2 Lateral Transfer Data Report Prncpal Investgator: Andrea Baptste, MA, OT, CIE Co-Investgator: Kay Steadman, MA, OTR, CHSP Sgnfcance Executve Summary: Accordng to the Bureau of Labor Statstcs, nursng ades, orderles, and attendants consstently ranked among the detaled occupatons reportng the most cases of workplace njures and llnesses durng the perod. Nursng ades, orderles, and attendants reported the thrd hghest number of njures and llnesses n Only truck drvers (heavy and tractor-traler) and laborers and materal movers (hand) had more cases. A healthcare patent was the leadng source of njury among nursng, psychatrc, and home health ades, accountng for 59 percent of ther njures from 1995 to The most recent data avalable from 2010 BLS data showed the rate of njury n healthcare had rsen 4% over 2009 data whch specfed 7.0%. The event leadng to njury: Overexerton was the cause of more than half (53%) of work-related njures and llnesses among nursng, psychatrc, and home health ades from Most of the cases of overexerton among nursng, psychatrc, and home health ades resulted from lftng patents. Indeed, ths group accounted for 65 percent of all such njures that occurred durng the 10-year perod. The purpose of ths analyss s to quantfy the workload placed on the caregver durng a lateral transfer usng the Samart Rollbord and other types of lateral ads. Snce lateral transfers are done at a hgh frequency and the healthcare patent s the leadng source of njury, to better understand and try to mnmze ths exerton, a bomechancal evaluaton was performed. Ths bomechancal assessment wll also help us determne whch devce s easer on the caregver s low back and shoulder and whch one causes more stran and stress durng the lateral transfer. Methodology A laboratory analyss was conducted to capture the dynamc bomechancal demands the caregver experences durng laterally transferrng a patent. The technology utlzed can provde objectve data of what s occurrng at each jont of a human subject. The results 1

3 collected and reported reman objectve due to the ntent of the evaluaton. A subject was labeled and moton was tracked n real tme, enablng us to look at forces and stresses on the low back. The evaluaton was conducted n a laboratory settng usng multple cameras, two dfferent weghted ADTs to represent the average weght and heght of a fully dependent patent. Data Analyss 1. Provde bomechancal data at the low back and shoulder durng a lateral transfer of an average and baratrc patent when usng the Samart Rollbord. 2. Compare the Samart Rollbord wth the followng other lateral transfer devces: an ncontnent pad/draw sheet (tradtonal method), a frcton reducng devce (FRD) sheet type, a slde board, an ncontnent pad over slde board and an ar asssted devce. 3. Calculate the bomechancal stresses on the lower back (L5/S1) and shoulder usng a 50 th and 95% tle anthropomorphc test dummy (ATD). 4. products n order of least to most physcally stressful or demandng. Desgn: Ths project smulated a commonly performed nursng task n a controlled laboratory settng (a lateral transfer). Ths task nvolves the caregver to logroll the patent to nsert the product and then pull the patent over to another surface laterally. The desgn and testng protocol was set up wth rest perods n order to avod caregver (subject) fatgue. Two dfferent ADTs were utlzed to represent the average weght and heght of a fully dependent patent (50 th percentle and 95 th percentle). Each ADT was log rolled to place the lateral transfer devce under t, and then the lateral transfer was performed. The lghter ADT weghed approxmately lbs., and represented the 50 th percentle patent, whle the other ADT weghed 223 lbs. and was representatve of 95 th percentle patent populaton. A female who was approxmately 5 4 tall and 167 lbs. n weght represented the caregver. Assstve Devces: The assstve devces used n ths evaluaton ncluded two surfaces, two ADTs, the testng apparatus and the followng lateral transfer devces: 1) Incontnent pad/draw sheet-standard method (IC pad) 2) Incontnent pad over slde board 3) Frcton Reducng sheet 4) Slde board 5) Samart Rollbord 6) Ar-Asssted Lateral Transfer Devce 2

4 Results: The analyss focusng on the stresses affectng the low back and the shoulder whle usng Samart Rollbord n comparson to other lateral transfer devces proved nterestng. Due to the nature of ths task, lateral transfers are hgh rsk due to the extended reach, pull force, log roll, weght of patent, and frcton of sheets whch all result n hgh forces. Caregvers are at rsk of njury from hgh peak forces through varous body segments. The area of concentraton presented n the result secton s the low back and the shoulder jont snce there have been numerous low back and shoulder njures reported as a result of ths task. Our goal was to quantfy these factors n an attempt to determne how much rsk s nvolved whle usng each devce. Peak force can be defned as the maxmum force developed durng a muscle acton. Bomechancal data s as follows wth the followng products: Incontnent Pad, Incontnent pad over slde board, frcton reducng sheet, slde board, Samart Rollbord, and ar asssted. The followng tables present peak resultant forces at the low back and shoulder usng the 50 th and 95 th %tle ADT s. Several types of log roll technques were analyzed to dentfy forces on the spne. Based on the vdeo on McAuley Medcal webste, the llustraton of logroll placement s to hold at the patent s shoulder and pelvc or hp area to logroll. However, our evaluaton shows that there are better technques to lower the forces to the spne. The technque used to logroll the patent can cause varyng degrees of stran on the caregver s low back and ths physcal stress n addton to the effort used to transfer the patent all contrbutes to the total forces exerted throughout the body. The type of lateral transfer devce used and the type of logroll used wll produce dfferent results on the spne. Therefore, forces at low back can be reduced by changng the method of the log roll as seen by data collected. 5

7 Summary: Based on the results, the analyss ndcates that Samart Rollbord ranks frst n terms of the least bomechancal loadng on the low back when laterally transferrng a 95 th %tle ADT, and second when laterally transferrng a 50 th %tle ADT. The Samart Rollbord also proved to reduce forces at the shoulder jont n comparson to other lateral transfer products. Results dsplay that the Samart Rollbord ranked frst n havng the lowest peak resultant forces at the shoulder durng the lateral transfer of the 50 th and 95 th %tle ADT s. Tme taken to perform the transfer was comparable to usng other transfer ads. The nature of the Samart Rollbord s that t has a frm board covered by a tube whch rolls easly, thus movng the patent. The beneft of ths s that the board offers a frm support surface for the patent durng the transfer and the tubular desgn offers the ablty to easly move the patent from one surface to another wth a reducton n frctonal force. The product s ablty to fold s of beneft snce storage s always a challenge n most facltes. The Samart Rollbord has proven to be a benefcal soluton n reducng the forces on the caregvers spne durng lateral transfers. It should be noted that the data presented here was attaned through bomechancal analyss and fndngs are objectve therefore the rankng of devces s not an endorsement of one product over another.
