2 a message from our chapter president Dear Friends, As the weather cools and the leaves start to turn, we approach my favorite time of the year. September brings World Alzheimer s Month and the start of our 2012 Walk to End Alzheimer s season. Our challenges and opportunities are great as the number of individuals and families affected by Alzheimer s disease continues to increase dramatically. World Alzheimer s Month provides the opportunity to join with others around the world to create awareness and take action in this global fight against Alzheimer s. The back cover of this newsletter includes some of the many ways that you can become involved in this world wide effort. The dates and locations of twelve walks are listed on Page 5. We encourage each of you to register and participate in a walk near your community. We also encourage you to form a team of friends, family members and coworkers and experience the excitement of the walk together knowing that you are making a difference. Without the funds raised through the Walk to End Alzheimer s, we would be extremely limited in our ability to provide programs and fund research. Each participant, each team and each dollar raised is important. If each individual receiving this newsletter would support Walk to End Alzheimer s in some way, we would exceed our goal and create awareness like never before! We welcome our two new board members, Charlie Williams and John Belissary. Charlie and John join our other board members as they provide leadership to our chapter. I would also like to thank Rev. Kaye Fox for serving as a member of the National Early Stage Advisory Group and sharing her experience and knowledge in this important effort. You will read in this edition about the many programs and services that we have here in South Carolina. We are here to support families who are faced with one of the most devastating of all diseases. The time is now because Alzheimer s won t wait. Together we can make a difference for those currently living with this disease as we move closer to realizing our vision of a world without Alzheimer s. Thank you for your ongoing support. Give us a call at to connect to these services! 24-Hour Helpline Staffed 24 hours/day, 7 days/week our helpline offers assistance in 140 different languages any time day or night. Information & Referral We offer a variety of educational literature, and our informed staff can make referrals to helpful community resources. Our chapter newsletter is also available by mail or . Caregiver Support Groups 90+ support groups are offered across the state to help caregivers cope and better understand the disease process. No-cost sitters can be provided to families to ensure that caregivers are able to attend support group meetings. Early Stage Dementia Support Services A diagnosis of dementia is a life-altering fact. We are currently seeking persons who have been diagnosed with early-stage Alzheimer s or related dementia and their carepartners who are interested in learning the basics of living with the disease while sharing experiences with those who understand. To learn more, please call MedicAlert + Safe Return We offer a nationwide wanderers safety and identification program. In partnership with local authorities, this program provides identification jewelry, important medical information, and emergency assistance to families. It has an impressive record of rapidly recovering individuals who wander away from home. Respite Our caregiver respite voucher program provides financial assistance to arrange short-term, professional care through in-home care agencies, adult day care centers, or short-term residential care. For an application, call or visit 24-Hour Helpline: Community & Professional Education Year-round, we offer conferences, workshops, and trainings for a variety of audiences including professionals in the medical and senior services industries, social workers, clergy, hospice professionals, businesses, law enforcement, as well as caregivers and persons with dementia. One of the benefits of early detection is that people in the early stages of Alzheimer s disease retain many abilities and can contribute in the planning of their own care and to the broader fight against Alzheimer s disease The Alzheimer s Association recognizes the importance of this valuable perspective and is pleased to welcome Summerville local Rev. Kaye Fox to the National Early-Stage Advisory Group. Fox, who has Alzheimer s disease, is a retired pastor and aerospace engineer with extensive experience in advocacy, fundraising, public speaking and volunteerism. She is also active in the Alliance For Full Acceptance in Charleston and Charleston Area Transgender Support. Fox lives in Summerville, SC, with her spouse Lisa. 8 caregiver tips Avoiding and Coping with Hospitalization Staying at a hospital can make anyone feel anxious and upset. The experience can be especially traumatic for a person with Alzheimer s disease or another dementia. The change from home to an unfamiliar environment and the added stress caused by other medical treatments and possibly pain are likely to make the person more confused and disoriented than usual. Knowing what to expect can help you understand these changes. Planning ahead can help you avoid unnecessary hospital visits and make unavoidable hospitalization less difficult for everyone. Avoiding unnecessary hospitalizations It can be difficult to get someone to go to the doctor, especially when that person has Alzheimer s disease or related dementia. However, a good relationship with a primary care physician can help prevent crisis situations that can lead to hospitalization. Whenever possible, keep regularly scheduled doctor s appointments, and schedule an appointment if there is a new concern about the person s health. Schedule the appointment at a time of day when your loved one typically feels rested (e.g., early in the day or just after an afternoon nap). Be creative if you need to. Tell the person you need to go to the doctor and ask if they will come along, or that you re going to the doctor and then out to lunch or for ice cream. Take the emphasis off the doctor visit. When a procedure is medically necessary, always ask if the procedure, test or treatment can be done in an outpatient clinic. Preparing for an expected hospitalization Ask questions. Be sure you understand the person s diagnosis, the name of the procedure, the risks and benefits, expected results, and expected length of recovery. Talk with the doctor about how the procedure and hospital stay are likely to affect the person s thinking and behavior. If anesthesia will be used, ask how it will affect the person s level of confusion. Have a family meeting or conference call to discuss responsibilities. For example, who will take the person for preadmission testing and who will stay at the hospital during surgery. Alternate visiting hours to share caregiving. Preparing for an unexpected ER visit or hospitalization Prepare for an emergency room visit or unexpected hospital stay by creating an emergency kit in advance, including: a list of current medications and food allergies copies of advance directives insurance information name and phone number of primary care physician name and phone number of friends or family members who could stay with the person in the emergency room while you are filling out forms a note explaining the person s dementia and particular needs or behaviors nonperishable snacks change of clothes extra disposable briefs if they are usually worn pen and paper to write down symptoms and doctor s or nurse s instructions Be sure that hospital staff who interacts with the person knows that he or she has Alzheimer s disease or another dementia. If the person is more confused in the hospital than at home, let the staff know that, too. Provide information about his or her personal habits, diet or eating preferences, and any medications that he or she is taking. Remain with the person as much as possible. Try to be in the room when the person wakes in the morning, when medications are given, catheters are inserted, or when the doctor stops by. If needed, hire an in-home care sitter to stay with your loved one when you can t be there. 8 Summer - Fall 2012

9 advocacy news SC law helps protect persons with dementia from termination of electric and natural gas services This spring, the SC General Assembly passed H 4108, a bill sponsored by SC Rep. Karl Allen of Greenville, designating persons with dementia as special needs account customers with electric and natural gas providers. This designation can help protect persons with dementia from termination of services. This designation is not automatic! You must contact your loved one s electric and/or natural gas providers to request a Special Needs Account Form, which must be submitted along with a medical statement from the person s doctor. Many providers, such as Duke Energy and the SC electric cooperatives, have this form available on their websites. Please make the call before the winter sets in, and help us spread the word to other family caregivers! Alzheimer s Caregiver Respite Program remains funded at last year s levels We are grateful to the South Carolina General Assembly for continuing to fund the Alzheimer s Caregiver Respite Program through the budget of the SC Department of Mental Health. After two years of budget cuts, we are thankful that the funds have been sustained at the current levels this year. A special thank you goes out to our advocates who took part in our State House Day in March or contacted your state legislators on behalf of this program... we could not do it without you! The National Alzheimer s Plan was released in May, and one of its key goals is to prevent and effectively treat Alzheimer s disease by In order to support this goal, the Alzheimer s Association convened leading experts to develop a goal-directed scientific agenda and to assess annual and 10 year research costs. The recommendations, developed by a work group of 20 of the nation s leading Alzheimer s researchers and vetted by more than 5,000 researchers, have set a benchmark for the level of investment necessary to achieve this goal. Now it s up to Congress. Please join us in reaching out to your members of Congress to urge their commitment to $100 million in resources for Alzheimer s research, education, care and support. 9

10 focus on research Bapineuzumab Fails in Second Phase 3 Clinical Trial, Mixed Results in Solanezumab (Eli Lilly) Phase 3 Trial On Monday, August 6, 2012, Pfizer and Janssen announced negative topline results from a Phase 3 clinical trial of an experimental therapy known as bapineuzumab in people with mild to moderate Alzheimer s disease who did not carry the ApoE4 Alzheimer s risk gene. In this 18-month, multicenter, randomized, doubleblind, placebo-controlled study of approximately 1,300 people with Alzheimer s, the experimental drug did not provide benefit on tests of cognition (memory, thinking) or function/disability. As a result, the companies have discontinued their bapineuzumab IV studies in patients with mild to moderate Alzheimer s. However, the negative results from this study do not mean that the amyloid hypothesis of Alzheimer s disease has been disproven. In fact, on August 24, 2012, Eli Lilly and Co. announced today interesting results from two Phase 3 studies of an experimental drug therapy called solanezumab in mild to moderate Alzheimer s disease. While solanezumab did not meet their primary endpoints, it showed a statistically significant slowing of cognitive decline in people with mild to moderate Alzheimer s disease. If this finding can be duplicated, it suggests that an Alzheimer s therapy targeting beta amyloid can have a beneficial effect on cognitive abilities in people with mild to moderate Alzheimer s. That would be a major step forward in the fight against Alzheimer s. The bad news. Bapineuzumab Phase 3 trials did not show benefits on tests of cognition and function in people with mild to moderate Alzheimer s disease. As a result, the pharmaceutical companies conducting the trials have discontinued their bapineuzumab IV studies. The good news. For the first time ever, longterm stabilization of Alzheimer s disease symptoms has been reported for an experimental therapy in clinical trails. In a test of Intravenous immunoglobulin (IVIG/Gammagard, Baxter) participants showed no decline on measures of cognition, memory, daily functioning and mood over the course of three years. The bottom line. We learn more about Alzheimer s from every study. These results-- even the setbacks-- provide critical information to the research community for application in future studies. Alzheimer s Treatment Study Reports Three Years with No Decline in Memory and Function The first report of long-term (three-year) stabilization of Alzheimer s disease symptoms with immunotherapy was reported in July at the Alzheimer s Association International Conference 2012 in Vancouver, Canada. Intravenous immunoglobulin (IVIG/Gammagard, Baxter) is a blood product that is administered intravenously. Each dose contains pooled antibodies extracted from the plasma of more than 1,000 blood donors. This type of treatment is typically used to treat immune deficiencies, autoimmune diseases, and acute infections. Positive results of a placebo controlled Phase 2 study in mild to moderate Alzheimer s were previously reported. The 24 participants in that study received six months of treatment followed by a 12-month open-label extension where all subjects received IVIG. Several doses were tested. To evaluate the long term effects of IVIG, participants were offered additional IVIG treatment at a single standardized dose (0.4mg/kg every 2 weeks) for an additional 18 months. Sixteen of the originally enrolled subjects received treatment through month 36, including five originally given a placebo and 11 treated with various doses of IVIG. The researchers found that: Study participants who were treated with IVIG 0.4g/ kg every two weeks for the full 36 months (n=4) had the best outcome, with no decline on several standard measures of cognition, memory, daily functioning and mood (ADAS-Cog, CGIC, 3MS, ADCS-ADL, NPI, QOL) at the three year endpoint. As a group, the 11 participants who received IVIG for the full 36 months had favorable outcomes in terms of their thinking abilities, behavior and daily function. The five participants who were initially treated with a placebo and then switched to IVIG declined while on placebo but experienced less rapid decline while receiving a uniform dose of IVIG. A Phase 3 trial is in progress with results expected in less than one year. These results will offer more definitive data on the efficacy of 18 months of IVIG treatment. While this news is exciting, it is important to note that additional research is needed.
