4 Presenters Mary Ann Barnard Mary Ann has over 10 years of experience in data collection and research project management, including qualitative, quantitative, phone and field data collection for a variety of social science and public health projects. Mary Ann has worked at the Fred Hutchinson Cancer Research Center and at the University of Washington for the Social Development Research Group and the School of Public Health. For the past 2 years she has been the study coordinator for the Pacific Northwest regional study center for the National Children s Study, including serving as lead coordinator for two national Improvement Work Groups one on participant retention and one on information management systems., PhD Dr. Crabb received her Ph.D. in Clinical Psychology in 2008 from the University of Ottawa, Canada, and has completed fellowships at the University of California, San Francisco and the Veterans Health Administration, Palo Alto. Her current roles include: licensed psychologist in private clinical practice, Staff Psychologist with Vericare Behavioral Healthcare, and Adjunct Faculty Member at Argosy University, San Francisco. She has advanced training and experience in the treatment of depression, anxiety, adjustment and transition issues, and stress related to medical illness, disability, and caregiving responsibilities. Dr. Crabb obtained her PST certification in 2008 and has applied PST with a wide range of patient populations, including research participants, residents of long-term care facilities, and private practice patients., MSN, PMHCNS-BC, CN Rita is a psychiatric clinical nurse specialist who has been licensed as a therapist in the state of California since She has her masters degree in psychiatric nursing from the University of Cincinnati. She has worked for Kaiser Permanente for the last 24 years, 11 years as an outpatient therapist in the psychiatric department and the last 13 years in the primary care department doing integrated behavioral health. She was a depression specialist in the original IMPACT depression care research. She currently is a consultant and a senior trainer with the AIMS Center. She has provided implementation and clinical support to numerous FQHC s across the country in this role. She is a frequent speaker presenting on the IMPACT model, problem solving treatment and cognitive therapy. She supervises and certifies providers in PST-PC. She has three publications in peer-reviewed journals regarding integrated care. Susanne James Susanne James is The John A. Hartford Foundation s primary liaison and technical assistance expert to Social Innovation Fund (SIF) sub-grantees. Her role is to ensure compliance with SIF grant requirements, and provide guidance on fiscal, organizational capacity, and federal regulatory matters. Prior to joining The John A. Hartford Foundation, Ms. James was the SIF Fiscal Coordinator at the Mayor s Fund to Advance NYC, where she was responsible for compliance oversight and fiscal management of the
5 Mayor s Fund/ NYC Center for Economic Opportunity $85 million SIF Grant. She provided technical assistance to strengthen sub-grantee organizations and reported the fiscal aspects of the SIF Award to the Corporation for the National and Community Service, and other SIF partners such as foundations and corporate investors. Ms. James has also served as the Chief Operating Officer for both imentor and CFY. She is a graduate of Dartmouth College. Wally Patawaran, MPH Wally Patawaran, MPH, is a Program Officer at the Hartford Foundation. Prior to his arrival in 2011, Mr. Patawaran advised health and aging services organizations on issues of strategy, operations, and marketing. Previously, he led performance improvement initiatives for Weight Watchers International, a health and wellness education company. He began his career as an investment research analyst in New York. Mr. Patawaran has degrees from the London School of Economics and Columbia University s Mailman School of Public Health with a specialization in health care services management, and a certificate in Health Informatics from Columbia s Department of Biomedical Sciences. He is active on non-profit boards and committees., MA has twenty five years experience managing a wide range of public health and health services projects and programs. Since 2004, she has managed the AIMS Center at the University of Washington and is currently Associate Director for Translation and Implementation. Prior to that, Diane worked at the Fred Hutchinson Cancer Research Center where she managed a research program focused on behavioral interventions for cancer prevention. Diane also provided mental health services in both inpatient and outpatient settings for over eight years. She received a bachelor's degree in Psychology and English Literature from Gonzaga University and a master s degree in Psychology from Seattle University., MD, MPH Dr. is an Assistant Professor in the Department of Psychiatry & Behavioral Sciences at the University of Washington. Dr. Ratzliff received her MD/PhD in 2005 from the University of California, Irvine. She completed her doctoral work in the Department of Anatomy and Neurobiology where her research focused on mechanisms of epilepsy development following head injury. She completed her residency training in psychiatry and served as the Chief Resident at the University of Washington Medical Center. Dr. Ratzliff currently serves as the Associate Director for Education for the Division of Integrated Care and Public health, works as a consulting psychiatrist for the Mental Health Integration Program (MHIP) and trains psychiatric residents at the University of Washington. Her clinical expertise includes primary care consultation, women s mental health and providing mental health care to underserved populations. Dr Ratzliff s academic pursuits include developing strategies to provide mental health education to members of integrated care teams, and studying the impact of integrated mental health care programs in special populations.
6 Jürgen Unützer, MD, MA, MPH Dr. Unützer is an internationally recognized psychiatrist and health services researcher. His work focuses on innovative models that integrate mental health and general medical services and on translating research on evidence-based behavioral health interventions into effective clinical and public health practice. He has over 200 scientific publications and is the recipient of numerous federal and foundation grants and awards for his research to improve the health and mental health of populations through patient-centered integrated mental health services. Dr. Unützer is Professor and Vice-Chair in the Department of Psychiatry and Behavioral Sciences at the University of Washington where he directs the Division of Integrated Care and Public Health. He also holds adjunct appointments as Professor in the School of Public Health (Departments of Health Services and Global Health) and as Affiliate Investigator at the Group Health Research Institute in Seattle, WA. Dr. Unützer directs the AIMS Center ( dedicated to Advancing Integrated Mental Health Solutions and the IMPACT Program ( which has supported national and international testing and implementation of an evidence based program for depression care. Dr. Unützer works with national and international organizations dedicated to improving behavioral health care for diverse populations and has served as Senior Scientific Advisor to the World Health Organization and as an advisor to the President s New Freedom Commission on Mental Health., PhD completed her PhD in Psychology from the University of Washington in 2007 working on a multi-site, randomized clinical trial of two behavioral treatments for relationship distress. Dr. Yi did a post-doctoral fellowship with Dr. Karen Syrjala at Fred Hutchinson Cancer Research Center and continues working with Dr. Syrjala. Dr. Yi has used problem-solving treatment with cancer survivors over the phone under the supervision of Dr. Mark Hegel. Her research interests relate to cancer survivorship issues, in particular survivors of hematopoietic cell transplantation (HCT), caregivers of HCT patients, and the use of technology for behavior change with an emphasis on addressing cancerrelated distress, depression and lifestyle changes.
