SUPERVISORS AND ADMINISTRATIVE STAFF LISTING South Bay Consortium Postdoctoral Residency Program Kaiser Permanente Northern California South Bay Consortium 2017/2018 Training Year Training Program Administrators R. Marcus Green, PhD I received my PhD in clinical psychology at Brigham Young University, where my studies emphasized child and adolescent treatment. I completed an internship at Spokane Mental Health in the Department of Child and Family Services in 2009, and completed a post-doctoral residency at Kaiser Permanente in the Child and Adolescent Psychiatry Department in 2011 at the San Jose Medical Center. Although I enjoy treating a wide variety of disorders, my special interests include behavior problems for children and teens of all ages, ADHD, and issues related to depression and anxiety. I believe that treatment is most effective when it is the result of the collaborative efforts of therapists, parents, extended family, school, and community resources working together to help a child or teen and believe in using evidence-based approaches to treatment that are well researched. I am currently a primary supervisor and co-training director for the post-doctoral residency program in the South Bay Consortium. I have found that I enjoy the energy and new perspectives that residents bring to our program. My goal is that each resident feel comfortable working independently by the end of their residency. Amara Brook, PhD I am a licensed clinical psychologist in adult psychiatry. I received my bachelor's degree from Harvard in 1996 and my PhD from the University of Michigan in After grad school, I was a psychology professor at Santa Clara University, where I studied self-regulation and enjoyed teaching and mentoring undergraduates. A strong desire to put research into practice led me to pursue additional clinical training through the California School of Professional Psychology and obtain my license. Prior to joining Kaiser in 2015, I worked in two VA medical centers (San Francisco and Northern Arizona) and with adolescents in schools. Now I enjoy helping patients improve their self-regulation and supervising post-docs. I have a strong interest in DBT, CBT, and ACT and love teaching groups. I especially enjoy working with young adults and I have particular interest and experience in treating bipolar disorder, borderline personality disorder, trauma, ADHD, and anxiety disorders. When not at work, I enjoy outdoor adventures with my family and friends. 1

sional=aboutme.xml&ctab=about Me&cstab=Professional Kathleen Bonal, PhD I received my PhD in Clinical Psychology from the University of Massachusetts at Amherst, and I did my internship at the V.A. Medical Center in West Haven, Connecticut. I have a long history of and strong interest in training, starting with training interns at the Institute of Living in Hartford, Connecticut (Hartford Hospital s Mental Health Network), and now training postdoctoral residents. I have been working in adult psychiatry for Kaiser Permanente in San Jose since My clinical interests include OCD, eating disorders, and projective testing. Although my background and training has been primarily psychodynamic, I do a lot of evidencebased practice. I also believe in the importance of nutrition, exercise, and self-care in good mental health. I enjoy the challenges of working with such a diverse population as we see in San Jose as well as with such a range of diagnoses and symptom presentations. Within the post-doctoral training program, I am the site training director for adult psychiatry in San Jose, and I supervise individual therapy as well as psychological testing. In working with the residents, I strive to help each trainee develop and nurture their clinical thinking and skills, as well as to help each trainee develop a sense of professional identity. My goal by the end of the training year is for the resident to feel more comfortable and capable in functioning autonomously as a psychologist. With psychological testing, my goal is to help the resident further hone their skills at making clinical inferences from test data and integrating those inferences into a cohesive report. I enjoy the energy and enthusiasm that residents typically bring to their training, and I am focused on making the training year as good and productive an experience as it can be for the resident. essional=aboutme.xml&ctab=about%20me&cstab=professional Individual Supervisors Andrew Berger, PhD Andrew.C.Berger@kp.org I received my Bachelors of Arts in Psychology from the University of California at Berkeley and my PhD in Clinical Psychology from the California School of Professional Psychology in Los Angeles. I have a special interest in the graduate level training of post-doc residents. A significant portion of my time is spent as Training Director of the South Bay Consortium. My clinical interests include teen and child family issues, depression and anxiety, in addition to difficulties coping 2

with life changes, such as divorce and blended families. I tend to see child and teen mental health issues from a family perspective, as it is difficult to separate a particular problem from the social environment. Although I see working with the family as key to addressing many mental health issues, there are times when an issue requires individual treatment and believe short-term problem-focused therapy can be very effective. I am also a member of our urgent on-call team, and do Emergency Department 5150 evaluations and Hospital Consults. I have supervised post-docs for over 10 years. I see supervision as more directive at the beginning and then becoming more collaborative as the year progresses, to help post-docs develop their own style and independent functioning, in preparation for licensed practice. I tend to conceptualize cases more dynamically but my interventions are more consistent with CBT, Family Systems and Integrative therapy. I find that supervising keeps me up to date on the more recent developments in clinical psychology, as I learn so much from our SBC residents. I really enjoy the collaborative approach to the supervision relationship. essional=aboutme.xml&ctab=about Me&cstab=Professional Jennifer Clark, PsyD Jennifer Clark, PsyD, completed her PsyD in Clinical Psychology at the American School of Professional Psychology at Argosy University, Washington, DC, in Prior to graduate school, she obtained her Bachelor of Arts in Psychology and Forensics at Chatham University in Dr. Clark completed her APA-Accredited Internship at Astor Services for Children and Families and enjoyed providing therapy, assessment, consultation, clinical supervision, and program evaluation in both residential and outpatient settings. In general, Dr. Clark has received clinical training in a wide variety of settings with a clinically and culturally diverse population of children, adolescents, young adults, and families. Dr. Clark participated in her postdoctoral residency at Kaiser Permanente, Santa Clara, South Bay Consortium, and joined Kaiser Permanente as permanent staff in June Her professional interests include mood and anxiety disorders, trauma, autism spectrum disorders, evidence-based treatment, family therapy, psychological assessment, clinical supervision, and the use of outcome measures. Dr. Clark currently provides individual, family, and group psychotherapy, leads parent classes, and participates in the intensive outpatient program. Dr. Clark utilizes an integrative, solution-focused, and strengths-based approach with children, adolescents, and their families. As a supervisor, Dr. Clark takes a collaborative, developmental approach. She strives to provide a supervisory experience that matches the trainee s developmental level, experiences, and needs. Dr. Clark is excited to become a part of the Kaiser South Bay Consortium Training Program and looks forward to supporting incoming postdoctoral residents in learning and growing as clinicians. essional=aboutme.xml&ctab=about Me&cstab=Professional 3

Harshita Desai, PsyD I received my Bachelors of Arts in Psychology and Human Development from the University of California, Davis, in 2009 and obtained my PsyD with an emphasis in family psychology from Azusa Pacific University in I completed my pre-doctoral internship at Monterey County Children s Behavioral Health, where I provided individual, group, and family therapy; case management; and psychological assessments to a diverse population of children, teenagers, and families who were exposed to issues including, but not limited to, complex trauma, mood disorders, substance abuse, psychosis, and chronic abuse/neglect. Prior to internship, I was fortunate enough to receive clinical training in a wide variety of settings including community mental health, an inpatient psychiatric facility, and school-based services. I participated in the postdoctoral residency program at Kaiser San Jose Child and Adolescent Services and was so pleased with my training experience that I decided to join as permanent staff in September My professional interests include mood and anxiety disorders, trauma, autism spectrum disorder, immigration issues, and behavioral problems in children and adolescents. I tend to conceptualize cases from a systemic, multidisciplinary approach and typically treat using evidence-based practices that are founded in principles of CBT and DBT. This year, I am so grateful I have an opportunity to provide secondary supervision and group supervision to our current postdoctoral residents. I value working with postdoctoral residents and find it exciting to help foster training while also learning from others. Peter Ly, PsyD Peter.Ly@kp.org I am a licensed Clinical Psychologist and obtained my doctorate degree from the California School of Professional Psychology (CSPP) in Alameda. Having received my pre-doctoral clinical training at the National Asian American Psychology Training Center of RAMS, Inc. in 2000, I completed my post-doctoral fellowship at Kaiser Permanente Milpitas in Child and Adolescent Psychiatry and joined the staff in I have worked in a variety of settings with adults, children, and adolescents. In addition to my experiences in community mental health centers, I have provided services in inpatient psychiatric facilities when I completed a practicum placement at Santa Clara Valley Medical Center, Barbara Aaron Pavilion (BAP) in I bring a range of training and experiences to my work here at Kaiser Permanente in groups and individual/family psychotherapy. I currently co-lead the Intensive Outpatient Program (IOP) for Middle School Age and am on the Centralized Triage team. My clinical interests include teen and child/family issues, depression, anxiety, and crisis interventions. I tend to conceptualize cases more dynamically but my interventions are more consistent with CBT, Family Systems, and Integrative therapy. I draw much of my work from Schema Therapy. Part of my role and responsibilities in the South Bay Consortium is that I am a primary supervisor and a Site Director. I enjoy both the clinical work and the privilege to supervise incoming post-doctoral residents. l=aboutme.xml&ctab=about Me&cstab=Professional 4

Christine Pearston, PsyD I completed my B.A. in Psychology and M.A. in General Experimental Psychology at the University of Hartford in Subsequently, I completed graduate training at Pace University, obtaining my M.S.Ed. in School Psychology in 2012 and my PsyD in School-Clinical Child Psychology in Prior to joining the Kaiser Permanente team as a postdoctoral resident in September 2014, I trained in a variety of mental health settings, including public schools, private schools for children with special needs, a university-based outpatient mental health clinic, a psychiatric inpatient hospital for children and adolescents, and community mental health agencies. When working with individuals and families, my primary goal is to provide a comfortable environment for open communication and collaboration. I often take a relational approach to therapy and incorporate a variety of evidence-based interventions into my practice. I look forward to working with you and your family to find solutions that work for you. rofessional=aboutme.xml&ctab=about Me&cstab=Professional Janelle Myhre, PhD I am a licensed clinical psychologist in the Department of Adult Psychiatry in Santa Clara. I received my undergraduate degree from New York University and my PhD from University of Arizona, with specialization in neuropsychology. I completed my internship and postdoctoral training through the VA (West Los Angeles and San Francisco, respectively) with focus on geropsychology. My areas of interest are in geropsychology, neuropsychology, and health psychology. My theoretical orientation is primarily based in behavioral and CBT approaches. I currently serve as the assessment supervisor for Adult Psychiatry/CDRP postdocs at the Santa Clara location of the South Bay Consortium. Clinically, I focus on treating a wide variety of adult patients with mental health disorders. I also lead our clinic's ADHD assessment program and facilitate an ADHD skills group, and am part of the Roving Psychology team working in Primary Care and Family Medicine. Meghan Luby, PsyD Prior to working at Kaiser Permanente, I worked in an elementary school, community mental health clinics, a special-day classroom, and in community-based early-intervention. While I have experience working with a variety of issues, my specializations include early childhood development, behavioral problems, trauma, and parenting. I have worked with infants, young children, adolescents, and adults. I typically incorporate attachment, psychodynamic, and family systems theory into understanding my patients presenting problems while integrating evidence-based techniques, narrative therapy, cognitive-behavioral therapy, and parenting 6

support into my practice. I believe in working collaboratively with families in order to identify goals, build upon strengths and create positive change. I received my B.A. from Kenyon College in 2005, and my M.A. and PsyD in Clinical Psychology from the Wright Institute in I completed my pre-doctoral internship at the Institute for Child and Family Health in Miami, Florida, and started as a post-doctoral resident at Kaiser Permanente Child and Adolescent Psychiatry Department, Santa Clara in sional=aboutme.xml&ctab=about Me&cstab=Professional Olga Rosito, PhD Olga Rosito, PhD, is a bilingual (English/Russian) licensed clinical psychologist with a specialty in working with older, traumatized individuals with mood and cognitive issues and their caregivers. I received my Bachelor of Arts in Behavioral Science and Psychology from San Jose State University and went on to complete a PhD in Clinical Psychology at Palo Alto University. As a graduate student, I conducted research on the impact of bereavement in family caregivers of patients with Alzheimer s disease. I also served as a TA for several graduate courses and assisted in administering a distance learning program for the MS in Psychology degree at Palo Alto University. I completed my pre-doctoral internship at the VA Sepulveda Ambulatory Care Center in Los Angeles, providing a wide range of services to veterans with PTSD and other mood disorders. I received specialized training in treating sleep disorders and utilizing acceptance commitment therapy (ACT) and mindfulness based approaches in the treatment of trauma. While at the VA, I supervised practicum students. Prior to joining Kaiser Adult Psychiatry Department in Santa Clara, I completed my postdoctoral training at the chemical dependency clinic at Kaiser Permanente in South San Francisco. I have a strong interest in treating older adults and individuals with traumatic experiences and anxiety. I specialize in applying ACT and mindfulness techniques; I also enjoy using evidence-based sleep interventions and working as part of the integrated primary care team. In addition to my clinical role at Kaiser, I am an adjunct professor teaching undergraduate and graduate level courses in general and cognitive psychology. onal=aboutme.xml&ctab=about Me&cstab=Professional Gail Shen, PhD Gail.H.Shen@kp.org Dr. Shen received her undergraduate degree from the University of Pennsylvania, and her PhD from Temple University in Philadelphia, Pennsylvania. She completed her pre-doctoral internship at the VA of Northern California before joining Kaiser Permanente as a post-doctoral resident in Mountain View's Department of Psychiatry. Dr. Shen has enjoyed supervising 7

postdoctoral residents for the past 6 years, and served as Site Director for Santa Clara's Adult Team from 2011 to Dr. Shen's clinical interests include working with patients with mood and anxiety disorders. Her research on bipolar spectrum disorders has been published in the Journal of Clinical Psychology and Bipolar Disorders. Dr. Shen also specializes in working with prenatal and postpartum women adjusting to the challenges of parenting. She currently facilitates a Dialectical Behavioral Therapy group as well as a Bipolar Support group. al=aboutme.xml&ctab=about Me&cstab=Professional Kristin Schleifer, PhD Kristin.Schleifer@kp.org I have had a long-standing interest and respect for the culture of children and adolescents, and the ways in which youth navigate challenges in life. I am very interested in family dynamics and working with young people to both provide support and help to develop tools that empower positive change, and movement towards health. I value being a part of an interdisciplinary team, that works together to address the needs of the families we serve. I completed my postdoctoral training at Kaiser Santa Clara Child and Adolescent Psychiatry in 2011, and am now a staff psychologist. As a graduate of the South Bay Consortium, I know the post-doctoral training program from both sides: as a resident and now as a supervisor. Our program provides a supportive environment for post-docs to get experience working with a full range of challenges, transitions, mental health issues and other stressors that affect children, teens and families today. This program would be a good fit for a person who loves to learn and is looking for a challenging, fast-paced environment that requires flexibility, independent decision-making, and sound clinical judgment, with the support and supervision of licensed providers and a collaborative clinical team. I believe this last year of training is very important for a resident to sharpen his/her skills and gain exposure in new areas. I enjoy supervising residents because of the energy and curiosity they bring to our team. I also enjoy helping new psychologists develop their professional identity and prepare for licensure. Prior to Kaiser Permanente, I worked in an elementary school, a general therapy practice, community mental health, a special day classroom, a middle school and an inpatient psychiatric unit. My graduate work and clinical experience has focused extensively on child, adolescent and family therapy. I have a special interest in integrating mental health and physical health. I enjoy working with pediatricians and other providers, and act as a liaison between the Department of Pediatrics and the Department of Psychiatry. One of my responsibilities includes going on rounds in the hospital with the pediatric medial team in our General Pediatric Unit and Pediatric Intensive Care Unit where I assess for mental health issues and evaluate patients for psychiatric hospitalization/5150. I also work with the Department of Outpatient Pediatrics to help connect the services we provide in Psychiatry to a larger group of families. I developed a Consultation Liaison program in Pediatrics to provide brief evaluations, make recommendations and provide 8

support to young people and their families who may not need ongoing care in Psychiatry. In this role I can determine if more intensive services are indicated, and can provide education to families about the benefits of groups, parent classes or individual and family therapy. I am also part of our IOP (Intensive Outpatient Program) for teens that have recently been psychiatrically hospitalized, and their families. This is an intensive, twice-weekly multifamily program that promotes communication and coping skills following discharge from the hospital. I believe that a time of crisis can often lead to major positive shifts and changes when families are open to looking at old challenges in new ways. I value integrating evidence-based practices in my work, including utilizing cognitive behavioral therapy and mindfulness techniques to help youth better understand the mind-body connection. I also incorporate psychodynamic, narrative and art therapy techniques in my practice. I believe that our childhood experiences can have a lasting effect on how we understand relationships and interact within the world. I received my BA in English from Saint Mary s College of California, Moraga. I received my MA in Children s Literature from Simmons College in Boston, and my MS in Clinical Psychology from the Pacific Graduate School of Psychology in Palo Alto, where I also earned my PhD in I completed my pre-doctoral internship at Miami Children s Hospital, and my post-doctoral residency training at Kaiser Permanente in essional=aboutme.xml&ctab=about Me&cstab=Professional David Severin, PhD david.a.severin@kp.org I received a BA in Psychology from Georgetown University and then earned a Master s degree in Forensic Psychology from CUNY-John Jay College of Criminal Justice. Following training at Bellevue Hospital in Manhattan, I went on to earn a PhD with an emphasis in Neuropsychology from Palo Alto University. I completed my internship in Rehabilitation and Neuropsychology from Mount Sinai Medical Center in NYC and then completed a two-year fellowship in Neuropsychology at UC Davis Medical Center. I have now worked at Kaiser Permanente San Jose for over 10 years and specialize in performing comprehensive neuropsychological evaluations for adults. The patients with whom I work have a variety of neurological disorders, including traumatic brain injury, stroke, dementia, Parkinsonism, Multiple sclerosis and any other disorder that has the potential to adversely affect cognitive health and overall daily functioning. In addition to my work in Adult Psychiatry, I also work within the KP Memory Clinic, where I assess and provide education to patients and their families. Within the postdoctoral program here at San Jose Kaiser Permanente, I provide supervision for the neuropsychology rotation. This rotation is not intended to train individuals for a practice in neuropsychology but rather to provide the postdoctoral fellow with education of the cognitive, behavioral and emotional sequelae of various brain injuries and the role neuropsychology plays 9

in evaluating and treating these patients. The overall goal is to enable postdoctoral residents to gain additional knowledge in neurological disease and integrate this knowledge into their psychological practice. sional=aboutme.xml&ctab=about Me&cstab=Professional Ashley Stokke, PsyD I earned my Bachelor s Degree at the University of Minnesota, with a degree in psychology, and obtained my Doctorate Degree in clinical psychology, at John F. Kennedy University, in Pleasant Hill, CA. I was fortunate to receive my pre-doctoral internship training at an inpatient psychiatric hospital in Sacramento. There, I trained and worked in a multi-disciplinary team and work with patients from child/adolescents, adults, and intensive care units. I conducted individual and group therapies, provide case management, and psychological assessments. I moved on to do my post-doctoral training at Kaiser Child & Adolescent Services in San Jose, where I continue to work as a staff psychologist. I am trained and work as a generalist, working with young children, teens, and families from various backgrounds and issues. I find that I particularly enjoying working with families with children/teens who struggle with Autism Spectrum Disorders and depression. I tend to conceptualize cases from a psychodynamic perspective, and utilize more integrative approaches from evidence-based practices in my work with families. I also highly value working collaboratively on our multi-disciplinary team, continuing to consult with them and grow as a clinician. I recently began supervising, which has been exciting as I hope to continue to work jointly with the incoming post-docs, and help foster their training throughout their year at Kaiser. ssional=aboutme.xml&ctab=about%20me&cstab=professional H. B. Wilder, PhD I grew up in Sunnyvale, CA, did my undergraduate work at Oregon State University and obtained a PhD in clinical psychology from the California School of Professional Psychology, in Berkeley, CA in I did clinical training at the Student Counseling Service at the California State University, Hayward and the Community Mental Health Center in Palo Alto, CA. I was one of the authors of the book Divorce: Crisis, Challenge, or Relief? I have also published in the area of co-occurring disorders. I have worked in Chemical Dependency in the Bay Area since I have been a staff psychologist at Pyramid Alternatives in Pacifica, Starting Point in Hayward and Walden House in San Francisco. I was Program Director, then Owner/Director of the Step One Program in Castro 10

I have worked at the Santa Clara CDRP since 1995 and have served as Program Director since nal=aboutme.xml&ctab=about Me&cstab=Professional Swetha Vajapey, PsyD swetha.vajapey@kp.org I received my B.A. in Psychology at Rutgers University and then immediately followed with a M.A. in Clinical Psychology through Argosy University. I decided to take a break from studying and worked in the field for two years before starting my doctorate in Clinical Psychology at George Washington University. I have been working at Kaiser for 6 years and have enjoyed learning from the variety of patients we see as well as my skilled colleagues. I enjoy working with individuals and groups and have an eclectic theoretical orientation including Psychodynamic, DBT and CBT. As a group supervisor, I tend to incorporate a more Socratic approach and invite supervisees to engage in critical thinking. I currently run or co-lead a Bipolar support group, a skills based group called Rapid Symptom Reduction, a Panic group and DBT group. fessional=aboutme.xml&ctab=about%20me&cstab=professional 11
