6 Wolfgang Aulitzky, MD is President of the SALZBURG Stiftung of the AAF and the Medical Director and member of the Executive Board of the American Austrian Foundation New York. He is Associate Dean for International Medicine and Distance Learning, Adjunct Prof. of Clinical Urology and Adjunct Prof. of Clinical Reproductive Medicine at Weill Medical College of Cornell University/NewYork-Presbyterian Hospital. He is also Associate Prof. of Urology at the University of Innsbruck. He is a member of many scientific Societies and was awarded the Zuckerkandl Prize of the Austrian Society of Urology in He received the Silver Medal (2005) and the Golden Medal of Merit of the Republic of Austria In his position as Director of the Medical Program of the American Austrian Foundation he initiated the Open Medical Institute, a scientific and educational collaboration of Weill Cornell Medical College, the Children Hospital of Philadelphia, Duke University Medical Center, College of Physicians and Surgeons of Columbia University, the Cleveland Clinic and leading hospitals in Austria. Dr. Aulitzky was a research associate at the University of Uppsala, Sweden and the Rockefeller University, New York. He received his training as an urologist at the University of Innsbruck and the General Hospital of Salzburg. As a clinician he works as Chairman of the Department of Urology at the Confraternität-Privatklinik Josefstadt in Vienna. He is the author of more than 140 publications on Urology, Andrology and Health Care Management and is co-author of many books on basic and clinical urology/andrology. 7 Hugh C. Hemmings Jr., MD, PhD, FRCA (Course Director) is Joseph F. Artusio Professor and Chair of Anesthesiology and Professor of Pharmacology at Weill Cornell Medical College, and Anesthesiologist-in-Chief at NewYork Presbyterian Hospital/Weill Cornell Medical Center. Dr. Hemmings earned a BS in Biochemistry from Yale College, a PhD in pharmacology from Yale Graduate School, and an MD from Yale Medical School. His graduate work in the laboratory of Paul Greengard, PhD was cited in his 2000 Nobel Prize in Physiology and Medicine. He completed postdoctoral work at The Rockefeller University, a residency in anaesthesia at the Massachusetts General Hospital, and a fellowship in cardiac anesthesia at NewYork Presbyterian Hospital/Weill Cornell Medical Center, where he has been a faculty member for 23 years. An internationally recognized neuropharmacologist, Dr. Hemmings is an expert in the synaptic effects of general anesthetics and mechanisms of neuronal signal transduction. His research in these areas is supported by grants from the National Institutes of Health. He is active in several national and international societies for anesthesiology and neuroscience, and serves as the Editorin-Chief for the British Journal of Anaesthesia. He has been elected Fellow of the Royal College of Anaesthetists and as a member of the Association of University Anesthesiologists. Dr. Hemmings has authored more than 100 articles in anesthesiology and neuroscience, and has edited four books, including Foundations of Anesthesia 2e published in 2006 and Pharmacology & Physiology for Anesthesia: Foundations and Clinical Application published in Hugh C. 8 Klaus Markstaller, MD (Co-Course Director is an anaesthesiologist and intensivist who engages in patient safety and perioperative medicine. His research focus is on functional lung imaging and pathophysiology of acute lung injury. Prof. Markstaller was born in Nuernberg / Germany. In 1998 he graduated magna cum laude at Ulm University and qualified as a professor of anaesthesia and intensive care at Johannes Gutenberg University in Mainz in He is a reviewer for institutional funding organisations (e.g. German Research Foundation, Alexander von Humboldt Foundation) and recognized scientific journals. Prof. Markstaller published a large number of articles in international peer reviewed journals and received numerous international research awards and grants. As a member of a group of investors he realized two patent applications in Germany and the USA. He is a member of the European Academy of Sciences and Arts and Fleischner Society and president elect of the Austrian Society of Anaesthesiology, Resuscitation and Intensive Care Medicine. Prof. Markstaller is currently head of the Department of Anaesthesia, General Intensive Care and Pain Medicine as well as founder and co-head of the Center of Perioperative Medicine at Vienna General Hospital (AKH)/Vienna Medical University. He lives in Vienna with his wife Monica. 9 Nikolaos ("Nick") Skubas, MD, DSc, FACC, FASE (Co- Course Director) is an anesthesiologist who specializes in cardiothoracic anesthesiology and perioperative echocardiography. Dr. Skubas grew up in Greece and received his medical degree from Aristoteles University School of Medicine, Thessaloniki, Greece. He subsequently completed his residency at Case Western University in Cleveland, Ohio and a two-year fellowship in Cardiothoracic Anesthesia at Washington University in Saint Louis, Missouri. Dr. Skubas' clinical research evolves around the applications of echocardiography and point of care testing in cardiac and general surgical cases. He is the Executive Section Editor in Anesthesia & Analgesia for Perioperative Echocardiography and Cardiovascular Education and an Associate Editor to Anesthesiology. He has been a member of the writing committee of the AHA/ACCF 2014 Valve Heart Disease Guidelines and serves as the co-chair of the Echo Week, the SCA organized Annual Comprehensive Review & Update of Perioperative Echo. He is the Chair of the Fellowship Committee of the American Society of Echocardiography. Dr. Skubas is currently on the faculty of Weill Cornell Medical College and New York Hospital as Professor of Clinical Anesthesiology and adjunct Professor of Anesthesiology in Cardiothoracic Surgery and serves as Director of Cardiac Anesthesia. 10 Gunisha Kaur, MD, MA is an anesthesiologist who specializes in global health and human rights. Dr. Kaur earned her B.S. from Cornell University in 2006, graduated from Weill Cornell Medical College in 2010, and completed her Anesthesiology Residency training at Weill Cornell Medical College/New York Presbyterian Hospital in She earned her master's degree in medical anthropology from Harvard University in Dr. Kaur's research interests are in physician education in global health and in human rights. Her research on the suppression of human rights in South Asia culminated in the publication of a book entitled Lost in History: 1984 Reconstructed. She has worked as a researcher and writer for Physicians for Human Rights and currently studies chronic pain assessment and management in survivors of torture. Dr. Kaur is Assistant Professor of Anesthesiology at Weill Cornell Medicine and New York Presbyterian Hospital. She is Director of the Department's Global Health Initiative and Program Director for the Anesthesia Global Health Fellowship. She lives in New York City with her daughter Jiya and her husband Simran, who is a Professor of Religion at Trinity University. 11 Klaus Ulrich Klein, MD is an anesthesiologist and intensive care physician who specializes in how to avoid perioperative lung and brain dysfunction. Dr. Klein grew up in Frankfurt/Main in Germany, and completed his undergraduate studies at Medical Schools in Erlangen- Nuremberg and Heidelberg-Mannheim. He graduated from Ruprecht-Karls-University in Heidelberg and completed his junior house officer rotation at The Royal London Hospital, Plymouth Hospitals and Torbay Hospital in the United Kingdom. He completed his residency at Johannes- Gutenberg University Medical Center in Mainz, Germany. After having moved to Medical University of Vienna, Austria, he became Associate Professor for Anesthesia and Critical Care Medicine. Dr. Klein s research interests include injury of vital organs such as the lungs, heart and brain in the perioperative setting. In particular, he is interested in unravelling the injurious effects of altered oxygen conditions (oxygen oscillations, hyperoxia) in acute lung injury and obstructive sleep apnea. Further, his research focuses on the protective effects of argon preconditioning on lungs and myocardium. Dr. Klein has an expertise in translational research environments, including cell culture experiments, small and large animal experiments and clinical research. Recent publications include manuscripts in the British Journal of Anaesthesia, European Journal of Anaesthesiology, Anesthesia and Analgesia and Shock. He is a peer reviewer for multiple major anesthesia and critical care journals and is an associate editor to the Journal of Neurosurgical Anesthesiology. Dr. Klein's research efforts have earned him ongoing grants from the Austrian Science Fund FWF and the Major of the City of Vienna Scientific Fund. Dr. Klein is currently on the faculty of Medical University of Vienna as Associate Professor of Anesthesiology and Critical Care Medicine. He lives in Austria with his wife Verena and his son Gianluca. 12 Peter Marhofer, MD is an anaesthesiologist and intensive care physician who specialized in trauma, paediatric - and regional anaesthesia. Dr. Marhofer grew up in Vienna, Austria and studied medicine at the School of Medicine, University of Vienna (now Medical University of Vienna). He started his fellowship in Anaesthesia and Intensive Care in 1994, and habilitated in Dr. Marhofer's research interest include regional anaesthesia and paediatric anaesthesia. He was part of the research group developing ultrasound in regional anaesthesia. He leads the scientific working group "Regional and Paediatric Anaesthesia" at the Department of Anaesthesia and Intensive Care Medicine, Medical University of Vienna. The group designs currently several volunteer studies in cooperation with the University Department of Clinical Pharmacology, Medical University of Vienna. This cooperation is only one part of a large international network, which was created during the last 20 years. Dr. Marhofer has a current H-factor of 33 with 109 Pubmed citations, more than 250 publications. He wrote 2 books and gave more than 500 national and international lectures. He is editorial board member of the highest ranked anaesthesia journal, the British Journal of Anaesthesia. Dr Marhofer is currently on the faculty of the Department of Anaesthesia and Intensive Care Medicine, Medical University of Vienna as Professor and the head of paediatric anaesthesia and paediatric intensive care medicine. He lives close to Vienna with his wife PD Dr. Daniela Marhofer (also anaesthesiologist and intensive care physician). He has one daughter (Melanie, 18 years old and medical student). 13 Milica Markovic, MD is a American Board-Certified Anesthesiologist who specializes in Regional Anesthesia and Acute Pain Management. Dr. Markovic grew up in Serbia and moved to the USA during high school. She was an undergraduate at Bennington College in Vermont, where she earned a bachelor of arts in psychology. She earned her MD at the Vanderbilt University School of Medicine in Nashville. Dr. Markovic completed internship training in Internal Medicine at the New York University Medical Center, and then completed residency training in Anesthesiology at the Beth Israel Deaconess Medical Center, Harvard University Medical School in Dr. Markovic held a Fellowship in Regional Anesthesiology and Acute Pain Medicine at the Hospital for Special Surgery in New York from Dr. Markovic joined the faculty at the Department of Anesthesiology at Weill Cornell Medical College in September Dr. Markovic is committed to advancing education worldwide in regional anesthesia and acute pain medicine. She instructs at prominent anesthesia meetings in the US and abroad. Dr. Markovic is also interested in perioperative medicine, and serves as an Assistant Director of Pre-anesthesia Evaluation Center, Her research interests are in preoperative evaluation, pain management, and telemedicine. The fellows ranged from residents to consultants in Anesthesiology and arrived from 32 countries and regions from Mexico to Mongolia and from Estonia to Ethiopia. Weill Cornell Medicine (4) and Medical University of Vienna (3) faculty presented a diverse range of educational topics in all aspects of anesthesia and perioperative medicine. Each presentation was followed by questions and answers to share the fellows diverse experience. For the first time in an Open Medical Institute seminar, topics on world refugee crisis, humanitarian disasters and global surgical agenda were included in the curriculum, to raise the fellows and faculty s awareness on peoples plight. On Tuesday and Thursday afternoon, the fellows presented their cases and answered questions from the audience. Six of the best presentations were selected to be shared over the internet and considered for future publication in peer-reviewed journals. Two favorite educational activities took place: a how-to publish workshop and a hands-on surface ultrasound workshop. In the latter, the fellows took turns to learn and become familiar how to use point of care ultrasound for regional anesthesia, cannulation of central venous structures and focused cardiac ultrasound. On Thursday evening, fellows and faculty enjoyed a chamber music recital by a dynamic duo of young musicians. At the farewell reception and graduation dinner, certificates of attendance were awarded to the participating fellows and the best overall participant was recognized. The fellows and faculty departed while remaining connected! A facebook page specific to the AAF Anesthesiology Seminar was founded for the participants to join and continue to interact in the future.

28 SEMINAR DIARY FROM FELLOW Diary of Valihanov Abror, MD Republican Research Center of Emergency Medicine Tashkent City, Uzbekistan 12 March 18 March Sunday, 12 March My journey to Salzburg took 16 hours, I arrived in Munich by two planes and travelled by train to Salzburg. From Salzburg Main Train Station I took bus number 6 and enjoyed magnificent views of Salzburg city on my way to Arenberg Castle. When I reached Volksgarten I got off the bus and asked a lady in the street: Can you tell me please, where the Arenberg Schloss is?. She quickly showed the way and said I must be a doctor. It was an interesting start. I checked in at 5pm, my room was beautiful, clean and was located on the 5 th floor. At 7pm I went down to the Reception Lounge and saw many of fellows, all looked a bit tired from a long journey, but were happy and excited. We were offered cocktails and had about an hour to get introduced to each other. Fortunately, everybody was wearing a badge provided by Arenberg staff, with a name and a country of origin on it, which made acquaintance a lot easier. At 8pm Prof. Aulitzky delivered a wonderful welcome speech, he congratulated all the fellows with successful application and mentioned that there were 37 participants from 32 different countries. 32 different countries? WOW, amazing how international and diverse our group was. Prof. Hemmings then briefly welcomed us and introduced to present faculty members. After the welcome reception we offered a tasty dinner. Despite there was a big table for faculty members, our teachers had dinner at our tables with us asking how was the journey, where are we from and many others. I found it utterly friendly. Everybody went to bed after the dinner, we had a busy week ahead of us. Monday, 13 March I woke up at 6am hearing birds singing and took a walk around Arenberg. Air was fresh, small wet path took me to the top of the hill where I could behold the center of Salzburg in front of me and Arenberg behind. It is amazing how the castle is so proximate to the city center and close to the nature. After the breakfast all went to the hall on the third floor, our classroom. Course Director, Prof. Hemmings introduced us to all faculty members and briefly clarified the schedule and aims of upcoming seminar. We started with Pre-seminar Test. There were 20 questions from various fields of Anesthesiology and Critical Care. The first lecture, Emerging mechanisms of general anesthesia, was delivered by Prof. Hemmings. He went through major mechanisms of general anesthesia step by step and made things a lot easier than in books, in the end I had more questions than answers. 10am Coffee Break Hot coffee and tea, cakes and fruits were waiting for us on the first floor. After the break Cardiac anesthetist, Prof. Nick, gave a phenomenal lecture, Focused Cardiac Ultrasound, which was very interactive. He showed how an anesthetist can have 5 simple views of heart in just 3min using basically any ultrasound machine. I will remember his words, FoCUs is your ultrasound stethoscope, simple, easy and informative, for my whole career. Then Pediatric anesthetist, Prof. Marhofer, shared his experience in ultrasound guided vascular access. I should admit the knowledge I gained during his presentation will make my vascular access procedures much safer pm Lunch Deputy Director of AAF Mrs. Bourg made a short presentation about the AAF and the Open Medical Institute, the main principles and ideas of this institution. The third lecture of the day was delivered by Prof. Markovich Care of Opiod Tolerant Patient. We had two Workshops after the lectures. The first by Gunisha Kaur about presenting a clinical case, which I believe was very helpful for all us in making our presentations more compact and accurate. Finally, the last workshop was on being published in a peer-reviewed journal. Prof. Hemmings, Chief

29 Editor of British Journal of Anesthesia, discussed how to write and organize our scientific papers in a successful way. Dinner was at 7pm. After the dinner all fellows were invited by Faculty to a local pub. Steinlechner was a nice pub. We occupied three big tables, our teachers paid for our drinks, which was awfully sweet of them. At 10pm I went back to Arenberg, perfect end to a busy day. Tuesday, 14 March The day began in the same fashion; I got up at 6am and walked till 7am. After the breakfast we all gathered in the lecture hall. Before the coffee break we had two lectures: Anesthetic Neurotoxicity was delivered by Prof. Hemmings and Ultrasound Guided Regional Anesthesia New Horizons by Prof. Marhofer. Prof. Hemmings lecture was deep and insightful as usual. Prof. Marhofer told us how the whole ultrasound guided regional anesthesia and vascular catheterization started in Vienna during his residency more than twenty years ago. 10am Coffee Break After the break we had more ultrasound lectures. Prof. Markovic discussed ultrasound guided major blocks which was quite interesting for the audience, cause most of the fellows did not practice regional anesthesia often. Prof. Marhofer delivered a lecture about pediatric regional anesthesia. He shared with unique photo and video materials from his practice during the presentation; like giving a regional anesthesia to a 500g baby or performing a thoracic epidural catheterization on a 3kg infant. Although I am not a pediatric anesthetist, but I found shared cases and the whole lecture fascinating pm Lunch We had two workshops after the lunch break. In the first workshop 12 fellows presented their cases. Most memorable ones for me were cases of Dr. Awais and Dr. Tsolomon. The next workshop was very practical and beneficial for my daily teaching and clinical practice. There were four stations with ultrasound machines and volunteers, so we could practice cardiac ultrasound, ultrasound guided regional anesthesia and vascular access on them. Prof. Marhofer signed and gifted me his book Ultrasound Guidance in Regional Anesthesia: Principles and practical implementation. He is a great guy. Dinner was at 7pm as usual. About 20 of us walked in the city center after the dinner, Salzburg is really beautiful at night. Wednesday, 15 March The first lecture of Wednesday was about refugee crisis and the problems and conditions that refugees have which might be relevant to an anesthesiologist. The lecture was delivered perfectly by Dr. Kaur. She showed a video trailer about refugees which was so emotional and made most of the audience cry. The second topic was about fluid responsiveness by Prof. Nick and as always it was very interesting and interactive. Take away message from his lecture blood pressure should not be an indicator or target for fluid resuscitation!. 10am Coffee Break Dr. Markovic talked about complications of regional anesthesia and ways to reduce them. I found her lecture very useful, because I perform regional anesthesia in my daily practice. Finally, the last topic of the day was Aortic Stents. Dr. Nick was the lecturer and we discussed various types of stents and their efficacy. Although I do not give anesthesia for cardiac or major vascular surgery, but the presented data by Prof. Nick was very simple to understand. I enjoyed his lecture again pm Lunch Wednesday was a really good day indeed, because we had no classes after the lunch and the weather in Salzburg was warm and sunny. Obviously, we all went to the city center right after the lunch. We visited the Festung, the Mozart Square and many other historical places of the old city. We stopped to have beer in a traditional pub, then we did shopping and walked around till the dinner time. After the dinner I stayed in my room in order to prepare my presentation for the next day. Thursday, 16 March The 4 th day of the seminar was richer than any other day for me we had four wonderful lectures and listened to 24 cases of fellows including mine. Prof. Ulrich Klein began the day with Update on ARDS

30 Treatment. However, most of us were very familiar with ARDS and its management guidelines, but the presented data by Prof. Ulrich Klein was amazing, especially the way he organized his lecture was very helpful in understanding the management strategy over the years and existing problems for future studies. I was also very interested in research projects by Professor where he used some new anti-edema drugs which were in phase II trials. Dr. Kaur talked about emergency humanitarian disasters and the role of anesthesiologists in such scenarios. 10am Coffee Break After the break Prof. Nick presented his last topic Early Recovery and Fast-Tracking, which was a large ongoing project in many clinics across the globe. We discussed the benefits and problems which may hinder ERAS implementation. The pinnacle of Salzburg seminars for me was the lecture by Co-Course director Prof. Markstaller Perioperative Medicine. When I first saw the time-table of our seminars a month ago, I thought what new could be said about this topic. I was very wrong. Prof. Markstaller talked about a whole new approach in management of surgical patients where anesthesiologists take part from the very beginning till the very end, where the collaboration with the surgeons is paramount. Amazingly, this system was implemented in the Medical University of Vienna where Prof. Markstaller is a head of department. I am already planning to come to MUV and see this system in action with own eyes pm Lunch We had 24 presentations by fellows after the lunch. I was the 12 th and was a bit nervous during the presentation. There were some very unique cases and I liked most the ones by Dr. Casian about the damage control in spinal surgery and Dr. Aurimas where the patient suffered from Ventricular Fibrillation during a simple ear surgery. After the dinner we enjoyed chamber music by Zsofia Szabo and Emese Badi which was organized on the third floor of the Arenberg. After the busy day where all of us finished our presentations and were feeling musical following the concert we wanted the fun to continue and went to an Irish pub Schamrock which was right in the city center. There we enjoyed live music, local beer and danced a lot. We came back to Arenberg at midnight. Friday, 17 March Friday, the final day of wonderful seminars, where all fellows and faculty members had already become friends was coming to an end. Today s programs had more practical approach than previous ones. Before the break Prof. Markovic talked about starting a regional anesthesia service pointing out important aspects and the role of each team members. Then Dr. Kaur discussed global surgical agenda where she stated that less and less people are having access to operations. Surprisingly, the number of anesthesiologists across the globe was not increasing in proportion to the actual demand, so in many developing countries anesthesia was indeed being provided by nurses. 10am Coffee Break When the coffee break was over we split into two groups and had Problem Based Learning Discussions with Prof. Markovic and Dr. Kaur, where everybody had to participate and express one s opinion. I found PBLD quite a useful teaching method and intend to use in my teaching practice with my residents. At last we had a post-seminar test. Questions seemed simple and easy this time. After the test we discussed test questions with Prof. Nick and we learned that most of us answered all correct pm Lunch Following the lunch, we were free till the Graduation Dinner at 7 in the evening. Many of us spent the afternoon ironing their clothes and preparing for the dinner. At 6pm all fellows and faculty members met again in the Reception s Lounge where wine, cocktails and cold drinks were offered. We spent an hour talking to each other, taking photos and enjoying the last moments of this surreal week. 7pm Graduation Dinner Graduation dinner was organized on the third floor. Prof. Aulitzky gave another wonderful speech. He congratulated all fellows with completion of seminars and we all were awarded certificates of attendance and CME certificates. Two fellows, who achieved most progress in tests received presents and additional certificates. Then I and other 5 fellows were awarded certificates for excellent case

31 presentation. I was beyond happy and awfully sad at the same time, because our journey in Salzburg was approaching its final moments. Feeling mixed emotions, we went to the city the irish pub, Schamrock, was full that day, because of the St Patrick s Day. We found another place and had fun. We became so close and nobody wanted to leave, we ended up staying out till 3am. When we got back to Arenberg some of our friends were already checking out and leaving for the airport and train station. We wished a good journey to each other. Saturday, 18 March I could not sleep that day, I packed my things and waited the day the to break. At 7am I had a breakfast where I met some of fellows for the last time and thanked all the staff I saw for their excellent service and checked out at 8am. It was a 30-minute walk to the Main Train Station of Salzburg. I took a train to Munich and from the Munich Airport I took my plane. It was a very productive and beneficial week to my future career, where we were able to get knowledge from the world famous specialists in anesthesiology. Thank you OMI for an unforgettable experience.
