During that same time period, I served as a member of the Finance Committee for the Gaza Baptist Mission, which included budgeting for all ministries of the Mission, including a 75-bed general hospital. I had primary responsibility for financial matters for the outpatient department at Fort Worth Rehabilitation Hospital ( ) as Outpatient Supervisor and during my tenure as Inpatient Program Manager at Hendrick Medical Center in Abilene, TX ( ). To run or not to run? This is the question I asked myself over and over. After all, serving as Treasurer and as a member of the Board of Directors for the FSBPT requires a huge time commitment and carries tremendous responsibility. If you are reading this statement, then you know my answer to that question, and I am ready to make that commitment and carry that responsibility. I am truly honored to be slated as a candidate for this position. Throughout my career as a physical therapist, I have always been an advocate for the consumers of our services. This led me to seek appointment to the Texas State Board of Physical Therapy and drove me to seek ways to become involved with the FSBPT. My first exposure to the FSBPT was as an item writer in Since that time, I have served on various task forces related to the NPTE and have been involved in the continuing competence initiative. Service as a member of the Finance Committee has helped me understand more fully the challenges and opportunities for the FSBPT to fulfill its mission. I firmly believe in this mission, and would be honored to serve as an officer and member of the Board.

2 If you ask those who know me, they will tell you that I can be quite opinionated and passionate; that I am not afraid to express my viewpoint and ardently argue my case. They will also tell you that once a decision has been made, I will work diligently to support and carry out the required actions. That would be my approach to serving on the Board. I would state my case with great conviction and support the decision of the Board with the same fervency.

I have found the past three years as Secretary to be challenging yet personally and professionally rewarding. As a member of the Board of Directors, I have been liaison to various facets of the Federation. These include exam development and security committees, practice analysis task forces, technical advisory committee and the education committee. In addition, I have been liaison to external organizations such as the Association of Test Publishers and have attended the APTA Board of Directors Meeting and the APTA Delegate Assembly. Future challenges to FSBPT include insuring that competent applicants have access to the National Physical Therapy Examination, investigating the implementation of possible supervised clinical practice for foreign trained applicants, facilitating an international network of physical therapy regulators through INPTRA, possible participation in federal initiatives to investigate potential shortages of physical therapy practitioners, and fostering effective communication with member jurisdictions. I have been involved with FSBPT for more than 15 years in a variety of member and chair positions including Resolutions, Nominating, Education, Examination, and FCCPT as well as positions with state components of APTA. I have served for 10 years as member and chair of the New York Physical Therapy Board. Throughout my over 40 year career as a physical therapist, I have been able to combine academic, research and clinical expertise in relation to physical therapy. I characterize myself as a team player who possesses a strong work ethic and a high degree of professionalism, having strong communication skills, as well as being an effective problem solver. I look forward to discussing issues with the delegates in Indianapolis and would appreciate your support for reelection to the FSBPT Secretary Position.

5 My interest in increasing my involvement within the FSBPT starts with my passion for regulation, compliance and the profession of physical therapy. My career has been strategically designed to lead me in this direction, that is where I find my personal and professional gifts lend themselves most appropriately. From the first moment I began working with the Federation I realized I found an organization which matched my skills, and desires most clearly. When contemplating what role to aspire to, it was clear to me with my experience, and appreciation for all things numerical and strategic, that the role of Treasurer was most suited to my expertise. The challenge is to have the confidence to fill a role with the history such as the position of Treasurer of the FSBPT Board of Directors. The amazing contributions of the past Treasurers will challenge anyone in this role to aspire to greatness. The teams and organizations I have been blessed to work with have reinforced my abilities in team building and strategic planning. These experiences have prepared me to work with organizational boards, bringing my expertise along with a focus on collaboration and effectiveness. I look forward to opportunities to learn and be challenged, while impacting growth and fiscal awareness within the Federation. This is why I have sought to be nominated, and elected Treasurer of the Federation of State Boards of Physical Therapy Board of Directors. Thank you.

6 Jeffrey Rosa I have served as the administrator for the Ohio PT Board since October In this position, I m also the administrator for the professions of occupational therapy and athletic training. Prior to my current position, I spent one year as the Legislative/Regulatory Specialist for the Ohio Board of Nursing. Member of the FSBPT Board of Directors since 2008 Candidate for FSBPT Vice President in 2010 Chair of the Council of Board Administrators in 2005 and 2008 Member of Finance Committee from 2004 through 2008 Attended all Federation annual meetings since 2004 NPTE Summit participant in 2011 Member of the Compensation Committee since 2010 Member of the Investment Committee since 2011 Executive Director of a state agency with an annual budget of over $850,000 and annual revenues of approximately $1 million Worked for 6 years as a Senior Budget Analyst for the Ohio Legislative Budget Office Vice Chair for the Ohio Boards and Commissions Management Group, at which the administrator s for Ohio s autonomous regulatory boards discuss best practices and other issues related to regulation Member of the Board of Certification Regulatory Conference Planning Committee since 2007 Member of the Ohio e-licensing policy group, which establishes policies regarding Ohio s licensing database Member of the Ohio e-licensing RFP committee, which is determining the requirements and selecting a replacement licensing system for Ohio s regulatory boards On the Board of Directors for a local community band I agreed to run for Treasurer so that, if elected, I can use my knowledge of, and passion for, the Federation to its fullest. Since my appointment as the administrator for Ohio s physical therapy board, I have been extensively involved with Federation activities. Early in my tenure with the Federation, I took a leadership role with the Council of Board Administrators and was appointed to serve on the Finance Committee. I was fortunate enough to be elected as the administrator on the Board of Directors in As the administrator for Ohio s autonomous physical therapy board, I also serve as the Executive Director of an independent state agency. As my role as the Executive Director, I am directly responsible for preparing the agency s biennial operating budget and forecasting future revenues to ensure that the agency raises enough funds to cover operational costs. Prior to my time with the Board, I served as a senior budget analyst with the Ohio Legislative Budget Office. In this position, I analyzed agency budget request for the Ohio General Assembly, as well as the fiscal impact of proposed legislation. The issues related to exam security and the implementation of fixed date testing for both the PT and PTA exams will have a significant impact, both operationally and fiscally, for the Federation. My current and previous work experience, as well as my experience serving on the Finance Committee for 4 years and now as a member of the Board of Directors for the past 4 years, makes me uniquely qualified to serve as the next Treasurer of the Federation. 7 Candidates for Director Mark Westover Cornwall, PT I was appointed to the Arizona Board of Physical Therapy in I was elected as Secretary in I then served as the Vice-President from 2007 to In 2010, I was elected as the President of the Arizona Board of Physical Therapy and have held that position since then. I have served as either a delegate or alternate delegate to the annual meeting of the FSBPT three times. The profession of physical therapy has been very good to me and I feel strongly that it is important for me to give back to it and try to make it better. I have strived to do this in many ways, including the provision of quality patient care, education of future therapists and serving within various organizations. Physical Therapy has a great deal to offer both society and the individuals that are treated by its practitioners. As a physical therapy educator, I want students to understand the great privilege that they have been granted by becoming a physical therapist and also the tremendous responsibility they have to make a positive impact not just in the lives of the patients that they treat, but also to the profession as a whole. My love of the profession and what it offers played a large role in my desire to become involved in the regulatory side of the profession. I have been a member of the Arizona Board of Physical Therapy for the past 7 years and have cherished and enjoyed every minute of it. I am now the President of that Board. I sincerely believe in the role of professional regulation and see it as yet another way to advance the profession. As such, disciplinary hearings become more about helping physical therapists and physical therapist assistants improve their patient care rather than simply punishing them. Seeking election to the FSBPT Board of Directors is yet another way for me to give back to the profession. I would bring to the Board a solid understanding of the NPTE, the intersection between education, practice and regulation and the need for documenting continuing competence. I feel that I work well in a team environment and prefer to gather as much information as possible before making decisions.

This position has allowed me to develop the ability to create short and long term goals, effectively work with others, and achieve successful outcomes. I have been an active member of the North Dakota Physical Therapy association including committee membership and leadership. I am honored to be considered for the position of Director on the FSBPT Board of Directors. The Federation is an organization of exceptional individuals, dedicated to achieving the mission of protecting the public. I am grateful and humbled to have the opportunity to interact, learn from, and serve the many people associated with the member jurisdictions and their Federation. The Federation continues to make substantial progress in achieving goals associated with the Areas of Focus. At the same time, the concerns surrounding the validity, reliability, and security of the examination continue to be a prominent issue for the organization. My experiences with the Federation began as an item writer and progressed to an appointment as an exam development committee member and eventual co-chair. The experience with the exam exposed me to valuable information regarding item types, the item bank, test content outline, exam form development and exam security. As a member of the Board of Directors, I have been able to capitalize on the knowledge of exam development during the NPTE Summit, recent practice analysis, and adoption of the new exam content outlines. I believe my experiences in exam development will be beneficial to the Federation as the critical issues of examination security and the future direction of the examination are addressed. While the NPTE is an important component of the Federation, a Director must also understand the complexity of supporting member jurisdictions in regulating the profession of physical therapy. To that end, I have participated in educational opportunities through the Federation and have engaged board administrators and jurisdictional board members in conversations to better understand the myriad of issues affecting regulatory bodies. I believe that the characteristics of integrity, hard work, humility, and accountability are integral to an effective member of the Board of Directors. I believe my interactions with members, volunteers, staff and leadership have demonstrated these characteristics during my previous service for the Federation. I truly appreciate your consideration of my candidacy for the office of FSBPT Board of Director.

My licensing experience in conjunction with my work with the FSBPT, its staff, and members over the past five years would be highly beneficial. Past member and current Chair of the DC Board of Physical Therapy Other experience pertinent to position Was member and Chairman of APTA Nominations Committee. The Nominating Committee is one of the most important elective bodies of an organization because it holds the growth and diversity of talent within its hands. It should seek out the emerging leaders and encourage active participation at all levels even if a particular job is not open for competition at the time. Even though the meeting times for the committee may be few, the committee should be working all year to ensure that it is gathering information and mentoring future leaders. This ensures a robust slate whenever it is needed and discourages recycling of the stalwart officers and Board Chairs. I am willing to pledge that I take this responsibility seriously and use all of my contact and interpersonal skills to convince our members that they are indeed ready to assume different roles, even if they have not yet reached that conclusion for themselves.
