4 Message from Editorial Board Respected Seniors, Friends and my Colleagues, Warm greetings from our Erode IMA new Team Last month we started Yoga & Anti stress Massage program which will be continued till Dec 2017 for Doctors & Doctors Family as a part of our IMA Family Club Activities was huge success with overwhelming response and well appreciated by all. We had state level CME on Cancer awareness and prevention strategies in which our District Collector inaugurated About Cancer a new web page about cancer for public from our Erode IMA website. We celebrated International noise pollution day by organising rally and had awareness program for student to understand ill effects of noise pollution. Unity will strengthen our Doctors community and IMA is one of such platforms to show our strength and fight for good causes. We EIMA along with National IMA observed National Black Day to opposed West Bengal Clinical Establishment Act. In the mid month of May, EIMA has organised International IMA Family Tour to Hong Kong and Macau. e wish them all happy and memorable journey. In June we have National level Nursing Home Board conference on 10th & 11th June 2017 at Hotel Club Melaange, Erode. I request all members to participate actively and make this event successful. We appreciate Dr. C.N. Raja & his team for their effort to bring renowned speakers for this conference. 8 ATTACK ON DOCTORS- CAN'T WE STOPIT? Dr KM AbulHasan The medical profession is going through a crisiswith regards to Doctor-Patient Relationship. The conflicts which came to light only tip of an iceberg. Many are not reported, solved locally. Let us look at it this way when we started our carrierroughly 30 years ago, this was not happening and also we did not even imagine.so, is it not our duty to straighten our profession and put things in order? even though we are not responsible for the situation. I s e e m a ny o u r c o l l e a g u e s s p e a k i n g pessimisticabout the future. Many of our wards not preferring medicine because of the prevailing scenario. Indian Medical Association has been putting lots of efforts to safeguard doctors. We have obtainedhospital Protection Act 48 of 2008 and similarly in some other states also to punish the culprits. Now we are trying for National Level Medical Professional & Hospital Protect Act and IMA has envisaged many measures to bring down this menace. We need to work for not only safe guarding doctors but also saving the medical Profession. This can be accomplished only if heads of the state and police institutions and IMA jointly form a committee which could handle any such incidents with utmost seriousness and immediate attention. I have suggestedthis in the last state council and accepted by the president and appointment with chief minister could happen any time. That is one aspect. The second one is education of the people regarding emergency care and its outcome. Luckily majority of our people still have trust in doctors. Only handful of people create problems, we can prevail over the situation if we all work with dedicationto restore a comfortable Doctor Patient Relationship. That would be the most valuable gift to our young and aspiring medicos If we don't fix this who will? 8

16 Examination of a Patient with Sciatica Dr.Palanikumar.C Orthopaedic and Endoscopic Spine surgeon, S I M S, Erode , orthodrpalani@gmail.com Sciatica is a term used to refer to a patients pain radiating posteriorly from the gluteal area till the heel or ankle. True sciatica is due to pressure on the lower lumbar nerve roots. The upper lumbar nerve roots involve the Femoral plexus and the nerves and DONOT cause classic sciatic pain. Most often the patient gets better with analgesic, muscle relaxant, rest and / or physiotherapy. The classic symptoms areoften exacerbated along the sciatic nerve distributionby coughing, sneezing, pushing, bending forward and lifting weight. The patient can have associated Sensory deficits, Motor deficits, Impaired knee, ankle jerk reflexes and Bladder, Bowel symptoms. An examination of the above can guide the physician to the severity of the problem. Here we discuss about the examination of the L-4, L-5 and S-1 nerve roots in more detail. After detailed history taking, physical examination starts with 1. Gait examination how does the patient walk? Does he have any tilt or list to one side and is it painful? 2. Inspection of the lumbar spine whether there is paraspinal spasm or any bend scoliosis or step or scar seen. 3. Range of ACTIVE movement examination as given in the picture below 4. Straight leg raising test or Lasegues test done with patient is supine position. It can be Active or Passive. The examiner raises the patients both lower limbs alternatively with the knees straight to note the angle at which the posterior and inferiorly radiating pain is maximum. The angle is recorded on both the affected and the normal sides. 17

17 5. Sensory examination is done for LIGHT TOUCH and PIN PRICK. For light touch a wisp of cotton is used and a sterile needle tip is used for pinprick. The results are recorded as below in a table 0 ABSENT 1- ALTERED increased or decreased 2 Normal The Dermatomes are tested at their AUTONOMOUS areas as depicted in pictures below. 6. Motor examination is done and recorded as per MRC grading. a. Grade 0 No movement b. Grade 1 Flicker of muscle movement c. Grade- 2 Movement possible with Elimination of Gravity d. Grade- 3- Full range of Movement against gravity e. Grade- 4 Slight weakness in comparision to the normal side f. Grade 5 Normal power Pictures that show the individual levels are given below along with acondensed examination protocol. L- 4 RADICULOPATHY Sensation is checked in the MEDIAL malleolous area Tibialis anterior motor power is checked Reduced knee jerk reflex may be present L- 5 RADICULOPATHY Sensation is tested ondorsum of FIRST web space Weakness of dorsiflexion of the great toe No reflex changes are found. S-1RADICULOPATHY Sensation along the Lateral side of foot Weakness on plantar flexion at ankle Absent ankle jerk reflex Based on this comprehensive clinical examination and recording of findings, the physician is well justified in ordering for further radiologic investigations like MRI. This comprehensive examination helps the physician narrow down the various differential diagnoses that could present like sciatic pain. 18

19 Percutaneous laser disc decompression versus conventional microdiscectomy for patients with sciatica: Two-year results of a randomised controlled trial. IntervNeuroradiol Jan 1: doi: / [Epub ahead of print] Abstract INTRESTING ABSTRACT Background Percutaneous laser disc decompression is a minimally invasive treatment, for lumbar disc herniation and might serve as an alternative to surgical management of sciatica. In a randomised trial with two-year follow-up we assessed the clinical effectiveness of percutaneous laser disc decompression compared to conventional surgery. Materials and methods This multicentre randomised prospective trial with a non-inferiority design, was carried out according to an intent-to-treat protocol with full institutional review board approval. One hundred and fifteen eligible surgical candidates, with sciatica from a disc herniation smaller than one-third of the spinal canal, were randomly allocated to percutaneous laser disc decompression ( n = 55) or conventional surgery ( n = 57). The main outcome measures for this trial were the Roland-Morris Disability Questionnaire for sciatica, visual analogue scores for back and leg pain and the patient's report of perceived recovery. Results The primary outcome measures showed no significant difference or clinically relevant difference between the two groups at two-year follow-up. The re-operation rate was 21% in the surgery group, which is relatively high, and with an even higher 52% in the percutaneous laser disc decompression group. Conclusion At two-year follow-up, a strategy of percutaneous laser disc decompression, followed by surgery if needed, resulted in non-inferior outcomes compared to a strategy of microdiscectomy. Although the rate of reoperation in the percutaneous laser disc decompression group was higher than expected, surgery could be avoided in 48% of those patients that were originally candidates for surgery. Percutaneous laser disc decompression, as a non-surgical method, could have a place in the treatment arsenal of sciatica caused by contained herniated discs. Dr. P. Jayanth Kumar 20
