5 Appendix A: Determining Severity of Need (#19 under All Projects) Utilizing question 18.a. in the Client Detail Annual Progress Report (run for calendar year 2015 for all projects) from the, the number of disabling conditions a person had is multiplied by the number of people experiencing that number of conditions (Column E below).
