1 NAMI Georgia Awards & Recognition 2016/2017 NAMI Georgia seeks to recognize the individuals, affiliates, volunteers, partners, legislators, and organizations who have worked tirelessly to support NAMI Georgia s mission of promoting recovery and optimizing the quality of life for Georgians with mental illness and their families through support, education, and advocacy, and research. The recipients of these awards will be recognized and honored at the NAMI Georgia Annual Meeting on Saturday, April 22, Individuals will also be recognized on the NAMI Georgia website and social media. For individual awards, any member, non-member, or affiliate may nominate someone for an award. Self-nominations are also welcomed and accepted. We strongly encourage each affiliate to nominate an individual in each award category and also to self-nominate their affiliate for each of the affiliate award categories. The sharing of stories and successes has more long-term importance than the actual awards given in any particular year. DEADLINE FOR NOMINATIONS: 5:00 pm on Friday, March 31, 2017 Nominations should include (but are not limited to): 1. Nomination Form Incomplete submissions will not be automatically rejected, but lack of information will naturally affect the outcome. 2. Narrative The narrative should explain in detail why an individual or group should be recognized and honored for a particular award category. 3. Data We highly recommend including the data (quantitative as well as qualitative) with the nomination. NAMI Georgia will do our best to obtain data, but it is ultimately the nominator s responsibility. 2 NAMI Georgia Affiliate Awards Categories for Consideration 2016/2017 l NAMI GA Affiliate of the Year Advocacy l NAMI GA Affiliate of the Year Membership l NAMI GA Affiliate of the Year Support l NAMI GA Affiliate of the Year Education l NAMIWalks Affiliate Fundraiser Award l NAMIWalks Affiliate Participation Award NAMI Georgia Individual Awards individuals are not required to be a member of NAMI NAMI Georgia Legislative A-Team Members This recognition is given to state and local legislators in appreciation for taking the lead to improve the Georgia mental health support system. NAMI Georgia Journalism Award This award recognizes a journalist who has written several impactful articles during the year highlighting 1) the problems that mental health faces in a broad sense, 2) the issues facing those living with mental illness, and 3) the success many Georgians have in living with mental illness. The award is given to the journalist who is impactful in each area and presents a balance of all three. NAMI Georgia Exemplary Psychiatrist Award This award allows NAMI members and NAMI Affiliates throughout the state to honor the exceptional contributions that a psychiatrist has made to improve the lives of people living with mental health conditions. NAMI Georgia Exemplary Therapist Award This award allows NAMI members and NAMI Affiliates throughout the state to honor the exceptional contributions that a therapist has made to improve the lives of people living with mental health conditions. NAMI Georgia Exemplary Certified Peer Specialist Award This award allows NAMI members and NAMI Affiliates throughout the state to honor the exceptional contributions that a CPS has made to improve the lives of people living with mental health conditions. NAMI Georgia Member Awards individuals must have a NAMI membership in good standing NAMI Georgia Volunteer of the Year Award This award recognizes the individual who gives of their time and talent to NAMI Georgia selflessly to advance the organization s mission to promote recovery and optimize the quality of life for Georgians with mental illness and their families through support, education, and advocacy, and research. IOOV presenters may be eligible for this award. NAMI Georgia Connection Facilitator of the Year This award recognizes the individual who goes above and beyond to facilitate Connection support groups while maintaining fidelity to the model. NAMI Georgia Family Support Group Facilitator of Year This award recognizes the individual who goes above and beyond to facilitate Family support groups while maintaining fidelity to the model. NAMI Georgia Peer-to-Peer Mentor of the Year This award recognizes the individual who goes above and beyond to facilitate Peer-to-Peer mentor classes while maintaining fidelity to the model. NAMI Georgia Family-to-Family Teacher of the Year This award recognizes the individual who goes above and beyond to facilitate Family-to-Family mentor classes while maintaining fidelity to the model. NAMI Georgia Hall of Fame This award recognizes a member whose efforts over time have made them stand out among heroes. These efforts can be in any, and usually are in many areas, that cannot be quantified. This is an all around award for excellence on behalf of NAMI Georgia. CIT Awards

3 CIT Officer of the Year Joey Award This honor is awarded to the law enforcement officer who possesses all the characteristics necessary to be an effective CIT officer and always tries to use his/her CIT skills first in any situation. Exemplary Family Member Award This honor is awarded to the family member who has dedicated much of his/her time to the CIT program by attending trainings and/or CIT meetings in his/her area on a regular basis. De-Escalation Trainer Award This honor is awarded to the CIT de-escalation trainer who is consistently available to aid in the instruction of de-escalation techniques at various CIT classes in his/her area or around the state of Georgia. Most Creative Role-Player This honor is awarded to the CIT role-player who generates the most creative role-play scenarios which are able to keep the students interested in the role-playing exercises. Recovery Advocate of the Year Award This honor is awarded to the consumer who has the courage to share his/her personal story openly and freely with law enforcement officers at various CIT trainings in an effort to help others better understand the effects of a mental illness. Coordinator of the Year This honor is awarded to the local CIT coordinator who demonstrates the ability to effectively organize and coordinate a 40-hour CIT training and is able to do so several times a year. (NAMI or Law Enforcement Coordinators are eligible.) Exemplary Faculty Member This honor is awarded to the faculty member who consistently offers his/her expertise to the CIT program through instruction and/or learning materials. Choice Site Visit Award This honor is awarded to the facility that provides exemplary services to consumers, has consistently hosted site visits for local CIT classes, and has promoted an environment for positive and constructive dialogue between law enforcement officers, consumers and providers. Gold Partnership Award This honor is awarded to the agency that provides exemplary support to the Georgia CIT program by providing faculty and/or coordinating site visits for classes. Leadership Award This honor is awarded to the agency executive who possesses exemplary leadership qualities which he/she utilizes to advance the CIT program.
