3 KEY TASKS EXPECTED OUTCOMES o Health and Disability Commissioner Providing consultative specialist palliative care to adult patients (and their family/caregivers and whanau) as an integrated member of the multidisciplinary palliative care team Providing patients, family/caregivers and whanau with timely information regarding palliative care service provision within the hospital, hospice and community setting and linking them to community services to improve care co-ordination, a seamless continuum of palliative care and overall outcomes of care Supporting patients, family/caregivers and whanau with complex decision making. Identifying patients in need of general and specialist palliative care including under-served hospitalized palliative populations: especially those with non-malignant disease, Maori and Pacific peoples. Supporting general hospital palliative care provision to facilitate integration of a palliative care approach into the care of patients who are in hospital with a life-limiting illness Initiating and/or contributing to clinical review/case conference meetings as appropriate Assisting staff as required to resolve medical ethical and professional issues Developing and fostering close links to community service including GPs, district nurses, and hospices Promoting an interdisciplinary team approach to patient care and discharge planning at all times Ensuring patient care is safe, clinical practice is appropriate and department practice is innovative and evidence based Participate and co-ordinate a team approach to new service development initiatives to ensure ongoing financial viability for the service Establishing standards of practice/protocols and clear expectations of staff within the interdisciplinary team Developing effective teamwork and collaborative relationships, as appropriate, within the Waitemata District Health Board -JOB DESCRIPTION - Page 3

4 KEY TASKS EXPECTED OUTCOMES interdisciplinary team Dealing with conflict situations, working to achieve a win-win situation Community Engagement Developing and fostering close links to community service including GPs, Residential Aged Care, District Nurses and other hospices Exercising persuasion and influence to further develop relationships Teaching/Research Developing and facilitating undergraduate and postgraduate teaching in palliative care for staff and undergraduate trainees Providing support and teaching as required for registrars and Advanced Trainees in palliative medicine Supporting continuing education programmes within the service Developing and implementing relevant research activities consistent with the strategy of the service Actively encouraging staff to identify opportunities for improvement, when appropriate Call Back Activities This section is draft for clarification with Hospice re first on call vs second on call and expected frequency of roster. Need to reflect how often required to do on call and for how long at a time. Administration Initiating and/or contributing to clinical review/case conference meetings as appropriate Maintaining accurate records at all times The employee is required to participate in the Medical On-Call roster for Hibiscus Hospice. The On-Call roster hours for weeknights are and across weekends and public holidays. When on call, the employee is required to remain immediately contactable and to attend the Hospice within 30 minutes of call. The employee is expected to be familiar with and to comply at all times with the Hibiscus Hospice Medical On-Call Policy. This includes Maintenance of Professional Standards requirements, such as attending organized academic clinical/cme meetings, to ensure ongoing vocational registration Maintain Palliative Medicine knowledge and clinical competence through continuing selfeducation including attendance at relevant courses/seminars/conferences and review of relevant literature Participate in teaching activities consistent with the strategy of the department and providing evidence based and best practice approach to palliative care education Participating in research activities and clinical development projects consistent with the strategy of the service Participating in annual performance appraisal with Clinical Director and Service Manager with 360 degree reviews as required Medical Team Leadership Provide leadership to the Medical Team including coaching, mentoring, supervision and role modelling Sound decision making Confident and credible in areas of responsibility Evidence of language, behaviour and attitude towards staff, patients and stakeholders is aligned with the values of both the Hospice and the DHB Clinical Governance, Quality Assurance & Service Development Participate in developing, reviewing and implementing, the service s strategic planning, resources and educational material Support the effective use of resources to meet priorities Participate in formal and informal palliative care quality activities (e.g. Morbidity and Mortality meetings, journal club, paper rounds and peer review). 6 KEY TASKS EXPECTED OUTCOMES pertaining to the hospital and hospice palliative care service Advocate for and adopting local, national and international palliative care standards and best practice Educate, up-skill, empower and support clinical staff in general palliative care provision Commit to quality improvement in clinical and management practice through participation in audit, research, professional development, benchmarking, credentialing, and accreditation Maintaining individual and team psychological health in order to ensure sustainability of workforce and avoid burnout. Statutory & Treaty of Waitangi obligations Ensures the professional and political integrity of WDHB by carrying out all functions in compliance of the Treaty of Waitangi and by demonstrating a serious commitment to keeping the treaty alive. 11 PERSON SPECIFICATION POSITION TITLE: Palliative Medicine Specialist Minimum Preferred Qualification Fellowship of the Australasian Chapter of Palliative Medicine (or equivalent) Eligibility for vocational registration as a medical practitioner with the NZMC Evidence of progress towards Maintenance of Professional Standards Programme Experience Experience in hospice and or hospital palliative care Trained in principles of evidence-based medicine Skills/Knowledge/Behaviour High standard of written and verbal communication Able to work as an integral member of a multidisciplinary team Able to maintain good professional relationships and be respectful of other team members skills A demonstrated belief in, and commitment to, promoting quality of life.
