2 Learning objectives To call the attention of physicians for the high prevalence and misdiagnosis of the battered-child syndrome and to help the recognition of radiographic findings related to the syndrome. In this poster we will review the main radiographic signs, emphasizing its importance in the management of this cases. Background The battered-child syndrome includes injuries in children from physical to psychological trauma, neglect and even sexual abuse. The first article on child maltreatment and abuse was published in 1860 by Ambrose Tardieu, in which he mentioned three aspects of this syndrome: the sociological, the demographic and the medical, without mention the radiographic signs. He recognized the child's caregiver as responsible for the attacks and described the typical injuries and emotional reaction of the victims. However, it had not been recognized as a clinical entity until 1930, when the clinical and radiographic factors were merged to establish a new era of violence against children. (Brogdon, 1998). Disciples of Caffey in Pittsburgh and Cincinnati have studied the problem deeply, but the attention of the radiology community in this entity can be credited to Silvermam, after his publication in (Brogdon, 1998). Still, a variety of names was applied to the syndrome. Caffey suggested traumatic stress syndrome father-child. Many other names began to emerge, as the Caffey's syndrome and Silverman Tardieu Ambriose's syndrome. Finally in 1962, Kempe named the syndrome as "battered child syndrome" in order to take attention to this serious clinical and social problem. (Brogdon, 1998). Page 2 of 26

In Brazil, it is estimated that battered-child syndrome affects 10:1.000 children and 2-3% evolve to death, most commonly in children under 4 years (mainly newborns and infants). Most offenders (80%) are the biological parents and the most common factors associated with this kind of violence are poverty, stress, alcohol and drugs. The right diagnosis is essential and depends on the anamnesis, physical examination and radiologic signs. Page 3 of 26
4 The main differential diagnosis are: Osteogenesis imperfecta; Congenital insensitivity to pain; Scurvy Congenital syphilis; Caffey disease; Menkes disease; Multiple fractures in severe stunting; Hypophosphatemia; Leukemia Neuroblastoma metastatic Sequelae of osteomyelitis Septic arthritis Imaging findings OR Procedure details Review of current literature on battered-child consulting the main databases (Pubmed, Bireme and Scielo) portals and electronic reference journals in Radiology and Pediatrics. Radiology may be the first to suggest a diagnosis of battered child syndrome, and therefore, the radiologist must be alert and have a high degree of suspicion to make the diagnosis. Different imaging modalities can be used, such as skeletal screening with conventional radiography, computed tomography, bone scan, ultrasound and magnetic resonance imaging. The findings of suspicion include: Incompatible anamnesis with the lesions Parental age between the second and third decades of life Metaphyseal injuries Multiple fractures and in unusual locations Skeletal lesions in different stages of healing Subdural hematomas and cerebral contusions Retinal hemorrhage Fracture of scapula Transphyseal fractures Page 4 of 26

26 Conclusion Considering the magnitude and impact that provides to victims, it is important to have this syndrome increasingly recognized as a public health problem, taking care of various levels of complexity, information systems and interdisciplinary approach to a more effective fight against this social misfit.
