1 The British Cardiovascular Society 2014 Workforce Survey A report to the BCS October 2014 Kevin Fox VP Clinical Standards, Andrew Wragg (Survey Clinical Lead), Gemma Cooper (BCS Office Lead for the Survey) Introduction: The BCS has conducted a data collection exercise on the United Kingdom Cardiology Consultant workforce since The aims are to establish an accurate estimate of the workforce and particularly to facilitate workforce planning. While the RCP collates some data there are no equivalent sources of this information that can be regarded as reliable. This year the RCP Census (actually undertaken in 2013) asked Consultant Cardiologists specific questions about their work. The responses and comments are included at the end of this survey report in Appendix B Methods: The primary source of information are the Cardiac Network Service Advisors. Information is requested for the workforce on (at Dec 31 st 2013): Number of cardiologists Number of whole time equivalents (WTEs) Number working less than full time Number of Honorary Consultants Number of new appointments in 2013 Number involved in On-Call General Internal Medicine Number of vacancies at Dec 2013 And this year Numbers involved in On-Call Cardiology Number of Consultants planning retirement (>50% reduction in clinical activity) in next 3 years Where information is not forthcoming or incomplete, the survey team contact Trusts, individuals and any potential sources of the workforce data to acquire as comprehensive a data set as possible. Data collection continued through Jan to July 2014

(New question for this year) How many of the consultant cardiologists plan to retire /reduce by >50% clinical activity within the next 3 years? 66.8 (New question for this year)

4 Comments: 1. The survey shows continued expansion within cardiology (1.4% WTE), although at a slower rate than last year. This may be a correction for over counting in The increase since 2012 has been 235 for WTE and 18% for overall consultant numbers 2. The proportion doing acute GIM is falling (14% to 12.8 %) 3. 55% of consultants are involved in on-call rotas 4. The number of partial of complete retirements was asked for the first time. Comparison is therefore not possible but the figure aids workforce planning including determining appropriate trainee numbers Comments on data accuracy: a. The efforts by the BCS Honorary Secretary to refresh the team of Network Specialty Advisors was important in collating the data this year and providing a secure basis for data collection in future years. The Networks themselves have suffered in recent reorganisations There was just one Network where data from 2013 was the last available. b. The increase in WTE is much lower than between 2013 and I commented last year that I was concerned about some double counting and I think this is supported by this year data. Overall the longitudinal trend over the last 3 4 years is likely to be accurate. c. The figures for Wales have been reviewed and it is clear there was double counting in the last survey (97). This year s figure (72) and the one from 2 years ago (69) seem robust. Conclusions I. The survey of consultant workforce in 2014 suggests continues expansion at a steady rate II. There are currently 1379 cardiologists working in the UK III. Workforce planning, especially trainee numbers should be set in the context of these data together with additional factors including - the continuing rise in prevalence of patients with complex cardiac disease - the additional workload through development of 7/7 services (acute and elective) IV. It remains feasible to collate these data, focussing on the Network Specialty Advisors as the re-organisation of Cardiac Networks has impacted on the capability for undertaking the survey V. Continued undertaking of this survey is recommended

9 Appendix B RCP Census 2013 focus on Cardiology Speciality data This year the RCP census included specific questions for Consultant Cardiologists. 11 COMMENT It is clear that the majority of cardiologists are part of a team providing a 7 day service in support for the acute medical take. While this ensures prompt specialist input it puts considerable strain of cardiology teams and the other aspects of their work. It supports the view that cardiology teams see the most valuable impact they can have is by supporting the acute medical take rather than being general physicians themselves. This is relevant to trainees and their choice of dual certification, where they can be part of the medical take, or single certification where they support the acute take which seems the commoner pattern.
