1 Biographies Angelo Antonini Angelo Antonini joined the Parkinson Institute in Milan, Italy, in 1997 and is now the medical coordinator for Information Technology and Clinical Research at the Department of Neuroscience at the Hospital Istituti Clinici di Perfezionamento in Milan, Italy. He is also aggregate Professor of Neurology at the University of Milan- Bicocca. After qualifying in medicine and training in neurology from the Università degli Studi di Roma La Sapienza, Rome, Italy. In November 1990 he completed his neurology training with honors and then he undertook a visiting fellowship at the PET Department of the Paul Scherrer Institute, Villigen, Switzerland before starting his PhD in neuroradiology under the supervision of Professor Klaus Leenders. In 1995, he received the first award from the National Parkinson Foundation for young researchers in Parkinson s disease. In 1996, he was awarded the Junior Faculty Award 1996/97 from the United Parkinson Foundation and Parkinson's Disease Foundation for his research in the field of Parkinson s disease. His research interests include the use of continuous subcutaneous infusion of apomorphine and subthalamic nucleus deep brain stimulus (STN-DBS) in the treatment of serious motor fluctuations and dyskinesia in patients suffering from advanced Parkinson s disease. During his academic career, he has published over 150 peer-reviewed articles, over 200 abstracts and several book chapters. He serves as reviewer for the main neurology journals and is on the editorial board of Movement Disorders. David Brooks David Brooks is Hartnett Professor of Neurology and Head of the Department of Clinical Neuroscience in the Division of Neuroscience, Faculty of Medicine, Imperial College, London, UK. He is also Head of the Neurology Group at the Medical Research Council Clinical Sciences Centre, Hammersmith Hospital, London. He is a member of the Research Advisory Panel of the UK Parkinson s Disease Society, and was a member of the UK Medical Research Council Neuroscience and Mental Health Board, and the UK Huntington s Disease Association. He also advises the German Dementia Network and Inserm. He currently serves on the editorial boards of several journals including Brain, Journal of Neural Transmission, Synapse, Neurotherapeutics and Current Trends in Neurology. In 2001, he was elected a Fellow of the Academy of Medical Sciences in the UK. Professor Brooks has lectured widely including the Stan Fahn lecture at the International Congress of Movement Disorders in 2003, and the Kuhl Lassen lecture at the Society of Nuclear Medicine, Toronto, in His research involves the use of positron emission tomography and magnetic resonance imaging to diagnose and study the progression of Alzheimer s and Parkinson s diseases, and their validation in biomarker therapeutic trials.to date, he has published over 300 reports in peer-reviewed journals, including Nature. His research is currently supported by grants from the UK Medical Research Council, the Alzheimer s Research Trust, UK Parkinson s Disease Society and industry.

2 K. Ray Chaudhuri K. Ray Chaudhuri is Consultant Neurologist and Professor of Neurology/Movement Disorders at King s College Hospital NHS Foundation Trust, University Hospital Lewisham, King s College London and the Institute of Psychiatry, London, UK. He is a recognised teacher and active researcher within the Guy s, King s and St Thomas School of Medicine, London, and is the Medical Director of the National Parkinson Foundation International Centre of Excellence at King s College, London, UK. He also serves as Chairman of RLS:UK and the international Parkinson s Disease Non-Motor Group. He is a member of the Movement Disorders Society appointments committee, and a member of the American Academy of Neurology task force on practice parameters for Parkinson s disease and restless legs syndrome. He is also an advisor for the Health Technology Assessment Committee at the Department of Health, UK, and the lead clinician for the 18-week pathway for management of Parkinson s disease initiative. He has published 168 papers including reviews, book chapters and is co-editor of four books on Parkinson s disease and restless legs syndrome (two in press). He has contributed extensively to public education through radio and television interviews, newspaper articles and videos. He has also lectured on Parkinson s disease and restless legs syndrome at international meetings in Japan, continental Europe, India and Australia. His major research interests are the drug treatment of Parkinson s disease and restless legs syndrome, Parkinsonism in minority ethnic groups both in the UK and abroad, and sleep problems in Parkinson s disease. In 2005, he was awarded the DSc degree by the University of London and received his personal Chair in neurology in Anthony Schapira Anthony Schapira is Head of the Department of Clinical Neurosciences, Institute of Neurology, University College, London and Professor of Neurology National Hospital for Neurology and Neurosurgery, Queen Square and the Royal Free Hospital, London, UK. Professor Schapira s research interests include the molecular and clinical aspects of neurodegenerative diseases with special emphasis on Parkinson s disease and other movement disorders. He has authored over 500 publications, is Co-Editor-in- Chief of the European Journal of Neurology and is on the editorial board of several journals of neurology and neuroscience. Professor Schapira is the recipient of the Harveian Medal, the Royal College of Physicians Clinical Science Prize, the 1998 European Prize for Clinical Science and the 1999 Opprecht Foundation Award. He was awarded the Duchenne Prize in In 1999, he was elected a Fellow of the Academy of Medical Sciences in the UK.

3 Fabio Baronti Fabio Baronti was born in Rome, Italy, where he attended Medical School and obtained his board certification in neurology. His post-doctoral training included a 3- year fellowship in the Experimental Therapeutics Branch at the National Institute of Neurological Disorders and Stroke in the USA and a PhD in clinical neuropharmacology. Since the beginning of his career, his interests have focused on the treatment of Parkinson s disease, with special attention to the control and prevention of motor fluctuations, and nonpharmacological treatment approaches. Since 1993, he has worked in Switzerland, where he founded a centre specialising in the treatment of advanced Parkinson s disease. He currently leads a Swiss neurorehabilitation hospital which hosts two special units dedicated to the treatment of Parkinson s disease and of epilepsy. He is a member of the Board of Directors of the Swiss Neurorehabilitation Society and of the Steering Committee of the Swiss Parkinson Association. Richard Brown Richard Brown is Professor of Neuropsychology and Clinical Neuroscience, King s College London, and Honorary Consultant Clinical Psychologist with the South London and Maudsley NHS Foundation Trust, London, UK. He is a Principal Investigator at the Medical Research Council Centre for Neurodegeneration Research at the Institute of Psychiatry at King s College London, and faculty member of the National Institute for Health Research Specialist Biomedical Research Centre for Mental Health. He has worked extensively in the field of clinical neuroscience for over 25 years and his primary interest is in the field of movement disorders and particularly Parkinson s disease where his research has covered laboratory-based and clinical research. His main research focus at the moment is mood disturbance. He recently served on two Movement Disorder Society Task Forces: one on dementia in Parkinson s disease and one on the use of rating scales in the assessment of depression in Parkinson s disease. He is also a member of the International Parkinson s Disease Non-Motor Group. Alberto Cifelli Alberto Cifelli is Consultant Neurologist at King s College Hospital, London, and Darent Valley Hospital, Dartford, Kent, UK. He was born and educated in Genoa, Italy, where he graduated in Medicine. He was awarded a Diploma of Specialist Training in Nuclear Medicine by the University of Genoa in 1995 following successful discussion of a thesis on cerebral blood flow and EEG changes in Alzheimer s disease. He became a member of the Royal College of Physicians (Lond) in 1999 and obtained a doctorate in clinical neurology from the University of Oxford in His research in Oxford focused on the evaluation of neuronal damage in multiple sclerosis using magnetic resonance and pathological techniques. He subsequently trained in neurology in Nottingham, UK, where he developed an interest in movement disorders. Currently, he runs a weekly movement disorders clinic in Dartford and collaborates with the Movement Disorders group at King s College Hospital.

4 Jaime Kulisevsky Jaime Kulisevsky is Director of the Research Institute of Sant Pau Hospital at the Autonomous University of Barcelona, Spain. He is also Director of the Movement Disorders Unit at the Neurology Department of Sant Pau Hospital, Coordinator of the Postgraduate Master Degree in Neuropsychological Diagnosis and Rehabilitation at the Autonomous University of Barcelona, and principal investigator for Parkinson s Disease Area of the Spanish Biomedical Network Research Centre for Neurodegenerative Diseases (CIBERNED-Instituto de Salud Carlos III). He is an active member of the Movement Disorder Society and represents them as a member of the International Task Force for Developing Rating Scales for Parkinson s disease, and is a member of the European section of the Executive Committee of the Movement Disorder Society. He acts as a reviewer for various scientific journals and is co-editor for movement disorders for the Spanish journal Revista de Neurología. Among his many publications, Dr Kulisevsky has published two sets of clinical guidelines on the treatment of Parkinson s disease. His particular research interests have been the cognitive and behavioural consequences of basal ganglia dysfunction, and the impact of antiparkinsonian treatment on cognition and behaviour in Parkinson s disease. He recently introduced a new cognitive scale specific for evaluating cognition in Parkinson s disease the Parkinson s Disease-Cognitive Rating Scale (PD-CRS). Gurutz Linazasoro Gurutz Linazasoro is Director of the Centre of Parkinson Research, Policlinica Gipuzkoa, San Sebastian, Spain. He is also Chief Executive Officer of Fundación Inbiomed, a private, non-profit research foundation dedicated to stem cell research and regenerative medicine, as well as Director of the Centro de Investigación y Terapias Avanzadas for Alzheimer s disease. Professor Linazasoro is a member of the medical advisory committee of the Spanish Parkinson s Federation and the Spanish Association of Dystonia. He has been awarded the Parkinson España prize on three occasions. He serves on the editorial board of Neurología and Revista de Neurología, and is the author of numerous papers in both Spanish and international journals, including Brain, Neurology and Movement Disorders. He has co-edited several textbooks, CD-ROMs and educational programmes about Parkinson s disease and movement disorders.

5 Pablo Martinez-Martin Pablo Martinez-Martin is a Neurologist and the Scientific Director at the Alzheimer s Disease Research Unit, CIEN Foundation Reina Sofia Foundation. He is also a member of the Centre of Excellence for Biomedical Research on Neurodegenerative Diseases (CIBERNED) and Professional Researcher of the Spanish Public Boards of Research in the Area of Applied Epidemiology, National Centre for Epidemiology, Carlos III Institute of Health (Ministry of Science and Innovation), Madrid, Spain. His interest in research is focused on movement disorders, especially Parkinson s disease, assessment instruments, outcomes research, dementia, prion diseases and the burden placed on caregivers. He is a member (Steering Committee, Expert Consultant, or Principal Investigator) of several Movement Disorder Society Task Forces for the revision of rating scales in Parkinson s disease (Unified Parkinson s Disease Rating Scale; anxiety, apathy and anhedonia; fatigue; dyskinesia; fluctuations) and international collaborative studies (Scale for Outcomes of Parkinson s disease PROfiling PARKinson s disease [SCOPA-Propark], Non-Motor Symptoms Group, Estudio longitudinal de pacientes con enfermedad de Parkinson [ELEP/EILEP]). He is also a member of the editorial board of Neurología and Revista de Neurologia, which are the official journals of the Spanish Neurological Society and the Spanish Autonomic Neurological Society, respectively. He was a member of the editorial board of Movement Disorders between 2002 and Michael Samuel Michael Samuel is a Consultant Neurologist with a specialist interest in movement disorders at East Kent Hospitals NHS Trust and King s College Hospital, London, UK. He completed his neurological and research training in London before taking a specialist fellowship in movement disorders in Toronto, Canada. He is Lead Clinician for Movement Disorders in East Kent Hospitals NHS Trust and Joint-Lead Clinician for Deep Brain Stimulation (Functional Neurosurgery for Movement Disorders) at King s College Hospital Movement Disorders Clinic for all aspects of movement disorders. His research interests include psychiatric aspects of Parkinson s disease and the effects of deep brain stimulation.
