2 Registration Information The UNC is proud to announce the 2015 UNC Eye Symposium with Technician Continuing Education Session, sponsored by the UNC School of Medicine. The Symposium will be held on Saturday April 11, 2015 at the William and Ida Friday Center for Continuing Education in Chapel Hill, NC and our faculty will be joined by six nationally recognized guest speakers. The UNC Eye Symposium comprises two sessions running concurrently, the Physician Session and the Technician Session. Continuing Medical Education (CME) credit is made available for the Physician Session by the UNC School of Medicine, while CE credit for the Technician Session has been approved by the Joint Commission on Allied Health Personnel in Ophthalmology (JCAHPO). 7 Policies COURSE DESCRIPTION The UNC Eye Symposium will provide the latest updates and insights in the diagnosis and treatment of eye disorders including anterior segment, glaucoma, age-related macular degeneration, diabetic retinopathy, oculoplastics, and pediatric ophthalmology. TARGET AUDIENCE This conference is designated for Ophthalmologists, Optometrists, Ophthalmic Technicians and Photographers. EDUCATIONAL OBJECTIVES The information presented at this educational activity should increase attendees ability to: 1. Utilize new treatments for inherited retinal diseases and macular degeneration. 2. Understand the importance of 24 hour intraocular pressure and implement management strategies to lower IOP throughout the day and night. 3. Choose appropriate treatment options for acute and chronic manifestations of Thyroid Eye Disease. 4. Use antibiotics in cataract surgery in accordance with the current standard of care. 5. Implement appropriate strategies for pediatric cataract management. ACCREDITATION This activity has been planned in compliance with and adheres to the Accreditation Council for Continuing Medical Education Guidelines for Commercial Support. The School of Medicine of The University of North Carolina is accredited by the ACCME to provide continuing medical education for physicians. Continuing education credits have been approved by the NC Board of Optometry for the Physician s Session. The UNC is accredited by the Joint Commission on Allied Health Personnel in Ophthalmology (JCAHPO) to provide continuing education credit to Allied Health personnel who attend the Technician Session. CONTINUING EDUCATION CREDIT Physicians Session The School of Medicine of The University of North Carolina at Chapel Hill designates this live activity for a maximum of 7.5 AMA PRA Category 1 Credit(s). Physicians should claim only the credit commensurate with the extent of their participation in the activity. Receipt of credit is contingent upon completion of an electronic evaluation within 30 days after completion of the course. More information on this requirement will be made available at the meeting. TECHNICIANS SESSION The Joint Commission on Allied Health Personnel in Ophthalmology (JCAHPO) designates this live activity for a maximum of 6.25 Group A credit(s). Confirmation of Ophthalmic Photographers Society (OPS) credit was not available at the time of printing but will be specified in future course materials. Allied Health personnel should claim only the credit commensurate with the extent of their participation in the activity. Receipt of credit is contingent upon completion of an electronic evaluation within 30 days after completion of the course. More information on this requirement will be made available at the meeting. DISCLOSURES The UNC School of Medicine and UNC Ophthalmology adhere to the ACCME Standards regarding industry support for continuing medical education. Disclosure of the planning committee and faculty s commercial relationships, if any, will be made known at the time of the activities. Speakers will also state when off-label or experimental use of drugs or devices is incorporated in their presentation. A list of supporters, if any, will be made known at the time of the activity. The Tech Session is not sponsored by JCAHPO; only reviewed for compliance with JCAHPO standards and criteria and awarded continuing education credit accordingly; therefore, JCAHPO cannot predict the effectiveness of the program or assure its quality in substance and presentation. REGISTRATION AND FEES Physician Session fees are $199 for MDs, ODs, and PhDs. There is no fee for Residents or Fellows in training who are not seeking CME credit. The Physician Session includes credit, continental breakfast, lunch, and breaks. Technician Session fees are $100 for the Morning Session only (3.75 Group A Credit Hours) or $150 for the Morning Session, lunch, and two (2) workshop sessions (up to 6.25 Group A Credit Hours). Workshop registrations are final and can only be exchanged with another willing registrant. ALL PARTICIPANTS MUST REGISTER ONLINE AT Registrations will not be processed unless accompanied by full payment. Please fully complete the survey at registration and also indicate the workshops you would like to attend (Technician Session only). Workshop availability is on a first-in basis. The deadline for all registrations is Friday, April 3, Registration for both sessions is limited. Late registrations may not receive all conference materials. All registrations must be received by April 3, When your registration is processed you will receive an electronic receipt and/or confirmation. If you have not received your confirmation by April 6, 2015, please call (919) to inquire. REFUND AND CANCELLATION POLICY Cancellations must be received in writing by fax, mail, or to the program coordinator by April 3, Cancellations received after this date will not be refunded. All cancellations will incur a fee of $75 for the Physician Session and $35 for the Technician Session and will be deducted from your refund (if applicable). Transfer of registration to another UNC School of Medicine conference in lieu of cancellation is not possible. Substitutions are encouraged. The University reserves the right to cancel or postpone any course due to unforeseen circumstances. In the event of cancellation or postponement, the UNC Department of Ophthalmology will fully refund registration fees but is not responsible for any related costs or expenses to participants, including cancellation fees assessed by hotels, airlines, rental car, or travel agencies. CONFERENCE LOCATIONS All Physicians Session activities will take place at the William and Ida Friday Center for Continuing Education, Tech Session activities will take place both at the Friday Center and at the Kittner Eye Center, Although every effort is made to provide a comfortable temperature in the meeting room, it is difficult to find a temperature that is comfortable for everyone. Please bring a jacket, sweater, or light wrap in case the room is too cool for your comfort. Special dietary requirements will be honored whenever possible. If you need to inform us of a special need, please be sure to notify our office no later than Friday, March 27, 2015.
