This workshop will provide education, skill development and the practical application of concepts that contribute to decreasing risk, improving quality and increasing safety in any care and service delivery environment where persons with dementia and related illnesses are served across the aging services continuum. Application has been made to obtain approval for nursing and NCERS (NHA) continuing education credits for both days of education. Spring Education Series Objectives Describe some of the current challenges of risk assessment and risk management in dementia care Share evidence based practices that promote the well-being of persons with dementia living in long term care communities as well as a positive work experience for their caregivers Build competency for aging service professionals to address the needs related to dementia care Develop and include dementia care specific elements in scope of service documents for any service line that serves persons with dementia or related illnesses Illustrate the characteristics of dementia-friendly communities in supporting resident safety and well-being Identify clinical care interventions that reduce the risk of falls for residents with dementia Discuss new CMS requirements for protecting Resident Rights

2 7:00 AM 8:00 AM Registration and Breakfast Day One 8:00 AM 8:15 AM Welcome and Introductions 8:15 AM 9:15 AM Session 1: Designing for Dementia Care in Scope of Service Session 1: This session will discuss scope of service and the important role it plays in effective risk management, quality improvement and safety efforts as it relates to day-to-day decision making and related organizational processes (e.g., matching resident needs to scope of service, care plan modification, discharges, transfers, staffing ratios, staff training). This will include the effective use of high risk assessments to more clearly describe the scope of services offered and to be used as decision making tools. Finally, using concepts of a therapeutic milieu for dementia care, the session will include methods to incorporate habitation approach domains into the scopes of services of various service lines throughout the aging services continuum for persons served who have dementia or a related disorder. Session 1A: Using a small-group exercise with worksheet(s) format, this session will provide an opportunity for the practical application of Session 1 concepts to map out an aging services provider s care and service elements by service line. Special emphasis will be placed on dementia care related programming in various industry service lines and levels of care. Examples of integrated scope of service and associated policies and procedures to be provided with session materials. Session 1 and 1A Learning Objectives Identify the role of scope of service in healthcare and aging services risk management Discuss the relationship between scope of service and other mission critical processes such as assessment of the person served, determining appropriateness of admission, and discharge or transfer Identify key elements in an effective scope of service document Recognize five (5) elements essential to a therapeutic milieu Discuss the domains of the habilitation approach that contribute to an effective therapeutic milieu in dementia care Create a care and service map to assist in writing scope of service documents 9:15 AM 9:45 AM Session 1A: Designing for Dementia Care in Scope of Service Now It s Your Turn (Participant Exercises) 9:45 AM 10:00 AM Break 10:00 AM 11:00 AM Session 2: Setting Realistic Expectations with Families Page 2

3 s Session 2: This session starts with a brief overview of Alzheimer s disease and related dementias. The session will then explore ways in which organizations can clearly communicate realistic expectations to families by sending consistent messaging throughout the continuum: during admission, transitions in level of care and end-of-life. Session 2A: Participants will work in groups and study case scenarios in which families communicate expectations beyond the providers scope of service, dissatisfaction with services, and difficulty recognizing physical and cognitive decline in their loved one. Participant teams will script positive communication strategies and action plans in response to family requests, concerns and grievances. Volunteer teams will share their strategies through role-play. Session 2 and 2A Learning Objectives Describe behavioral and cognitive changes characteristic of dementias Explain the importance of setting realistic expectations for residents with dementia and their families List common therapeutic communication techniques Select statements that promote consistent messaging Recognize frequent misconceptions held by families trying to cope with dementia illnesses Construct methods that reduce misunderstanding and promote collaboration between providers and families 11:00 AM 11:45 AM Session 2A: Setting Realistic Expectations with Families Now It s Your Turn (Participant Exercises) 11:45 AM 12:45 PM LUNCH Prescriptives Platform Feedback Forum* Mimi McCahill, *Participation at this roundtable forum is optional, however attendance is strongly encouraged 12:45 PM 1:45 PM Session 3: Physical Domain Using Physical Design to Manage Risk, Improve the Quality of Life and Increase Safety in Dementia Care s Session 3: This session discusses the important role that the physical domain plays in supporting or detracting from a therapeutic milieu, which in turn can lead to environmental factors that help to inhibit or proliferate incidents and adverse event-related risks specific to persons served who have dementia or related illnesses. We will identify key areas of focus that support milieu elements such as cuing, safe and intentional wandering, visual supervision and important interior and exterior hazard and safety issues. Additionally, this session will include content about the importance of regular environmental safety rounding and hazard surveillance in any service line where persons served have dementia or related illnesses. 4 Session Learning Objectives Identify vital physical factors that influence risks associated with care delivery, staff effectiveness and safety in relation to dementia care Extend design elements in the physical domain to service lines beyond special care dementia units Utilize effective environmental rounding and hazard surveillance practices specific to dementia care that increase safety and support quality of life Identify hazards related to the physical domain as they specifically relate to areas of high risk when serving persons with dementia and related disorders 1:45 PM 2:30 PM Session 4: Falls Management for Residents with Dementia Session 4: Falls and fall-related injury continue to be a health challenge for all Member organizations. Individuals with dementia fall twice as often as cognitively intact people and are more likely to have injurious falls. This session will discuss the current research data and look at targeted interventions which can help to reduce the incidence and severity of falls. Session 4A: Using case-studies, participants will work in teams to evaluate available assessment tools and look at specific interventions that address root causes of falls commonly seen in residents with dementia. Volunteer teams will share a summary of their findings. Session 4 and 4A Learning Objectives Identify the risk factors for falls in people with dementia Theorize effective system-level interventions to reduce the incidence of falls Recognize the essential elements of fall prevention for residents with dementia Compare assessment tools aimed at identifying residents with dementia who are at risk for falling 2:30PM 3:30 PM Session 4A: Participant Exercises 3:30 PM 4:00 PM Summary and Wrap-Up Page 4

5 DAY TWO 7:00 AM 8:00 AM Registration and Breakfast 8:00 AM 8:15 AM Welcome and Review 8:15 AM 10:15 AM Session 5: Supporting Dignity and Preserving Resident Rights: Abuse Prevention Mary Rodger Session 5: This session begins with an overview of the new CMS Nursing Home Requirements of Participation related to abuse prevention, including mandated dementia management and abuse prevention training. Suggestions on how to measure the effectiveness of dementia care and abuse prevention training will be examined. Next, the session will discuss system-level approaches to reducing resident abuse, including specific suggestions on how risk managers can partner with human resources managers to implement proactive programs that support care-givers. Session 5A: The session will end with interactive exercises using sample state survey deficiencies on resident rights and background checks. Participant will review and appraise mock plans of correction. Session 5 and 5A Learning Objectives Describe the key elements of effective dementia management and abuse prevention training Recognize system-level approaches that support quality of life for residents and a quality work experience for direct care staff Analyze mock resident rights survey deficiencies Critically review mock survey plans of correction 10:15AM- 10:30 AM Break 10:30AM 12:00 PM Session 6: Recognizing and Communicating the Changing Needs of Residents with Dementia as They Decline Mary Rodger Session 6: Using a combination of lecture and adult learning techniques, this session discusses the importance of systems that recognize, communicate and respond to the changing needs of residents with dementia experiencing decline. Maintaining accurate, up-to-date documentation and record-keeping is addressed. Effective communication of the resident s need for higher level of care is reviewed. Session 6A: Participants will be provided thought provoking ethical questions regarding topics such as end of life decisions and transfers within the continuum Page 5
