3 OPTIMAL CARE AND NEW THERAPIES FOR PURPOSE: This CME program is designed for community cardiologists, primary care physicians, and other affiliated staff interested in the prevention and management of heart failure, heart transplant evaluation, post-transplant care and surgical options in heart failure. This symposium will enable physicians and practitioners to gain insight in to the latest diagnostic and therapeutic advances for patients with advanced cardiac disease. COURSE OBJECTIVES: At the conclusion of this program, participants should be able to: Appropriately stratify patients at risk by actively monitoring for risk factors and risk markers for advanced heart disease and employing early intervention strategies to reduce risk. Appropriately employ evidence-based clinical treatment guidelines for the treatment of advanced heart failure, including when to refer patients for interventional surgical procedures or transplant evaluation.
