2 IVF Egg Sharing with anonymous recipient COMPLETE PACKAGE S An all-inclusive package for women who meet the egg donor criteria, need IVF or ICSI treatment and choose to share their eggs with an anonymous recipient, offering great value IVF, peace of mind and no hidden costs. Fees include: Medical consent treatment consultation and nursing consultations Screening tests for sharer and male partner if applicable Stimulation cycle for sharer Semen preparation of partner sperm if applicable Egg retrieval Attempted fertilisation of at least 4 eggs Embryo transfer Pregnancy scan or follow up appointment within 3 months Also included: All drugs for sharer ICSI if required Blastocyst culture of embryos Embryo freezing and storage for one year Frozen embryo transfer if not pregnant from first cycle (to be taken within 6 months) HFEA Fee 80 Fees exclude: Donor sperm if required 800 Embryoscope 350 Cancellation Policy: Sharer withdraws consent to donate after medical consult, but prior to stimulation 500 refund Cycle cancelled after stimulation due to insufficient response to stimulation 200 refund No eggs collected or failed fertilisation no refund Sharer withdraws consent to donate after stimulation no refund and full cost of IVF/ICSI plus drugs payable A minimum of 8 eggs is required for the donation to take place. If less than 8 eggs are retrieved, the sharer keeps all the eggs and the donation is cancelled (at no extra cost) There is no refund if any part of the complete package is not used
3 IVF Egg Sharing with anonymous recipient ACCESS PACKAGE S2 500 An affordable basic package for women who meet the egg donor criteria, need IVF or ICSI treatment and choose to share their eggs with an anonymous recipient, offering access to IVF at a fraction of the usual price. This may appeal to women who are either not eligible for NHS funded treatment or who have had an unsuccessful NHS funded cycle. Fees include: Medical consent treatment consultation and nursing consultations Screening tests for sharer and male partner if applicable Stimulation cycle for sharer Semen preparation of partner sperm if applicable Egg retrieval Attempted fertilisation of at least 4 eggs Embryo transfer Pregnancy scan or follow up appointment within 3 months Also included: All drugs for sharer HFEA Fee 80 Fees exclude: Donor sperm if required 800 (use of donor sperm may require ICSI, but there is no additional charge for this) ICSI if required (low cost for sharers) 500 Blastocyst culture of embryos 600 Embryo freezing and storage for one year 500 Frozen embryo transfer if not pregnant from first cycle 1350 Embryoscope 350 Cancellation Policy: Sharer withdraws consent to donate after medical consult, but prior to stimulation refund of HFEA fee 80 Cycle cancelled after stimulation due to insufficient response to stimulation no refund No eggs collected or failed fertilisation no refund Sharer withdraws consent to donate after stimulation no refund and full cost of IVF and drugs payable A minimum of 8 eggs is required for the donation to take place. 4 IVF Egg Sharing with a same sex partner or known recipient and an anonymous recipient COMPLETE PACKAGE S An all-inclusive package for women in a same sex relationship who meet the egg donor criteria and wish to donate half their eggs to their partner and the other half to an anonymous recipient. This package is also suitable for women who have found their own egg donor who is willing to share half her eggs with an anonymous recipient. It is a complete package price for IVF treatment and includes all treatment and drug costs for the donor and the known recipient. It offers great value IVF, peace of mind and no hidden costs. 5 Cancellation Policy Continued: Donor withdraws consent to donate to anonymous recipient after stimulation no refund and full cost of IVF known recipient cycle plus drugs payable A minimum of 8 eggs is required for the donation to the anonymous recipient to take place. If less than 8 eggs are retrieved, the known recipient keeps all the eggs and the donation to the anonymous recipient is cancelled at no extra cost There is no refund if any part of the complete package is not used

7 Cancellation Policy Continued: No eggs collected or failed fertilisation no refund Donor withdraws consent to donate to anonymous recipient after stimulation no refund and full cost of IVF known recipient cycle plus drugs payable A minimum of 8 eggs is required for the donation to the anonymous recipient to take place. If less than 8 eggs are retrieved, the known recipient keeps all the eggs and the donation to the anonymous recipient is cancelled (at no extra cost) Up to six sessions of counselling are included whilst you are receiving treatment at Complete Fertility and for up to six months after your last treatment cycle. Additional sessions are available at a cost of 65 per session. Failure to attend counselling appointment 50. Administrative fees Request of copy of medical records 25 Failure to attend medical consultation (if less than 24 hours notice given) 100 Failure to attend nurse appointment (if less than 24 hours notice given) 50 Terms The full package cost is payable at your medical consent treatment consultation. Complete Fertility reserve the right to take payment from supplied credit or debit card details in the event of non-payment of invoices by the due date. These prices are effective from 1st October 2017 and supersede all previous price lists. Prices are subject to change without notice.
