The Background
More & more we are seeing this:
The Trend (hopefully!!) Re-emphasising good nutrition Meal preparation When necessary individualize Emphasise individual responsibility
The Advice
The Supplement Industry
Profit-driven!!
Is there a role for supplements in sport? Sat 25 June 2011
Today: Dr Amy Eichner (USA Anti-doping Agency) Worldwide phenomenon! Athletes carry the consequences USA legislation changes. Consumer advocacy (media)
Today Andrew Breetzke, sports lawyer Allowing for exceptions destroys the concept. The future: legal action; $5.4mil!!
Today Shelly Meltzer, dietitian in sport Lack of objective & scientific testing Only withdrawn from market once a critical mass of adverse findings!! Individualisation most NB interventions 1 st (sound nutrition) Position stand Adult & Youth (SASMA 2011)
Today Pieter VD Merwe, A Doping Control Supps not regulated by govt Labelling issues on label, Banned substances in off-the-shelf supps Small doses lead to positive doping tests
Today Lize Havemann-Nel, dietitian in sport Does it work? Yes 1-2%; responders/non-responders Immediate performance & training gains Safe likely but risk of contamination Supps to be seen in context!
Today Andrew Bosch, exercise physiologist CHO intake improves performance (100s of research studies) Protein aids recovery from exercise (Tarnopolsky) 7% CHO + 2% Prot may improve performance? (untrained vs trained aths) Protein hydolysate ( g/kg) may assist with post-ex mus soreness / recovery
Today The natural- Nico Pfitzenmaier, triathlete -recover Plant-based, raw, whole food Chocolate addiction bad?!! Supps should not be a shortcut.
Today The manufacturers and retailers Complex process to regulate In-house controls by retailer (Dischem) U18 use
Today - well-enough informed? 3 rd Party testing as a trend Efficacy?
Today Glen Hagemann Reasons for taking Prevalence in the adolescent population Health side effects of some supplements Benefits vs risk
Today Alan Lion- Issues at school level: Doping Supplements (pupils, coaches, parents..) Appropriate? What? When?
Is there a role for supplements in Sport? Yes!! But the industry has made it so much more complicated!
Is there a role for Supplements in Sport? The issues: Efficacy? (Peer review!!) Side effects / Interactions? Contamination
Is there a role for Supplements in Sport? There is no such anything as zero risk. Professional sportspersons Recovery, CHO, protein, creatine School Rarely (recovery); emphasise sound nutritional & training principles No sponsorships / road shows Not sold at schools Regulation, quality control (internal & 3 rd party), individualization, professional advice. In the absence of govt regulation, truly independent 3 rd party testing probably provides the next most appropriate form of regulation (but not foolproof!); 1 step only!
Is there a role for Supplements in Sport? Clinicians, scientists, athletes, industry Professionals obligation & responsibility Young athletes education, counter dogma Manufacturers & Retailers Thank you for your Days of unscrupulous marketing and sale of your supplements are kind coming attention. to an end. Sports medicine professionals are uniting in their approach based on what science teaches us and our experience (many unpleasant) shows us Stop contaminating our children with untruths, devious marketing & conflicts of interest and let them rediscover the educational value of untainted sports participation!
"The most important thing in the Olympic Games is not to win but to take part, just as the most important thing in life is not the triumph but the struggle. The essential thing is not to have conquered but to have fought Let the games begin!
So what? Claims NOT scientifically proven (efficacy?) Contents change from batch to batch Cannot be sure they have been fully or accurately labeled Some contain ingredients not listed in references Lack of quality assurance
Long term safety unknown Drug-supplementation interactions unknown Labels do not always identify banned substances Dietary supplements can contain banned substances
Not regulated in same way as pharmaceuticals As a result most information is derived from the industry itself - media, internet, Should your coach still be advising on the use of supplements? nutritionally trained professional
Is there ANY place for supplements? Malnourished Specific nutritional deficiency Insufficient calorie intake. Immediate replenishment /Recovery? Individualised Dietician/Sports Physician
3. The Doping Underworld
Supplements Limited evidence of benefit Potential side effects Potential contamination Counters culture of sound nutrition & training Drugs Cheating Physically & Psychologically damaging Criminal act!
be doing?

Supplement Policy Understand & promote principles of nutrition - School website, letters, notices Understand relationship with appropriate training No supplements to be sold at school No supplements to be bought from pharmacies or stores No sponsorships from supplement companies Nutritionist-guided meal & replenishment policy
Ethics! Unified approach as clinicians. The basics diet & training! No supps U18 Diet advice Drug-free Publicise & educate Report colleagues.
The use of illegal performanceenhancing is regrettably a reality in school sport. sporting & social Conclusion
Conclusion As medical and allied professionals we are obliged to: Educate that Drug use for sport or body enhancement is Illegal Physically & psychologically dangerous Cheating Supplements may not be effective and contain banned contaminants Help institute processes to regulate this
Conclusion In our increasingly competitive world we have, encouraged by nutritional charlatans, forsaken the important yet simple principles of sports nutrition Supplementation should always be tertiary to appropriate Thank training you and eating. for your An acceptable way of consistently kind attention. being a world-beater is to cheat. There are enough professionals doing a fine job of that! Through an extension of the education and guidance that your excellent school provides, intervention is required in order to reclaim the true glory, spirit & tradition of youth sport as we knew it.
