Presented by the Physicians of the Orthopaedic & Spine Center of the Rockies Accreditation This activity has been planned and implemented in accordance with the accreditation requirements and policies of the Accreditation Council for Continuing Medical Education through the joint providership of the University of Tennessee College of Medicine and the OrthoForum. 3 Overview The symposium will focus on bone and joint issues that may be encountered in patients by many primary care, occupational medicine, ER physicians and PAs and NPs, as well as chiropractors, therapists, RNs, ATs, and RTRs. Participants will review information relevant to orthopaedic diseases and treatments, and be able to apply new knowledge and skills. Symposium objectives will be provided in the conference materials. The presenters from the Orthopaedic & Spine Center have fellowship training in their subspecialty and/or board certification. Certificates & Credits AMA: The University of Tennessee College of Medicine designates this live activity for a maximum of 5 AMA PRA Category 1 Credit(s) TM. Physicians should claim only the credit commensurate with the extent of their participation in the activity. PAs, NPs and Nurses: Physician Assistants, Nurse Practitioners and Nurses may use these credit hours toward certification renewal. This credit is acceptable by the American Academy of Physician s Assistants (AAPA), American Nurses Credentialing Center (ANCC) and American Academy of Nurse Practitioners (AANP). PTs, OTs, Chiropractors: We have provided a general certificate of participation from the CME provider in years past. If you need to verify that this is acceptable for your professional society s credits in 2017, please check with your society or credentialing board. ATs and RTRs: We anticipate making application to your credentialing bodies for your specialty-specific certificates. We may not have a decision until closer to the conference date.

Dallow, MD, Primary Care Sports Medicine Specialist *The breakouts are 30 minutes long, and then repeated, allowing guests to attend two. The breakouts are scheduled for 30 minutes based on ACCME requirements that credits be awarded in 15-minute increments. 2:30 p.m. Symposium Concludes Certificates CME and general certificates of attendance will be available from the University of Tennessee Graduate School of Medicine following the conference. If you need credits for the conference from UTGSM, you will fill out a credit request form at check-in day of the event, including your address. Trumper, MD Orthopaedic Surgeon, Sports Medicine Knee Surgery Rocci Trumper specializes in operative sports medicine and disorders of the knee (including knee replacement). A member of OCR since 1995, Dr. Trumper earned his medical degree at the University of South Dakota. He completed residency at State University of New York, Syracuse, and sports medicine fellowship at Methodist Sports Medicine Center, Indianapolis. He is one of OCR s team physicians for Colorado State University. Sean G. Grey, MD Orthopaedic Surgeon, Sports Medicine Shoulder Surgery Sean Grey specializes in sports medicine and disorders and treatment of the shoulder, including shoulder replacement surgery. Dr. Grey joined OCR in He completed medical school at the University of Cincinnati, residency at the University of Colorado, and sports medicine fellowship at Orthopedic Associates of Aspen/CU. Dr. Grey is a team physician for Colorado State University. Wesley P. Jackson, MD Orthopaedic Surgeon, Athletic-Reconstructive Surgery of the Foot and Ankle Wes Jackson specializes in athletic and reconstructive surgery of the foot and ankle, including ankle replacement surgery Dr. Jackson joined OCR in He completed medical school at Northwestern University, residency at Henry Ford Hospital, and foot-ankle fellowship at University of Texas/ Houston-Baylor. Robert M. Baer, MD Orthopaedic Surgeon, Trauma-Fracture Surgery Rob Baer specializes in trauma and fracture surgery. Dr. Baer joined OCR in He earned his MD at St. Louis University. Dr. Baer completed his residency at Henry Ford Hospital, and an orthopaedic trauma fellowship at Panorama Orthopedics/ St. Anthony s Central.

7 Conference Presenters continued Riley W. Hale, MD Orthopaedic Surgeon, Hip Replacement Trauma-Fracture Surgery Riley Hale specializes in anterior hip replacement and traumafracture surgery. He joined OCR in 2016 after five years of practice with a health system medical group in northern Colorado. Dr. Hale completed medical school at the University of Nevada, residency at the Mayo Clinic, and a fellowship in orthopaedic trauma at Florida Orthopaedic Institute-Tampa General Hospital. Joshua T. Snyder, MD Orthopaedic Surgeon, Hip Arthroscopy Surgery Josh Snyder specializes in hip arthroscopy and sports medicine. He joined OCR in 2015 after six years of practice with a health system medical group in northern Colorado. Dr. Snyder earned his MD from Chicago Medical School. He completed his residency at Loyola University Medical Center and a fellowship in sports medicine at the University of Chicago Hospital. Steven J. Seiler, MD Orthopaedic Surgeon, Hand & Upper Extremity Surgery Steve Seiler specializes in hand and upper extremity injuries and surgery. Dr. Seiler joined OCR in He completed medical school at University of Texas/San Antonio, residency at University of Tennessee/Campbell Clinic, and hand-upper extremity fellowship at the Hand Center of San Antonio. Michael J. Houghton, MD Orthopaedic Surgeon, Athletic-Reconstructive Surgery of the Foot and Ankle Mike Houghton specializes in athletic and reconstructive surgery of the foot and ankle, including ankle replacement surgery. Dr. Houghton joined OCR in He completed medical school at the University of Colorado, residency at the University of Wisconsin Hospital, and fellowship at Harborview Medical Center, Seattle. He is part of an NIH study comparing ARS with ankle fusion.

Dr. Clark joined OCR in He earned his MD at the University of New Mexico. He completed his residency at the University of South Carolina/Greenville Hospital System, and fellowship in joint replacement at the University of Utah. Bret C. Peterson, MD Orthopaedic Surgeon, Hand and Upper Extremity Surgery A native of Fort Collins, Dr. Peterson joined OCR in He earned his medical degree from the University of Colorado. Dr. Peterson completed his residency at Duke University and hand-upper extremity fellowship at the University of Washington. Dr. Peterson serves as the course director for the conference. Kurt T. Dallow, MD Primary Care Sports Medicine Specialist Kurt Dallow s specialty is non-operative sports medicine. Like Dr. Hale, he joined OCR in Dr. Dallow has practiced family medicine and sports medicine in northern Colorado since He also served as a faculty member at a family medicine residency program. Dr. Dallow earned his MD at the University of Colorado. He completed his residency at Sioux Falls, S.D., Family Practice. He is board certified in family medicine and has a C.A.Q. in sports medicine.
