3 1 Femoral neck resection 3 Broaching The level of the cut of the femoral neck is determined during the operative pre operative planning using x ray templates. The anatomic landmarks are confirmed intraoperatively. The cut is at 45 to the anatomical axis of the femur. Connect the broach to the appropriate handle and progressively impact the cancellous bone up to the neck cut. A positioning rod can be inserted into the broach handle to help visualise anteversion. 45 The definitive size is reached when the broach is stable in torsion and well seated. IMPORTANT : The reamer inserting mark is the impactor - broach junction. 2 Preparing the greater trochanter and the metaphysis The opening of the femoral canal and preparation of the greater trochanter can be initiated with a small rasp or box chisel. It can be necessary to rasp the inside of the greater trochanter in order to avoid placing the broaches and implant in varus. 4 Reaming for insertion of a collared stem Install the reaming axis on the reamer. Present the powered reamer and ream over the broach in place until impingement of the reamer on the broach. This operation results in a perfect line between the femoral surface and the upper surface of the broach. The calcar reamer softens the surface of the calcar for insertion of stems with collar. 4 5 Trial reductions Standard, offset and coxa vara necks allow for trial reduction using the broaches, and to determine the centre of rotation. It is necessary to press the cone up to the stop to block the trial at the correct length. The collar neck increases with each stem size. 8 Femoral heads Trials on the implant It is possible to carry out trial reductions with trial heads. Definitive head assembly The head and neck assembly allows for a dynamic trial reduction. Clean and dry the morse taper. Place the definitive head on the morse taper, and impact it with a sharp hammer blow on the impactor, which is also used to reduce the articulation. 6 Cleaning of trial necks The trial necks can be dismantled for cleaning. Stem extraction Assembly 7 Definitive stem implantation Assemble the gripping mechanism over the morse taper (the flat proximal part of the taper must be fully in contact with the bottom of the gripper), tighten the nut with the positioning rod and connect the broach impactor. Cementless version Disassembling The stem is placed into the femoral canal. Final impaction is achieved when the stem coating is flush with the level of the cut. While disassembling, unscrew the nut with the positioning rod. In case of difficulties, please send us the extractor and the stem cleaned. Cemented version The stem is introduced progressively into the cement mantle by hand pressure using the constrained impactor. This impactor when seated in the stem, allows adjustment of orientation, anteversion can be visualised with the positioning rod. The stem is fully embedded when the top of the cement landmark is level with the cut. The non constrained impactor maintains the stem under pressure during cement curing. 6 7
