2 The Invention of the Anatomical Femoral Stem The cemented SPII Model Lubinus hip system has been implanted in around 500,000 cases to date. Its success story begins as far back as the seventies, when Link raised the bar for fi xation stability of hip implants by developing the biomechanical concept. The SPI one of the most important innovations in the history of hip arthroplasty first came on the market in 1978 to an initially lukewarm reception from some quarters of the medical world. Some doctors took the view that an anatomically shaped femoral stem could only be explanted with great difficulty, if at all a theory we managed to disprove, recalls Dr. Hans-Herrmann Lubinus, who headed up the Lubinus Clinicum in Kiel between 1969 and 2003 and who developed the SPI and its successor, the SPII, in collaboration with Link. This was not the only reason why the critical voices soon fell silent: the progress which the invention of the first anatomical femoral stem in implantology represented was simply too significant to ignore. His son, Dr. Philipp Lubinus, is convinced that the success story of the femoral stem is far from over. The SPII stem s excellent longterm results for example, in the Swedish Hip Arthroplasty Register provide a springboard for the implant s continued expansion, says the Head of Implantology at the Lubinus Clinicum. 4 The introduction of different CCD angles is one of the most important advances in the SPII system Perfect fit Link s development of the biomechanical concept meant that the company could offer an alternative for hip arthroplasty which was superior in many respects to the straight-stem prostheses then typically available. The SPII was the first design to take into consideration the fact that the proximal femoral canal is S-shaped, something which had been disregarded in the design of earlier femoral stems. Once the stem conformed to the anatomy of the bone, not only did we almost neutralize rotational forces but we also achieved uniform load distribution, which drastically improved fixation stability, explains Helmut D. Link. Another advantage of the anatomical stem over a straight model is that it enables the implantation of thicker stems. What is more, the stem is surrounded by a uniform cement mantle, which further reduces the risk of loosening compared with straight stems. Important advances While the SPI was a great success in the early years, the biomechanical concept did not make a real breakthrough until This is when Link replaced the SPI onepiece prosthesis with the SPII Model Lubinus, a modular system with femoral heads and acetabular cups of different sizes. It was finally possible to optimally adapt the hip prosthesis to fit the patient s unique anatomy during the operation. With that, the SPII, as the world s first modular hip system with an anatomical femoral component, was unstoppable. Almost 500,000 patients worldwide have had successful joint replacement surgery thanks to the SPII. In addition, national and international competitors applied to Link in numbers for a licence to use the patent and enable them to produce their own anatomical stems, with the result that a great many more patients benefit from the invention. The SPII has been further developed over the years and brought in line with the latest findings in implantology. Indeed the introduction of different CCD angles and increased modularity were important steps for the prosthesis to be capable of adapting to the unique biomechanical needs of each individual patient with even greater flexibility. Other advances include combining it with the Beta Cup acetabular cup system and the development of an anti-allergic coating. The basic principle of the anatomical stem, however, has remained unchanged to this day. Dr. Hans-Hermann Lubinus is one of the fathers of the SPII, which has been available as a modular system since 1984

5 In a Word, Excellent For years, the SPII Model Lubinus has had a track record of excellent long-term results in the Swedish Annual Hip Report: proof positive of the outstanding performance of the anatomical hip stem. The figures speak for themselves: according to the latest edition (2007) of the Swedish Annual Hip Report*, the SPII Model Lubinus has a survival rate of 94.7% after 16 years, based on 60,949 cases across a large number of different centres. Once again, the Link classic has demonstrated that the concept of a cemented total hip replacement with an anatomical femoral stem is one of the most important achievements ever in hip arthroplasty. The Swedish Hip Arthroplasty Register testifies in black and white to the long success story of the SPII hip stem. In addition, in Sweden the implant is among the cemented hip stems with the lowest risk of revisions, loosening and periprosthetic fractures, says Professor Johan Kärrholm from the Department of Orthopedics of Sahlgrenska University Hospital in Mölndal, who is one of the authors of the report. Study provides compelling evidence Recording a large number of implantations and covering a wide spectrum of surgeons, the Swedish Hip Arthroplasty Register shows as almost no other study does what long-term results a patient can expect after a hip implantation. For a great many surgeons, therefore, the SPII s results in the Swedish Annual Hip Report were what prompted them to use the implant in their clinic. Professor Kärrholm is a firm supporter of the SPII. Back in the eighties I worked with the SPII hip stem in the orthopedic clinic in Umeå, he says.
