1 HUMAN REPRODUCTION SYSTEM The human reproductive system usually involves internal fertilization by sexual intercourse. During this process, the male inserts his erect penis into the female's vagina and ejaculates semen, which contains sperm. A small portion of the sperm pass through the cervix into the uterus, and then into the fallopian tubes for fertilization of the ovum. Only one sperm is required to fertilize the ovum. Upon successful fertilization, the fertilized ovum, or zygote, travels out of the fallopian tube and into the uterus, where it implants in the uterine wall. This marks the beginning of gestation, better known as pregnancy, which continues for nine months as the foetus develops. When the foetus has developed to a certain point, pregnancy is concluded with childbirth or labor. During labor, the muscles of the uterus contract and the cervix dilates over the course of hours, and the baby passes out of the vagina. Human infants are nearly helpless and require high levels of parental care. Infants rely on their caregivers for comfort, cleanliness, and food. Food may be provided by breastfeeding or formula feeding. The female reproductive system has two functions; The first is to produce egg cells, and the second is to protect and nourish the foetus until birth. The male reproductive system has one function, and it is to produce and deposit sperm. Humans have a high level of sexual differentiation. In addition to differences in nearly every reproductive organ, numerous differences typically occur in secondary sexual characteristics. STRUCTURE FEMALE REPRODUCTIVE SYSTEM The human female reproductive system is a series of organs primarily located inside of the body and around the pelvic region of a female that contribute towards the reproductive process. The human female reproductive system contains three main parts namely the vagina, which leads from the vulva which is the vaginal opening to the uterus, the uterus which holds the developing foetus and the ovaries, which produce the female's ova. The breasts are involved during the parenting stage of reproduction, but in most classifications, they are not considered to be part of the female reproductive system. The vagina meets the outside at the vulva, which also includes the labia, clitoris and urethra and during intercourse this area is lubricated by mucus secreted by the Bartholin's glands. The vagina is attached to the uterus through the cervix, while the uterus is attached to the ovaries via the fallopian tubes. Each ovary contains hundreds of egg cells or ova (singular ovum).
2 The female reproductive system Approximately every 28 days, the pituitary gland releases a hormone that stimulates some of the ova to develop and grow. One ovum is released and it passes through the fallopian tube into the uterus. Hormones produced by the ovaries prepare the uterus to receive the ovum. The lining of the uterus, called the endometrium, and unfertilized ova are shed each cycle through the process of menstruation. If the ovum is fertilized by sperm, it attaches to the endometrium and the foetus develops. Ovaries The ovaries are a pair of small glands about the size and shape of almonds, located on the left and right sides of the pelvic body cavity lateral to the superior portion of the uterus. Ovaries produce female sex hormones such as oestrogen and progesterone as well as ova (commonly called "eggs") which are the female gametes. Ova are produced from oocyte cells that slowly develop throughout a woman s early life and reach maturity after puberty. Each month during ovulation, a mature ovum is released. The ovum travels from the ovary to the fallopian tube, where it may be fertilized before reaching the uterus. Fallopian Tubes The fallopian tubes are a pair of muscular tubes that extend from the left and right superior corners of the uterus to the edge of the ovaries. The fallopian tubes end in a funnel-shaped structure called the infundibulum, which is covered with small finger-like projections called fimbriae. The fimbriae swipe over the outside of the ovaries to pick up released ova and carry
3 them into the infundibulum for transport to the uterus. The inside of each fallopian tube is covered in cilia that work with the smooth muscle of the tube to carry the ovum to the uterus. Uterus The uterus is a hollow, muscular, pear-shaped organ located posterior and superior to the urinary bladder. Connected to the two fallopian tubes on its superior end and to the vagina (via the cervix) on its inferior end, the uterus is also known as the womb, as it surrounds and supports the developing foetus during pregnancy. The inner lining of the uterus, known as the endometrium provides support to the embryo during early development. The visceral muscles of the uterus contract during childbirth to push the foetus through the birth canal. Vagina The vagina is an elastic, muscular tube that connects the cervix of the uterus to the exterior of the body. It is located inferior to the uterus and posterior to the urinary bladder. The vagina functions as the receptacle for the penis during sexual intercourse and carries sperm to the uterus and fallopian tubes. It also serves as the birth canal by stretching to allow delivery of the foetus during childbirth. During menstruation, the menstrual flow exits the body via the vagina. Vulva The vulva is the collective name for the external female genitalia located in the pubic region of the body. The vulva surrounds the external ends of the urethral opening and the vagina and includes the mons pubis, labia majora, labia minora, and clitoris. The mons pubis, or pubic mound, is a raised layer of adipose tissue between the skin and the pubic bone that provides cushioning to the vulva. The inferior portion of the mons pubis splits into left and right halves called the labia majora. The mons pubis and labia majora are covered with pubic hairs. Inside of the labia majora are smaller, hairless folds of skin called the labia minora that surround the vaginal and urethral openings. On the superior end of the labia minora is a small mass of erectile tissue known as the clitoris that contains many nerve endings for sensing sexual pleasure. Breasts and Mammary Glands The breasts are specialized organs of the female body that contain mammary glands, milk ducts, and adipose tissue. The two breasts are located on the left and right sides of the thoracic region of the body. In the center of each breast is a highly pigmented nipple that releases milk when stimulated. The areola, a thickened, highly pigmented band of skin that surrounds the nipple, protects the underlying tissues during breastfeeding. The mammary glands are a special type of sudoriferous glands that have been modified to produce milk to feed infants. Within each breast, 15 to 20 clusters of mammary glands become active during pregnancy and remain active until milk is no longer needed. The milk passes through milk ducts on its way to the nipple, where it exits the body. THE FEMALE REPRODUCTIVE CYCLE
4 The female reproductive cycle is the process of producing an ovum and readying the uterus to receive a fertilized ovum to begin pregnancy. If an ovum is produced but not fertilized and implanted in the uterine wall, the reproductive cycle resets itself through menstruation. The entire reproductive cycle takes about 28 days on average, but may be as short as 24 days or as long as 36 days for some women. Oogenesis and Ovulation Under the influence of follicle stimulating hormone (FSH), and luteinizing hormone (LH), the ovaries produce a mature ovum in a process known as ovulation. By about 14 days into the reproductive cycle, an oocyte reaches maturity and is released as an ovum. Although the ovaries begin to mature many oocytes each month, usually only one ovum per cycle is released. Fertilization Once the mature ovum is released from the ovary, the fimbriae catch the egg and direct it down the fallopian tube to the uterus. It takes about a week for the ovum to travel to the uterus. If sperm are able to reach and penetrate the ovum, the ovum becomes a fertilized zygote containing a full complement of DNA. After a two-week period of rapid cell division known as the germinal period of development, the zygote forms an embryo. The embryo will then implant itself into the uterine wall and develop there during pregnancy. Menstruation While the ovum matures and travels through the fallopian tube, the endometrium grows and develops in preparation for the embryo. If the ovum is not fertilized in time or if it fails to implant into the endometrium, the arteries of the uterus constrict to cut off blood flow to the endometrium. The lack of blood flow causes cell death in the endometrium and the eventual shedding of tissue in a process known as menstruation. In a normal menstrual cycle, this shedding begins around day 28 and continues into the first few days of the new reproductive cycle. Pregnancy If the ovum is fertilized by a sperm cell, the fertilized embryo will implant itself into the endometrium and begin to form an amniotic cavity, umbilical cord, and placenta. For the first 8 weeks, the embryo will develop almost all of the tissues and organs present in the adult before entering the foetal period of development during weeks 9 through 38. During the foetal period, the foetus grows larger and more complex until it is ready to be born. Lactation Lactation is the production and release of milk to feed an infant. The production of milk begins prior to birth under the control of the hormone prolactin. Prolactin is produced in response to the suckling of an infant on the nipple, so milk is produced as long as active breastfeeding occurs. As soon as an infant is weaned, prolactin and milk production end soon after. The release of milk by the nipples is known as the milk-letdown reflex and is controlled by the
5 hormone oxytocin. Oxytocin is also produced in response to infant suckling so that milk is only released when an infant is actively feeding. MALE REPRODUCTIVE SYSTEM The male reproductive system is a series of organs located outside of the body and around the pelvis region of a male that contribute towards the reproduction process. The primary direct function of the male reproductive system is to provide the male sperm for fertilization of the ovum. The major reproductive organs of the male can be grouped into three categories. The first category is sperm production and storage. Production takes place in the testes which are housed in the temperature regulating scrotum, immature sperm then travel to the epididymis for development and storage. The second category are the ejaculatory fluid producing glands which include the seminal vesicles, prostate, and the vas deferens. The final category are those used for copulation and deposition of the spermatozoa (sperm) within the male and these include the penis, urethra, vas deferens, and Cowper's gland. Major secondary sexual characteristics includes larger, more muscular stature, deepened voice, facial and body hair, broad shoulders, and development of an adam's apple. An important sexual hormone of males is androgen, and particularly testosterone. The testes release a hormone that controls the development of sperm. This hormone is also responsible for the development of physical characteristics in men such as facial hair and a deep voice.
6 EXTERNAL GENITALIA The penis The penis, the male organ of copulation, is partly inside and partly outside the body. The inner part, attached to the bony margins of the pubic arch (that part of the pelvis directly in front and at the base of the trunk), is called the root of the penis. The second, or outer, portion is free, pendulous, and enveloped all over in skin; it is termed the body of the penis. The organ is composed chiefly of cavernous or erectile tissue that becomes engorged with blood to produce considerable enlargement and erection. The penis is traversed by a tube, the urethra, which serves as a passage both for urine and for semen. The body of the penis, sometimes referred to as the shaft, is cylindrical in shape when flaccid but when erect is somewhat triangular in cross section, with the angles rounded. This condition arises because the right corpus cavernosum and the left corpus cavernosum, the masses of erectile tissue, lie close together in the dorsal part of the penis, while a single body, the corpus spongiosum, which contains the urethra, lies in a midline groove on the undersurface of the corpora cavernosa. The dorsal surface of the penis is that which faces upward and backward during erection. The penis has a rich blood supply from the internal pudendal artery, a branch of the internal iliac artery, which supplies blood to the pelvic structures and organs, the buttocks, and the inside of the thighs. Erection is brought about by distension of the cavernous spaces with blood, which is prevented from draining away by compression of the veins in the area. The scrotum The scrotum is a pouch of skin lying below the pubic symphysis and just in front of the upper parts of the thighs. It contains the testes and lowest parts of the spermatic cord. A scrotal septum or partition divides the pouch into two compartments and arises from a ridge, or raphe, visible on the outside of the scrotum. The raphe turns forward onto the undersurface of the penis and is continued back onto the perineum (the area between the legs and as far back as the anus). This arrangement indicates the bilateral origin of the scrotum from two genital swellings that lie one on each side of the base of the phallus, the precursor of the penis or clitoris in the embryo. The swellings are also referred to as the labioscrotal swellings, because in females they remain separate to form the labia majora and in males they unite to form the scrotum. The scrotum is supplied with blood by the external pudendal branches of the femoral artery, which is the chief artery of the thigh, and by the scrotal branches of the internal pudendal artery. The veins follow the arteries. The lymphatic drainage is to the lymph nodes in the groin. The testes The two testes, or testicles, which usually complete their descent into the scrotum from their point of origin on the back wall of the abdomen in the seventh month after conception, are
7 suspended in the scrotum by the spermatic cords. Each testis is 4 to 5 cm (about 1.5 to 2 inches) long and is enclosed in a fibrous sac, the tunica albuginea. This sac is lined internally by the tunica vasculosa, containing a network of blood vessels, and is covered by the tunica vaginalis, which is a continuation of the membrane that lines the abdomen and pelvis. The tunica albuginea has extensions into each testis that act as partial partitions to divide the testis into approximately 250 compartments, or lobules. Human male structures involved in the production and transport of semen Each testis is supplied with blood by the testicular arteries, which arise from the front of the aorta just below the origin of the renal (kidney) arteries. Each artery crosses the rear abdominal wall, enters the spermatic cord, passes through the inguinal canal, and enters the upper end of each testis at the back. The veins leaving the testis and epididymis form a network, which ascends into the spermatic cord. The lymph vessels, which also pass through the spermatic cord, drain to the lateral and preaortic lymph nodes. Nerve fibres to the testis accompany the vessels; they pass through the renal and aortic nerve plexuses, or networks. ACCESSORY ORGANS The prostate gland, seminal vesicles, and bulbourethral glands These structures provide secretions to form the bulk of the seminal fluid of an ejaculate. The prostate gland is in the lesser or true pelvis, centred behind the lower part of the pubic arch. It lies in front of the rectum. The prostate is shaped roughly like an inverted pyramid, its base is directed upward and is immediately continuous with the neck of the urinary bladder. The urethra traverses its substance. The two ejaculatory ducts enter the prostate near the upper border of its posterior surface. The prostate is of a firm consistency, surrounded by a capsule of fibrous tissue and smooth muscle. It measures about 4 cm across, 3 cm in height, and 2 cm front to back and consists of glandular tissue contained in a muscular framework. It is imperfectly divided into three lobes. Two lobes at the side form the main mass and are continuous behind the urethra. In front of
8 the urethra they are connected by an isthmus of fibromuscular tissue devoid of glands. The third, or median, lobe is smaller and variable in size and may lack glandular tissue. There are three clinically significant concentric zones of prostatic glandular tissue about the urethra. A group of short glands that are closest to the urethra and discharge mucus into its channel are subject to simple enlargement. Outside these is a ring of submucosal glands (glands from which the mucosal glands develop), and farther out is a large outer zone of long branched glands, composing the bulk of the glandular tissue. Prostate cancer is almost exclusively confined to the outer zone. The glands of the outer zone are lined by tall columnar cells that secrete prostatic fluid under the influence of androgens from the testis. The fluid is thin, milky, and slightly acidic. Despite the differences between the adult female and male are derived from the intermediate mesoderm. The three main fetal precursors of the reproductive organs are the Wolffian duct, Müllerian ducts, and the gonad. Endocrine hormones are a well-known and critical controlling factor in the normal differentiation of the reproductive system. The Wolffian duct forms the epididymis, vas deferens, ductus deferens, ejaculatory duct, and seminal vesicle in the male reproductive system and essentially disappears in the female reproductive system. For the Müllerian Duct this process is reversed as it essentially disappears in the male reproductive system and forms the fallopian tubes, uterus, and vagina in the female system. In both sexes the gonad goes on to form the testes and ovaries, because
9 they are derived from the same undeveloped structure they are considered homologous organs. There are a number of other homologous structures shared between male and female reproductive systems. However, despite the similarity in function of the female fallopian tubes and the male epididymis and vas deferens, they are not homologous but rather analogous structures as they arise from different fetal structures. Gametogenesis occurs when certain types of germ cells undergo meiosis to split the normal diploid number of chromosome(n=46) into haploids cells containing only 23 chromosomes. In males, this process is known as spermatogenesis and takes place only after puberty in the seminiferous tubules of the testes. The immature spermatozoon or sperm are then sent to the epididymis where they gain a tail and motility. Each of the original diploid germs cells or primary spermatocytes forms four functional gametes which is each forever young. The production and survival of sperms require a temperature that is lower than the normal core body temperature. Since the scrotum, where the testes are present, is situated outside the body cavity, it provides a temperature about 3 C below normal body temperature. In females, gametogenesis is known as oogenesis which occurs in the ovarian follicles of the ovaries. This process does not produce mature ovum until puberty. In contrast with males, each of the original diploid germ cells or primary oocytes will form only one mature ovum, and three polar bodies which are not capable of fertilization. It has long been understood that in females, unlike males, all of the primary oocytes ever found in a female will be created prior to birth, and that the final stages of ova production will then not resume until puberty. However, recent scientific data has challenged that hypothesis. This new data indicates that
10 in at least some species of mammal oocytes continue to be replenished in females well after birth.
