Summry: A rndomised controlled clinicl tril ws used to ssess response to therpy. 82 dogs with clinicl signs of osteorthritis were enrolled in the study of which 16/82 (19.5%) were then excluded due to lck of rdiogrphic evidence of osteorthritis. The remining 66 dogs completed the study. The min reeds represented were cross reeds 2/82 (24%), Lrdors, Retrievers nd Border Collies s the three min pure red cses 22/82 (27%) nd the remining dogs were vriety of pure reeds. 7 REVIEW OF IN-VIVO STUDIES (Supporting Equine Pper) LOW QUALITY OF EVIDENCE FOR GLUCOSAMINE-BASED NUTRACEUTICALS IN EQUINE JOINT DISEASE: REVIEW OF IN VIVO STUDIES. W. Person, M Lindinger; Equine Veterinry Journl (29) 41(1)- Summry: Nutrceuticls re incresingly pplied to the mngement of equine rthritis nd joint disese, prticulrly those sed upon glucosmine in horses ppered more thn 25 yers go, it ws not until 1992 tht isolte studies egn to e reported. Since tht time,15 in-vivo ppers hve een pulished in the equine literture, usully on products lredy commercilly ville nd often seeking evidence for efficcy. These studies demonstrte n encourging trend to mnufcturers of these products investing in reserch, ut most do not meet qulity stndrd tht provides sufficient confidence in the results reported. This review discusses the entirety of pulished in-vivo reserch on glucosmine-sed nutrceuticls (GBN) for horses, including Cosequin, Cortflex, Synequin, Ssh s EQ (4CYTE Equine), Myristol, chondroitin sulphte, glucosmine sulphte nd glucosmine hydrochloride; nd considers experimentl limittions of this reserch long with their impct on interprettion of results. A qulity score ws clculted for ech pper ccording to preset qulity criteri. A minimum qulity stndrd of 6% ws set s the threshold for confidence in interprettion of results. Of the 15 ppers reviewed, only 3 met the minimum qulity stndrd. Key Findings: It is concluded tht the qulity of studies in this re is generlly low, prohiiting meningful interprettion of the reported results. 4CYTE Equine (Ssh s Equine) hs otined the highest score (68% - see tle 3 Supp1) ccording to these predetermined criteri. In prticulr, the strength of the 4CYTE Equine reserch is tht: Control horses fed plceo diet were included in oth studies (sfety study nd induced inflmmtion study). A sfety study ws done for 84 dys t 7 times the mintennce dose. Synovil (joint) fluid ws ssessed fter 14 dys of 4CYTE Equine supplementtion. Intr-rticulr inflmmtion ws induced nd the effect of 4CYTE Equine fed horses ws mesured compred to plceo fed horses. The importnt thing to relise here is tht the reserch done on 4CYTE Equine focused on ojective outcome mesures (lood tests, urine tests, evlution of GAGs nd enzymes in synovil fluid, joint circumference etc.), rther thn the sujective ssessment (lmeness, flexion tests etc.) of n evlutor. Summry: In previous in-vitro study involving the DN, simulted digest of the product significntly inhiited the inflmmtory response of equine crtilge explnts to exogenous IL-1. To correlte those in-vitro dt with in-vivo dt, IL-1 ws injected into inter-crpl joints of horses to simulte n inflmmtory chllenge in the present study (of 22 helthy horses). 12 horses were fed, 15, 45 or 75mg of DN (3 horses/tretment) dily for 84 dys. Generl helth nd clinic pthologic vriles were monitored t intervls. Ten other horses received or 15mg of DN/d (5 horses/tretment) for 29 dys. One inter-crpl joint in ech horse ws injected twice with IL-1, (1 nd 1ng on dys nd 1, respectively) nd the contrlterl joint ws similrly injected with sline solution. Synovil fluid prostglndin E2, (PGE 2 ) sulfted glycosminoglycn (GAG), nitric oxide (NO), protein concentrtes nd leukocyte counts were nlysed efore nd t intervls fter injections. Synovil fluid PGE 2 pg/ml % Fig 4- Feeding of DN inhiited the increse of PGE 2 (54.6% decrese) nd GAG induced y the intr-rticulr injection of IL-1 highlighting DN nti-inflmmtory nd chondroprotective properties s lso demonstrted in in-vitro studies Control 4CYTE Equine Key Findings: Administrtion of the DN (up to 75g/d) to horses for 84 dys did not induce ny dverse effects. Results suggested tht dministrtion of the DN my e useful in preventing inflmmtion ssocited with rthritis nd degenertive joint disese in horses. Intr-rticulr injection of the IL-1 cused incresed concentrtion of synovil fluid PGE 2 nd GAGs consistent with inflmmtion nd crtilge degrdtion ssocited with rthritis. Feeding of DN inhiited the increse of PGE 2 (54.6% decrese) nd GAG induced y the intr-rticulr injection of IL-1 highlighting DN nti-inflmmtory nd chondroprotective properties s lso demonstrted in in-vitro studies. Feeding DN led to decrese in the circumference of joints swollen y the intr-rticulr dministrtion of IL-1 consistent with inhiition of incresed joint effusion seen in cses of rthritis. Pge 5

Summry: The University of Guelph conducted series of experiments evluting the effects of Epiitlis on crtilge inflmmtion nd degrdtion induced y interleukin-1 (IL-1). Reserch models pplied in this study were designed to promote in-vitro conditions consistent with the desired context of use of Epiitlis : i.e. tht the product ws to e dministered orlly; the trget tissue ws rticulr crtilge. 2 Prolifertion: equine chondrocytes increse in cell numer (%) Epiitlis Indomethicin Fig 3 - Equine Chondrocyte Prolifertion When chondrocytes were incuted with Epiitlis they produced concentrtion-dependnt increse in prolifertion. Indomethcin however, showed trend towrds inhiiting chondrocyte prolifertion Frction concentrtion (mg/ml) Key Findings: Results of the present study supported the hypotheses tht Epiitlis nd DN modulte response of rticulr crtilge explnts to exogenous IL-1 without dversely ffecting viility or metolism of chondrocytes. The use of indomethcin s reference nti-inflmmtory drug provided n llopthic enchmrk ginst which the effects of these 2 nutrceuticls could e compred. Simulted digests of NZGLM, SC, nd lone (the 3 constituents of the DN other thn Epiitlis ) re known to hve nti-inflmmtory or crtilge-spring effects in IL-1 stimulted crtilge explnts. The dt otined in the present study indicted tht the oil from the seed of BO lso prtilly inhiits IL-1 dependent PGE 2 production y crtilge explnts. The heterogeneous composition of the DN (i.e., NZGLM, SC, lone, nd BO) resulted in significntly improved NO - nd PGE 2 - inhiiting properties, compred with the effects of the individul ingredients lone t the sme dose. This synergistic effect my result from ech ingredient influencing divergent PGE 2 - or NO-synthetic pthwys. In the present study, cell viility susequent to IL-1 chllenge ws significntly incresed y Epiitlis sim nd DN sim. These increses were not result of reduced poptosis, s might hve een predicted from the NO inhiitory properties of the DN, ecuse the increses in viility resulted from n increse in stining of live cells ut not decrese in stining of ded cells. The DN sim, perhps through provision of Epiitlis (which hd lmost equl effect on cell viility), my sim produce n gonist effect on the prostnoid receptor involved in chondrocyte prolifertion, resulting in n increse in live-cell stining. Conditioning with NZGLM (.18mg/mL) resulted in medi [GAG] tht ws significntly lower thn stumlted controls. Conditioning with AB (.6 nd.18mg/ml) significntly reduced medi [GAG] fter 48h of IL-1 exposure. Key Findings: SC nd NZGLM significntly inhiited IL-1 induced PGE 2 synthesis nd IL-1 induced GAG relese, nd AB ws n effective inhiitor of IL-1 induced NO production. The three test nutrceuticls ffect t lest three mjor pthwys involved in the ctolic cycle of rthritis nd my prove importnt tretments nd/or preventtives for the pin nd degrdtion ssocited with this condition.
