and technology partners to establish common outcomes. Implementation of new services, approaches and initiatives for telehealth and telecare in Scotland.
15 There are many overlaps and connections with other ALLIANCE work: Self management projects My Condition, My Terms, My Life ALISS Change Fund: Enhancing the Role of the Third Sector Life Changes Trust People Powered Health and Wellbeing Self Directed Support.
