How to make a case to the new clinical commissioning groups (CCG) If CCGs do not eventuate basics should still apply Who to look for and how to recruit
Statement of context (facts & figures) Incidence / prevalence of stroke in your community Psychological issues that occur with stroke Impact on rehabilitation outcome and cost to the community
bed days care utilisation institutionalisation GP visits and acute admissions carer stress
Statement of context (policy) Royal College of Physicians Psychological care is best delivered in a service with full access to clinical psychology. Each stroke rehabilitation unit and service should be organised as a single team of staff with specialist knowledge and experience of stroke and neurological rehabilitation including clinical psychologists (RCP, 2008) A considerable increase in clinical psychology has been called for (RCP, 2008)
Statement of context (policy) NSF Long Term Conditions QR4, QR5, QR8 Co-ordinated multidisciplinary care Counselling and psychological support to help people adjust Psychology to help people maintain their basic health and well-being at home (DoH, 2005) Stroke Improvement targets Patient survey findings
Statement of context (policy) - Helsingborg Declaration on European Stroke Strategies Assessment by a neuro-psychologist should be available when needed (Kjellstrom et al., 2006) British Psychological Society Psychological Services for Stroke Survivors and their Families - Revised Briefing Paper 19 (BPS, 2010)
Statement of context (policy) Accelerating Stroke Improvement (ASI 6) target Proportion of patients who have received psychological support for mood, behaviour or cognitive disturbance by six months after stroke ( was 40% by April 2011) (NHS Improvement, 2010)
Statement of context (policy) Patient survey findings Over 40% do not think they got enough emotional support from the NHS after leaving hospital (Healthcare Commission, 2006)
Statement of context (policy) National Audit Office Progress in Improving Stroke Care At least some psychological support after stroke should be provided by psychologists (Comptroller & Auditor General, 2010)
Why a Clinical Psychologist? Nurses, occupational therapists, counsellors and others may have a role in the assessment and management of behavioural, emotional and cognitive concerns after stroke. However no other profession has undergone the extensive training that makes CPs the specialist in these areas, allowing them to manage these issues comprehensively, deal with complex cases and at the same time be able to contribute at an advanced level to audit, research, team development and training.
Garner user support Not just refer to their views in position papers, get their contribution personally and specifically to your proposal Stroke Association, Speakability, Connect, Different Strokes Stroke users groups in your local area; patients and carers
Maximise the perspective of the clinicians who will be the new NHS decision makers: What does the care pathway look like now? How would it look with the clinical psychologist in place? What would that mean for the clinician?
What sort of psychologist do you want: Clinical psychologist or clinical neuropsychologist?

Clinical psychologist or clinical neuropsychologist? Clinical Neuropsychologist: generally band 8b and above Can you offer to sponsor the training to secure an appointment?
How will you set up the position to be attractive to potential applicants? -Employed by a local Mental Health Trust or directly by your service? -What professional support will be available for the individual, particularly in the latter case?
Advertising NHS Jobs BPS The Psychologist and online Directly through the Division of Neuropsychology
Advertising Directly through the Clinical Neuropsychology training courses: University of Glasgow University of Bristol
SUMMARY -Establish the need (statistics and policy) -Cost / benefit analysis -Involve the third sector -Focus on the clinicians point of view -Use a focussed approach to marketing
