11 CONCUSSION PROTOCOL BE PREPARED Parents, coaches and referees should all understand what a concussion is, how to identify it in athletes, and what to do if you suspect a concussion in an athlete. What is a concussion? It is a complex process where a blow to the head or body results in forces on the brain that move it inside the skull. This leads to physical symptoms (e.g., headache or dizziness), cognitive symptoms (e.g., memory loss or confusion) or emotional symptoms (e.g., being unusually tearful). CONCUSSIONS USUALLY DO NOT SHOW UP ON CT/MRI. See the following for more information on concussion. cdc.gov/concussion/headsup aan.com/practice/sports-concussion-toolkit If 12 or older, obtain a baseline concussion test before the sports season. This can be obtained through: School athletic trainer/physician Sports med/concussion specialist AT THE TIME OF INJURY Which symptoms after a blow to the head or body are suspicious for concussion? Headache Dizziness Nausea Change in vision Light or noise sensitivity Change in behavior Confusion about basic information (day of the week, opponent, plays, recent events, etc.) If a concussion is suspected, does the child have any of the following? Breathing problems Unconscious one minute or longer Neck pain or numbness/tingling Seizure Worsening headache Worsening confusion Persistent balance or walking abnormality >1 episode of vomiting YES URGENT TRANSPORT TO HOSPITAL FOR EVALUATION Ohio Youth Concussion Law: Any person removed from practice or play because of a suspected concussion may not return that day and needs written clearance by an appropriate medical provider before return to play. NO Remove from play Encourage rest and restrict physical/cognitive activity Limit exposure to loud noises/bright lights Monitor for change in symptoms Team coach and parents establish plan for physician evaluation IF ATHLETE WAS TRANSPORTED TO EMERGENCY DEPARTMENT ED should not make return-to-play decision. MAKE AN APPOINTMENT WITH DOCTOR IMMEDIATELY! Within 72 hours, follow up with physician (e.g., pediatrician, primary care or sports medicine) for reassessment, recovery plan, possible school restriction and/or additional medical referrals Symptoms can worsen in the initial days after a concussion If athlete has severe/worsening headache, vomiting two or more times, increasing confusion/irritability, seizure or weakness/numbness at any time following concussion, rapid physician referral or transport to emergency department RECOVERY AND RETURN TO PLAY Encourage brain rest and avoid additional injury: No activities that could cause other concussions (e.g., sports, bike riding, etc.) No exercise unless cleared by a doctor If symptoms worsen during or after an activity, stop and wait 48 hours before trying again Get a good night s sleep; no late nights or sleepovers You do not need to wake the athlete every hour Avoid loud noises, bright lights and overly stimulating activity: Limit TV time, cell phone use/ texting, video games, computer time, etc. No loud music, including concerts and dances No sporting event attendance Limit homework/reading or any activity that requires extended periods of concentration to minute blocks Stop activity if symptoms worsen For some athletes, changes in school may be needed. Possible changes include: A break from school and homework A progressive return to school (half days, regular breaks in a quiet space during the school day, etc.) Avoiding classes with loud noises (choir/band), bright lights (computers) or physical activity (PE) Temporary changes in schoolwork may be needed (no or reduced tests/homework, extra time on work, etc.) WHEN TO SEEK A REFERRAL FOR A SPECIALIST: If symptoms persist for three to four weeks or more If symptoms are worsening or interfering with school If two or more concussions If history of ADHD/learning disorder, neurological disorder, depression/anxiety or other medical problem that can interfere with recovery YES Refer to physician/neuropsychologist who specializes in concussion NO 12 OR YOUNGER: Symptom-free at rest, youth concussion testing (if available) is read as normal by a specialist, and physician recommends return to play 13 OR OLDER: Symptom-free at rest, concussion testing (if available) is read as normal by a specialist, and physician recommends return to play NO YES Complete progressive return to sport under the supervision of a physician, athletic trainer, physical therapist or other qualified member of your sports concussion management team. OHIO LAW REQUIRES WRITTEN CLEARANCE BEFORE RETURN TO PLAY ONE SIZE FITS ALL DOES NOT APPLY TO CONCUSSIONS. A concussion treatment plan should be targeted to a specific athlete and his/her specific symptoms. Not all of these recommendations may apply for a given athlete. Also, additional recommendations may be needed. Consult your doctor for the best recovery plan. Symptoms usually resolve in the initial weeks after injury. 20 University Hospitals PLAY UHSports.org University Hospitals PLAY UHSports.org 21

Athletes are encouraged to undergo baseline testing prior to the season, and if they sustain a concussion, testing is repeated to identify when symptoms have returned to normal and an athlete is ready for a return to sport, school and other activities. Should complications occur in recovery, expert neuropsychologists who specialize in concussion evaluation and testing will evaluate the patient, which can assist with treatment plan development, school recovery plans and evaluating responses to treatments. Our neuropsychologists are specially trained in sports concussion and have years of experience managing concussions at all levels of sport, including the use of tools such as Immediate Post-Concussion Assessment and Cognitive Testing (ImPACT Concussion Testing). Further evaluation into memory changes, concentration problems and other thinking/behavioral changes associated with concussion can be made through more comprehensive evaluation by neuropsychologists. This comprehensive evaluation helps to guide treatment recommendations, monitors an athlete s response to treatments, can provide a fuller understanding of whether an athlete with a concussion has fully recovered, and can assist with detailed recommendations for return to school and other activities. Whether you are a professional athlete, weekend warrior or little leaguer, our rehabilitation team will help you get back in the game. Sports injury, orthopedic and post-operative rehabilitation ACL injury prevention Overhead athlete injury prevention Manual therapy Many children and adolescents have emotional difficulties, and regrettably, many of them never get the help they need. Our team at the UH Rainbow Babies & Children s Division of Child and Adolescent Psychiatry provides a full scope of psychiatric evaluation and treatment services, either directly or through referral to affiliated staff and programs. Our division offers an environment on the leading edge of treatment, research and education, with a clear focus on children and adolescents. Individual evaluation determines the best treatment for each child, and the first step we take is always getting to know your child. Unlike standard medical illnesses such as colds or earaches, emotional problems don t always present clear-cut symptoms. However, you may become aware of behavioral or emotional inconsistencies that may be a cause of concern, including sudden anger or outburst, reduced level of communication, unexplained drop in school performance or disinterest in formerly enjoyed activities. VINCENT CARINGI, MD Department of Psychiatry Child/Adolescent Euclid Avenue The rehabilitation staff at UH is fully certified and highly skilled. Our therapists work one on one with patients to optimize your rehab for the quickest recovery possible and communicate directly with the sports medicine physicians to provide the best possible care and progress to rehab appropriately. Additionally, we work with athletes to decrease pain and swelling, restore range of motion and increase strength following a sports injury or surgery. In addition, multiple services are offered for sports performance that increase speed, explosive power and jumping ability. Leading-edge services such as dry needling, blood flow restriction rehabilitation, instrument-assisted soft tissue mobilization, Alter G treadmill training and sport-specific video analysis are additional resources to help athletes reach their fullest potential on the field of play. We offer comprehensive treatment options for both routine and more complex conditions and are able to specialize plans based on unique needs, including these targeted therapies: Comprehensive exercise prescription Concussion rehabilitation and management Aquatic therapy Hand therapy Running gait video analysis Alter G treadmill training Blood flow restriction rehabilitation Dry needling Joint mobilization and manipulation Cupping Kinesiotaping Instrument-assisted soft tissue mobilization Return-to-play Sport-specific video analysis You know your child better than anybody else does, so use the above to help determine if a call to the doctor is necessary. Remember that many emotional conditions begin with these types of symptoms and can grow more serious if left untreated. Based on the needs of your child, treatment options could include individual, group and family psychotherapy; individualized behavior therapy; psychopharmacotherapy; or a combination of these. 16 NUTRITION SERVICES Sports nutrition is the study of evidence-based practice on the role of fluids, diet, nutrients and supplements related to the performance of recreational and competitive athletes, on site and during travel. A sports dietitian is a highly trained food and nutrition expert who can help athletes and fitness-minded individuals and teams meet their nutrition and performance goals. A sports dietitian has additional training and certifications that relate to the role of nutrition and performance. Reaching performance goals can be achieved through: Counseling individuals and groups on daily nutrition for performance and health Making sense of the latest scientific evidence through practical sports nutrition recommendations Tracking and documenting outcomes of nutrition services Serving as a food and nutrition resource for coaches and trainers Providing sports nutrition education for health/wellness programs, athletic teams and community groups Our sports dietitians also provide medical nutrition therapy that focuses on behavioral and lifestyle changes that positively affect health and athletic performance. They offer care that consists of nutrition assessment, diagnosis, intervention and monitoring/evaluation. The benefits of meeting with a sports dietitian include: Better knowledge of the role of carbohydrates, proteins, fats and fluids on athletic performance Better understanding of what types of foods are most beneficial for performance Assistance with timing of meals and fluids to provide optimal performance Recovery foods and meal suggestions to assist the athlete with getting back into the game sooner UH s clinical nutrition therapy has adult and pediatric dietitians who are food and nutrition experts with training and credentials to translate the most up-to-date sports nutrition research into healthy living and optimal performance.
