1 National Falls Prevention Resource Center Purpose Increase public awareness and educate consumers and professionals about the risks of falls and how to prevent falls. Serve as the national clearinghouse of tools, best practices, and other information on falls and falls prevention. Support and stimulate the implementation, dissemination, and sustainability of evidencebased falls prevention programs and strategies. 1

5 Colorado Colorado Department of Public Health and Environment A Matter of Balance Stepping On Tai Ji Quan: Moving for Better Balance 2,000 older adults and adults with disabilities Stepping On or Matter of Balance will be embedded into 20 hospital trauma systems with a sustainable funding mechanism. 5
6 Florida Health Foundation of South Florida A Matter of Balance Otago Exercise Program Tai Ji Quan: Moving for Better Balance Others: Enhance Fitness, Enhance Wellness, and Medicare Part D Medication Therapy Management Services 3,000 older adults and adults with disabilities Increased capacity to deliver billable evidence-based fall prevention programs among over 60% of Area Agencies on Aging in Florida. 6

8 Iowa Iowa Department on Aging A Matter of Balance Arthritis Tai Chi Stepping On 1,000 older adults and adults with disabilities Expand programs for older adults with mobility impairments, those living in rural areas, and adults with disabilities, specifically those with brain injuries or Parkinson's disease. 8
9 Massachusetts Elder Services of the Merrimack Valley A Matter of Balance Tai Ji Quan: Moving for Better Balance Otago Exercise Program 1,900 older adults and adults with disabilities Mobilize at least 50 public/private partnerships at the state and community levels. 9
10 Michigan Tribal Grantee Little Traverse Bay Bands of Odawa Indians Tai Ji Quan: Moving for Better Balance Yak Trax Walkers 130 Tribal Elders of the Odawa Nation The Advisory Board will develop a falls prevention program sustainability plan that will also align with goals to prevent and reduce the incidence of diabetes among Elders. 10
11 Michigan Tribal Grantee Match-E-Be-Nash-She-Wish Band of Pottawatomi Indians Intervention Stay Safe, Stay Active 35 Tribal Elders of the Gun Lake Tribe Expand the diabetes care program to incorporate fall risk assessment and recruitment of participants for the falls prevention program, and share the results and lessons learned with other tribes through the existing Diabetes Program Partnership. 11

13 North Carolina North Carolina Division of Aging and Adult Services A Matter of Balance Tai Ji Quan: Moving for Better Balance Otago Exercise Program 1,500 older adults and adults with disabilities A centralized online hub will be established to offer a single point of entry for participants, providers, and implementers to receive and disseminate falls and falls-risk activities and programs, and to conduct data entry, program evaluations and quality assurance. 13
14 New Hampshire Foundation for Healthy Communities A Matter of Balance Tai Ji Quan: Moving for Better Balance 360 older adults and adults with disabilities Create a self-sustaining referral system with falls prevention programs filled to capacity. 14
15 Utah Utah Department of Health (UDH) Intervention Stepping On 336 older adults and adults with disabilities In collaboration with the University of Utah and Intermountain Healthcare, UDH will develop a pathway referral system to enroll at-risk older adults in falls prevention classes. 15
16 Vermont Vermont Department of Health Intervention FallScape 1,000 older adults and adults with disabilities The capacity of Emergency Medical Service agencies to deliver a standardized, evidence-based falls prevention program in the community setting integrated with Area Agencies on Aging and existing prevention networks. 16
17 Wisconsin Tribal Grantee Mole Lake Sokaogon Chippewa Community Intervention Stepping On 60 older adults and adults with disabilities Increase in the number of community experts, physical therapists, vision specialists, community safety specialists and medication specialists who are part of Stepping On, which will in turn increase self-care outcomes among participants. 17
