Abstract Background: Malnutrition is the one of the greatest problems among Sudanese children. This condition resulting from faulty nutrition, it weakens the immune system and causes significant growth delay. Growth assessment is the measurement that best defines the health and nutritional status of children. Methods: A cross-sectional community study, in which we explored nutritional status of 300 preschool Sudanese children with the help of Corresponding author Sitana E Abdelrahman sitanaalhaj@gmail.com structured interviewed questionnaire and anthropometric measurements, from July 2014 to February 2015 in Wad Medani Great Locality, Gezira State, Sudan. Results: The results showed that, the prevalence of stunting, underweight, wasting, overweight and obesity were 18.0%, 10.6%, 4.0%, 13.6% and 3.3% respectively. The children were classified according to the body mass index as 44% as severe thinner, 24.6% moderate thinner and 21% mild thinner. Meat consumption has positive effect on body mass index. Conclusion: This study indicated high prevalence of stunting among preschool Sudanese children. The distribution of weight 182

2 for age for three years children shifted toward lower values compared to CDC standards. Keywords: Growth monitoring, malnutrition, preschool children, stunting, wasting Introduction Nutrition is defined as qualitative and quantitative requirements of diet necessary to maintain health. (1) Adequate nutrition is essential during childhood to ensure healthy growth, proper organ formation and function, a strong immune system, and neurological and cognitive development. (2) Although food is essential, it has often been lacking in the qualitative and quantitative point of view resulting in the occurrence of malnutrition. (3) Malnutrition affects the rate of morbidity and mortality among the young children and also poses a threat to their physical and mental development. The very young are less able to cope physiologically with nutritional deficiencies than older children and adults. (4) The two main anthropometric indicators used to define malnutrition, stunting, and wasting.wasting is a deficit in tissue and fat masscompared with the amount expected in a child of the same height or length, and may result either from failure to gain weight or from actual weight loss. (5) Stunting signifies slowing in skeletal growth and is frequently found to be associated with poor overall economic conditions, especially mild to moderate, chronic or repeated infections, as well as inadequate nutrients intake. (6) Nutritional status in early life is important for laying down the foundation of healthy adulthood through its effect on growth and development and subsequent on quality of life, productivity and economic development in later adult life. (7) Methodology This was a descriptive and analytical crosssectional community study of nutritional Table 1: The prevalence of under weight, overweight, obesity, stunting and wasting. 183 status of preschool Sudanese children aged 3-5 years, during July 2014 to February 2015 in Wad Medani Greater locality. The sample size of 300 children was calculated according to the method described by Boelaert et al. (8) Weight, height and mid upper arm circumference of the selected children were measured.weight measurement was taken by using digital scale. Weight was recorded to the nearest 10 grams. (9) The height was measured by using a Holtain stadiometer. The measurement was recorded to the nearest 0.1cm. (10) Anthropometric incidences of height/age, weight/age and weight/height were used to estimate children s nutritional status according to Center for Disease Control and Prevention (CDC) cutoff point. The sociodemographic characteristics and food habits of the subjects were collected usingpretested questionnaires administered to the children mothers. Results Nutritional status assessment by using anthropometric percentile: From Table (1) children showing underweight were 10.6% (32 children) of the study population which is equivalent to 5 th percentile weight for age, while 13.6% (41 children) of the study population were overweight which is equivalent to 90 th percentile weight for age, and 3.3% (10 children) were obesity which is equivalent to 95 th percentile weight for age. Also, our findings show that 18.0% (54 children) of the study population with stunting which is equivalent to 5 th percentile height for age and 4.0% (12 children) of the study population were wasting which is equivalent to 5 th percentile weight for height. All measured values were compared to CDC standard. Percentile curves of weight for age, height for age and weight for height were submitted. 3 Figures 1 & 2 compare the distribution of weight for age for males and females, respectively, with CDC standard. There is a deviation of data below the median CDC reference standard especially for three years old children. Figure 1: Weight for age percentile curve for males. 184

7 Figures 5 & 6 show the data of weight for height charts for males and females, respectively, the distribution of children seemed to be similar to that of CDC standard. Figure 5: Weight for height percentile curve for males. 188

9 The maximum body mass index (BMI) value 31kg/m 2 while the minimum value was 11 kg/cm % of the children in the range of kg/cm2. (Table 2) Table 2: Body mass index categories of the data. BMI Number Percent (%) The BMI of 132 children ( 44%) were severe thinner with BMI 16kg/m2, 24.6 % were moderate thinness, with BMI and 21% were mild thinness with BMI according to cut-of-point recommended by WHO multicentre Growth Reference Study Group. (11) Table 3 shows the mean and Standard deviation of BMI and food groups. Regarding vegetable consumption, children who consumed less than four units of vegetables per day (minimum requirement), had the lower mean BMI while the children who consumed more than four unit had greater BMI but the probability of correlation were not significant (p = 0.490). Concerning fruits intake, children who consumed more than two units of fruit (minimum requirement) had higher BMI than those who took less than two units but the probability of correlation were not significant (p = ). The mean BMI is greater in the group who consumed more than three units of meats (minimum requirement) and lowest with children who consumed less than three units. The difference was significant (p = 0.050). With regard to consumption of milk, children who consumed more than 2 cups of milk a day (minimum requirement) had better BMI than the others. The probability of correlation were not significant (p = 0.692). Regarding bread and cereal consumption, the mean BMI of children who consume more than four units (minimum requirement) is greater than the other groups, the probability of correlation was not significant (p = 0.167). 190 Table 3: Mean and standard deviation of (BMI) and food groups. (12) However, underweight is most common in the South-central Asia (30%), followed by Western, Eastern, and middle Africa (22%), (19%) and (17%) respectively and South-Eastern Asia (17%). (13) Regarding overweight our finding indicated that: 13.6% of the children were overweight and 3.3% were obese which is lower to the percentage of overweight and obesity among preschool children in Egypt which is 20% (13) and higher than that found in Central Africa and Southern Africa which are 8% and 9%, respectively. (13) Our study indicated that, the prevalence of stunting was 18% which is lower than that found among children of the same age in Sub Saharan Africa which was reported as 39% by Souganidis et al. (14) and in northwest Ethiopia

10 which was 24%. (15). On the other hand it was higher when compared to that found in Egypt, Alexandria which was 15%. (16) Concerning the prevalence of wasting, we found that, 4% of the children were suffering from wasting. This finding was lower than that found in Sub-Saharan Africa which was 6% reported by Souganidis et al, (14) and higher than that found in Egypt, Alexandria, which was 3.6% reported by El Said et al. (16) In comparison of our results with previous work done by Abd Elmoieam m (17) among preschool children in Elobied town, Kordofan State, Sudan, she reported that, 5% of the children were wasting, which is slightly greater than our study while, the prevalence of the underweight was 10% which is as same as our finding and 9% of the children with stunting which is lower than our finding. Growth chart in the form of weight for age, height or stature for age and weight for height are important tools for the measurement of growth of children. (18) The comparison of height for age, showed that children in the data were shorter than those in CDC chart. Children might fail to meet their potential for growth because of poor intakes of energy protein and other nutrients these result was supported by Abdalla et al. (19) who reported that most supplementary food offered to the children in Sudan are cereal-based stuffs which are naturally deficient in protein constituents. In this study, 44% of the children were severe thinner, 24.6% were moderate thinner and 21% were mild thinner and according to their BMI. These values lower than that mentioned by Idris (20) who conducted study in Fato village Kassala state, he reported that 85.5% of the children were severe thinner, 10.8% were moderate thinner, and 1.7% were mild thinner. This study showed the correlation of body mass index and food group. It was observed that, vegetables intake had no effect on BMI. These result confirm previous finding of (21) Mathews et al. who mentioned that frequency consumption of vegetables grains and nuts were inversely related to the risk of being overweight. Most of the children in the study take less than the minimum requirement of fruits. Fruit consumption had no effect on BMI. This finding is similar to Skinner et al. (22) who mentioned that, there is no statistically significant differences in children s BMI related to fruit intake. The meat intake had positive effect on growth. A similar finding was reported by Vergnaud et al. (23) who stated that positive association between all type of meat (red meat, poultry and processed meat) and weight gain. Our study showed that, there was no positive correlation between milk intake and BMI. Several studies of milk consumption on preschoolers and school-aged children reported that there is no relationship between the amount of milk intake and BMI z-score. (24) On the other hand, analyzed data for preschool children as early childhood longitudinal survey, for assessing the association of daily serving of milk intake with BMI indicated that children who consumes more than 3 services were more likely to be overweight and obese than those drinking serving daily. (4) The bread cereals consumption had no significant effect on BMI. These results are in agreement with Cho et al. (25) who indicated that children who eat cereal (cooked or ready-to-eat cereal) or quick breads for breakfast have significantly lower BMI. Also Peter et al. (27) mentioned that regular breakfast cereal consumption is associated with lower BMI and less risk of being overweight or obese. In conclusion, this study indicated high prevalence of stunting among preschool Sudanese children. The distribution of weight for age for three years old children shifted toward lower values compared to CDC standards. Meat consumption had positive correlation with BMI. Conflict of Interest: None.
