3 Main Information Project Name Increasing awareness of osteoporosis screening among physicians and patients with DEXA A summary of the project and its reasons (15 lines maximum) Osteoporosis is the most common bone disease and affects both men and women. Clinical and public health implications of this disease in the UAE are considerable because of the mortality, morbidity, and cost of medical care associated with osteoporotic fractures. low- Diagnosis is made on the basis of a impact or fragility fracture or low bone mineral density assessed by central dual-energy x-ray absorptiometry. Increased awareness about osteoporosis and importance of early detection among the local and resident population. Identify and set goals and project document To achieve and maintain an increase in osteoporosis screening with DEXA scans in women>50 and men> 60yrs. To identify younger patients who are at increased risk of low bone density who may benefit from early detection. Increase and prolonged health promotion strategy to increase osteoporosis awareness pamphlets and physician advice. Transformational Project pg. 3
5 2. Project Plan and Execution The project plan has been executed in the Al Rahba Hospital to begin with the actual awareness campaign. The scope of the project includes: - increasing awareness among the hospital physicians - Followed by increase awareness among criteria patients by holding osteoporosis month activities at the Al Rahba Hospital where patients would be encouraged to come for osteoporosis work-up. The goal behind this is to increase the number of people who visit the hospital for Dexa by 30%. This would be by physician advice and text messages Transformational Project pg. 4
6 - Improve patient compliance by providing same day appointments and same day results for at least 90% of the referred patients This result would be achievable and would be a motivation for the project to be executed at a much greater level 3. Develop an organized and comprehensive plan for internal and external communication The physicians would also need to be communicated about the implementation of such a project. The internal communication within the team would be done through s as well as personal meetings. The communication with the physicians would be done through CME as well as multi-disciplinary communication channels. 4. Develop a quality assurance plan Maintaining quality of the project deliverables would be an important task as the project progresses. For this, regular examinations of the deliverables would be done and adherence to the necessary standards would be ensured. The other major quality assurance standard would be to manage the quality through checklists and a fixed evaluation criterion that would be established in the planning stages itself. 8 Third Criteria: Resource Management and Control 1. Manage financial resources related to the project The financial resources that would be used for this particular project would be minimal. While there would be no major investment for the project, there would be minor costs that would need to be incurred by the hospital administration. The project would not require investment as the equipment as well as the expertise required is already present. The expenses that would be incurred would be maintenance expenses as well as the marketing and promotional expenses for awareness campaign. 2. Manage and direct knowledge resources and identify a systematic and comprehensive plan The resources that would be contributing to the project would include doctors, radiologists and radiographers who have the required expertise to carry out Osteoporosis program. The project member who would be assisting with the implementation of the project would schedule the necessary appointments and make sure that the doctors are divided evenly among the patients who would be undergoing DEXA tests during the campaign and project duration. 3. Manage and direct suppliers There would be no significant suppliers for the project with the exception of the vendors and retailers who would be assisting with the creation of promotional materials for awareness. 9 4. Assure optimum Resources use The patients who would be willing to go for the tests would be given specific time slots at which they would need to report to the doctor s office. These time slots would be prepared with consideration to the schedule of the doctors and the hospital facilities. This would ensure that the resources are not over allocated. 10 Fourth Criteria: Risk Management and Project sustainability Following are the risks that can possibly impact the project and its success in a major way: Risks Impact Probability Overhead Costs Medium Low Underestimation of Medium High Resources Scope Creep Low Low Fifth Criteria: Project Execution 1. Develop a system for Managing Records and project activities and assure Document support and operation execution, Control, review, update and develop solutions and preventive and corrective action Documentation would be a primary requirement in order to make sure that the activities that are carried out for the project are managed effectively. In this regard, one of the main aspects that could be included in this regard would be the management of the resource and ensuring that the overall performance of the project is carried out effectively. For this, it would be advisable that the project team meets regularly and plans out the major deliverables and timelines related to the project. This would also be documented. At the end of the awareness campaign there would be a final project documentation which would include all the major processes carried out and the results that would be derived. Develop a comprehensive plan or methodology to close the project including all aspects of the project The project closing would be decided based on the extent to which the project is able to meet the criteria that has been set for the project success. The project would not be completely shut down once the results are achieved. Instead of that, there would be a signing off meeting where further actions would be decided. If the project is successful, the project can continue on a passive level where patients are constantly made aware about osteoporosis and are recommended to take the tests. Furthermore, the final project documentation would also be discussed and presented to the hospital administration. Transformational Project pg. 10
12 Sixth Criteria: Results Project KPIs and results a. The number of patients coming for DEXA in Radiology department increases by 30% in 3 months b. The financial aid from the hospital as well as external sources to concerned department increases by 6-8% within a month Team KPIs and results a. The team is able to cater to at least 90% of DEXA requests on a daily basis Project Effect Indicators a. There would be at least 30% spike on the number of patients undergoing DEXA scans for osteoporosis assessment. b. Similar awareness program can be replicated in other hospitals based on the level of success attained Transformational Project pg. 11
