The Chronic Care Initiative-Medication Reconciliation Plus Initiative (Project Director) --This is an expansion of the Pharmacy Home Project that gathers information from case managers in hospitals, case mangers doing home visits and clinical pharmacists embedded in primary care practices to compare drug use information versus fill history to create a summary document for clinicians as a Medical Home activity.

The North Carolina E-prescribing Initiative (Project Director) This is a multistakeholder project to increase e-prescribing adoption in hospitals and clinics in North Carolina this utilized eight in-the-field e-prescribing facilitators to provide promotion, education and support for e-prescribing adoption and persistence. 3. The Pharmacy Home Project (Project Director) - This project aimed to create a Pharmacy Home, virtual or otherwise, where drug use information from multiple sources is gathered to better inform prescribing and intervention strategies. Thirty-five pharmacists and over 600 case managers under the direction of Community Care of North Carolina (CCNC) are now engaged in the project across the state of North Carolina. (October 2006-Current) 4. The Instant Approval Evaluation and Improvement Project (Project Director) - Beginning June 1, of 2007 North Carolina Medicaid implemented a unique prior authorization program. This program allowed the prescriber to hand write on the prescription that criteria which exempted a patient from the traditional prior authorization process and allowed the pharmacist to override the prior authorization at the point of sale. The primary goal of this novel program was to reduce the length of gaps in therapy. (June June 2008) 5. A+KIDS (Collaborator)- A collaboration between the North Carolina Division of Health and Human Services and Community Care of North Carolina and Infina technology group to create a documentation and care coordination tool that is plugged into the point of sale for pharmacy overrides when proper documentation is entered into an online patient registry. Software Development Projects: 1. InterventR x (Project Consultant) - InterventR x was built for use in the North Carolina Nursing Home Polypharmacy Initiative. The software provided pharmacy claims information to consultant pharmacists to guide value-added drug regimen reviews. Pharmacists were able to submit pending and completed interventions and bill for the service using the software. (April December 2003) 2. The Pharmacy Home (Electronic Pharmacy Record) (Co-Designer) - The Pharmacy Home Project Software is being developed to create a customizable electronic pharmacy record that combines drug use information from multiple sources (medical chart, administrative claims, retail pharmacist and patient interview). It has been developed as an online-offline application to enable consultant pharmacists to work with patients regardless of setting (clinic, patient home, or in-transit). The application and the underlying databases support CCNC s Pharmacy, Care Management and Provider Portals. (October 2006-Current)
